Three weeks after radial forearm free flap phalloplasty, a 28-year-old female-to-male transsexual presented with ischaemia of the neophallus. A salvage procedure was started 7 hours after onset of ischemia. Thrombectomy of the arterial pedicle did not result in venous return of the arterial inflow. Perfusion of the flap with streptokinase restored venous return. Flap perfusion was confirmed by intraoperative direct angiography of the neophallus. Six months after this salvage procedure, the flap is doing well and tactile sensibility has occurred.

We have treated 50 patients (52 cases) of entrapment of the sensory branch of the radial nerve in the forearm (Wartenberg syndrome) between January 1988 and July 1992. Conservative treatment achieved 71% excellent and good results. Operation was followed by 74% excellent and good results. The indications for each type of treatment are discussed. De Quervain's disease was associated in 50% of cases, and it is important to diagnose Wartenberg's syndrome before operating on the tenosynovitis in order to avoid unexpected postoperative complications and medicolegal problems.

A patient was first seen with a flexion deformity of the thumb interphalangeal joint that was initially diagnosed and treated as traumatic in origin. A painful forearm soft tissue mass was ultimately discovered; biopsy of the flexor pollicis longus tendon showed metastatic adenocarcinoma. Extensive evaluation failed to locate the origin of the primary tumor. Treatment consisted of local radiation and systemic chemotherapy. After 1 year, the patient is alive and well, has no evidence of disease, and refuses correction of the thumb interphalangeal flexion contracture.

In an analysis of 29 cases of three different Monteggia-type fractures in children, the mechanism of injury was a hyperpronation force on the extended elbow. The follow-up period ranged from one to three years with a mean of 18 months. Closed reduction of these fractures was always difficult to maintain because the lesions are unstable. Closed reduction was successful in 25 of the 29 cases and open reduction was required in four. A slight residual dorsal angulation remained in 18 cases with fracture of both bones of the forearm, but healing in this position was acceptable.

The prevalence of lactase deficiency and the relationship between lactose and calcium malabsorption in postmenopausal osteoporosis has been assessed in 46 subjects. Malabsorption of lactose occurred in 25 (54%) of the subjects and was associated with a significantly lower milk intake. In a prospective, controlled study 58 patients aged under 60 years with Colles' fractures were treated either by a forearm plaster or by the application of an external fixator. In 94% of those treated by a fixator it was possible to insert the distal pins of the frame into the fracture fragment, the fixation obtained being sufficient to forgo additional splintage. The external fixator proved more effective at holding the manipulated position, and the radiological loss of position during fracture union was minimal compared with that seen in patients treated in plaster.

The places of origin of 7 out of 8 patients with CJD coincide with the distribution of sheep-rearing in central and southern Italy, confirming the suggested link between this disease and eating and-or handling sheep CNS tissue. Since 6 of the 8 cases were women, it seems more likely that the virus responsible for CJD enters the body through a break in the skin of the hands and forearms in the process of foodhandling rather than via the Digestive tract. This aspect should be borne in mind when investigating the problems of CJD transmission.

It is a specific clinical picture with extensive soft tissue gas and swelling of the forearm. Usually, the clinical course is without any other symptoms. Therapy is not necessary because gas disappears in few days. The etiology of the disease is not clear. Gas can be generated by the chemical reaction between a metallic foreign body and tissue fluid. Spread of benzine evaporation has been considered as a possible source as well. The disease is very rare and its knowledge is important in differential diagnosis of gas gangrene.

Two cases of rare desmoid fibromas are being reported, localised in the extremities. After a literature review of these tumors, the clinical characteristics are pointed out and difficulties in therapy discussed. The tumors occasionally appear on the forearm or the thigh of juvenile patients and show a similar development. In the first case, 7 operations with resection of the n. medianus and n. ulnaris were necessary to prevent a recurrence. In the second case, we saw a solitary and rapidly growing tumor, surrounding the sciatic nerve with a great tendency to recur.

Following traumatic retrosternal dislocation of the clavicle, the patient presented with thoracic outlet syndrome 6 months postinjury with swelling and cyanosis of the right upper extremity, and cramps of the right arm and forearm after heavy manual work. When open reduction of the dislocated clavicle failed, the medial half of the clavicle was resected. Four years postoperation, the patient is doing well, and is asymptomatic. A review of the literature, anatomy, mode of injury, mechanism of injury, pathology, clinical picture, diagnosis and treatment are discussed.
