[Outcome of mitral valve replacement in Iceland].

INTRODUCTION: Mitral valve replacement (MVR) is the second most common valvular replacement procedure after aortic valve replacement (AVR). Studies on the outcome of MVR in Iceland have been missing. We therefore studied short and long-term results following MVR in Iceland, MATERIAL AND METHODS: A retrospective nationwide study on 64 patients (mean age 59 years, 63% males) that underwent 66 MVR procedures in Iceland between 1990-2010. Clinical data was retrieved from patient charts and overall survival estimated. The mean follow-up was 7.4 years. RESULTS: Mitral regurgitation or stenosis was

Determination of flow profiles of different mechanical aortic valve prostheses using phase-contrast MRI.

AIM: After heart valve replacement in aortic position turbulences may occur in the aortic outflow tract. Valve-induced turbulences and retrograde flow represent a loss of efficiency of cardiac output in respect to antegrade kinetic energy of blood flow. Aim of this study is the evaluation of the flow profiles of monoleaflet and bileaflet valves after aortic valve replacement. METHODS: The flow profiles of three different mechanical aortic heart valve prostheses were compared using phase contrast magnetic resonance imaging (MRI) in vivo. One monoleaflet (group A, 30 patients) was compared with

Impact of device landing zone calcification patterns on paravalvular regurgitation after transcatheter aortic valve replacement with different next-generation devices.

OBJECTIVE: Residual paravalvular regurgitation (PVR) has been associated to adverse outcomes after transcatheter aortic valve replacement (TAVR). This study sought to evaluate the impact of device landing zone (DLZ) calcification on residual PVR after TAVR with different next-generation transcatheter heart valves. METHODS: 642 patients underwent TAVR with a SAPIEN 3 (S3; n=292), ACURATE neo (NEO; n=166), Evolut R (ER; n=132) or Lotus (n=52). Extent, location and asymmetry of DLZ calcification were assessed from contrast-enhanced CT imaging and correlated to PVR at discharge. RESULTS: PVR was

One-Year Outcomes of Novel Transcatheter Edge-to-Edge Mitral Repair System in Patients With Functional Mitral Regurgitation.

BACKGROUND: Transcatheter edge-to-edge repair (TEER) improves outcomes in patients with heart failure and moderate-to-severe functional mitral regurgitation (FMR) who remain symptomatic despite the use of maximal doses of guideline-directed medical therapy (GDMT). The SQ-Kyrin-M system is a novel TEER device designed for FMR treatment. AIMS: This multicenter, prospective, single-arm study (ClinicalTrials.gov number: NCT05988450) evaluates the safety and efficacy of the SQ-Kyrin-M system in severe symptomatic FMR. METHODS: A total of 125 eligible patients (mean age 65.5 ± 8.1 years) with hear

The use of imaging in new transcatheter interventions: an EACVI review paper.

Transcatheter therapies for the treatment of valve heart diseases have expanded dramatically over the last years. The new developments and improvements in devices and techniques, along with the increasing expertise of operators, have turned the catheter-based approaches for valvular disease into an established treatment option. Various imaging techniques are used during these procedures, but echocardiography plays an essential role during patient selection, intra-procedural monitoring, and post-procedure follow-up. The echocardiographic assessment of patients undergoing transcatheter intervent

Impact of patient-prosthesis mismatch following aortic valve replacement on short-term survival: a retrospective single center analysis of 632 consecutive patients with isolated stented biological aortic valve replacement.

OBJECTIVES: The impact of patient-prosthesis mismatch (PPM) after aortic valve replacement (AVR) on short-term and long-term mortality remains controversial. The objective of this study was to evaluate the incidence and severity of PPM and its impact on short-term survival in a large cohort of patients treated with isolated stented biological AVR in a single institution. METHODS: We analyzed retrospectively data of 632 consecutive patients with aortic stenosis undergoing isolated stented biological AVR between January 2007 and February 2012 at our institution. PPM was defined as an indexed ef

Hyperacute flash pulmonary oedema after transcatheter pulmonary valve implantation: The melody of an overwhelmed left ventricle.

Percutaneous transcatheter Melody Valve implantation has achieved standard of care for the management of certain patients with right ventricular outflow tract dysfunction. With its widespread use, some rare and potentially fatal complications, such as right ventricular outflow tract rupture and coronary artery compression, have been reported. We report hyperacute flash pulmonary oedema after Melody Valve implantation for the first time in two patients and describe some possible predictors.

Risk-adjusted survival after tissue versus mechanical aortic valve replacement: a 23-year assessment.

BACKGROUND AND AIM OF THE STUDY: Detailed analyses of risk-adjusted outcomes after mitral valve surgery have documented significant survival decrements with tissue valves at any age. Several recent studies of prosthetic aortic valve replacement (AVR) also have suggested a poorer performance of tissue valves, although analyses have been limited to small matched series. The study aim was to test the hypothesis that AVR with tissue valves is associated with a lower risk-adjusted survival, as compared to mechanical valves. METHODS: Between 1986 and 2009, primary isolated AVR, with or without coro

Reconstruction of the fibrous trigone.

A 48-year-old man with a history of infective endocarditis and severe aortic regurgitation had undergone prosthetic aortic valve replacement at another institution. Two months later, the patient developed prosthetic valve endocarditis with an aortic root abscess and an aorto-left atrial periprosthetic valvular fistula through the detached posterior annulus of the mitral valve. We repaired the fistula by constructing a fibrous trigone made of bovine pericardium. We also replaced the prosthetic aortic valve with another prosthetic valve, while protecting the native mitral valve.

Aortic root replacement with a mechanical valve conduit: A single-institutional experience.

BACKGROUND: There is a lack of consensus regarding the best strategy for aortic root replacement with current guidelines supporting the use of mechanical valves in younger patients when a valve repair is not possible. We sought to describe our institutional experience with mechanical aortic root replacement. METHODS: This was a descriptive study using an institutional database of aortic root replacements with a mechanical valve composite graft performed at our center from 2010 to 2022. All patients undergoing aortic root replacements with a mechanical valve were included. Patients undergoing

Oral, gut, and skin microbiota characterization in patients with heart valve disease with or without infective endocarditis: a pilot study.

The relationship between host microbiota and infective endocarditis (IE) has been investigated by few studies. We compared the oral, gut, and skin microbiota profiles of patients awaiting heart valve replacement, both with and without IE, and correlated the profiles with the patients' clinical data, including the etiological agents and antibiotics received. Thirty-six patients were enrolled in a prospective, observational pilot study: 25 with IE (cases) and 11 with non-infective valve disease requiring surgery (controls). Clinical data and oral, fecal, and skin samples were collected before su

The hemi-Yacoub procedure and valve repair in a patient with Turner syndrome and bicuspid aortic valve.

Bicuspid aortic valve is a common congenital anomaly often associated with aortic valve degeneration and ascending aorta dilation, leading to stenosis or insufficiency. We present the case of a 46-year-old female with Turner syndrome, a Sievers type 1 bicuspid aortic valve, moderate aortic insufficiency and a dilated aortic root. Given her age and anatomical features, a valve-sparing aortic root replacement using the hemi-Yacoub technique was performed. This technique preserved the patient's native aortic valve, thereby avoiding the need for anticoagulation therapy. Postoperatively, no residua

Outcomes After Isolated Aortic Valve Replacement in Patients with Bicuspid vs Tricuspid Aortic Valve.

To compare the outcomes in patients undergoing isolated aortic valve replacement with bicuspid (BAV) vs tricuspid aortic valves (TAV). A total of 1204 consecutive patients (BAV, n = 454; TAV, n = 750) underwent isolated aortic valve replacement (AVR) between 2002 and 2009 at a single institution. Adverse aortic events were defined as the occurrence of aortic dissection or the ascending aorta diameter greater than 50 mm or sudden death during follow-up. Propensity score matching yielded 318 patient pairs. Follow-up was obtained on 1156 patients. The mean length of follow up was 10.4 ± 3.9 years

[Surgical treatment of tricuspid regurgitation associated with left valvular heart disease: about 162 cases].

Tricuspid regurgitation (TR) is a prognostic factor associated with the outcome of patients undergoing surgical treatment of left valvular heart disease. The purpose of this study was to evaluate the postoperative outcome of patients with tricuspid regurgitation associated with left valvular heart disease and to identify factors associated with surgical failure. We conducted a retrospective study of 162 patients over the period January 2009 to July 2019. The study included all patients undergoing surgical treatment of TR and left valve repair. The average age of patients was 39.70 years ± 10.8

Modified reimplantation of bicuspid aortic valve.

We present a modified bicuspid aortic valve reimplantation operation using a unique technique of second line, continuous suturing during implantation of the aortic valve scallop to the prosthesis that mimics stentless, minicylinder implantation.  The feasibility of bicuspid aortic valve repair with associated aortic root management has been demonstrated. However, this repair seems to be less durable than tricuspid aortic valve repair, and this may be partly because of the connective tissue disorders that are an inherent feature of bicuspid aortic valve disease.  Annular progressive dilatation

Valvular heart disease: a primer for the clinical pharmacist.

Valvular heart disease is a commonly encountered clinical condition that is not taught in most undergraduate and graduate pharmacy programs, leaving the practicing pharmacist without basic knowledge to expand on and subsequently apply to direct patient care. Unlike other areas of cardiology in which thousands of patients are recruited in many well-designed randomized clinical trials, data assessing treatments for valvular heart disease are limited and often consist of retrospective case series or observations. Our goal is to provide a basic overview of chronic valvular heart disease, with emph
