[Prevention of osteoporosis and pathological calcifications].

Based on physiological data about the influence of magnesium compounds on the degree of saturation of blood plasma with calcium phosphates, we have hypothesized in 1988 that magnesium deficiency is the real cause of osteoporosis and pathological calcifications. The incidence of these diseases is among the elderly as high as 40%. These diseases are the reason that there is such a discrepancy between the morbidity and mortality curve for the elderly as a function of age. Meanwhile it has been proven that a. in all osteoporotic patients the intracellular magnesium content of the erythrocytes is l…

Beneficial Effects of Long-Lasting Bicarbonate-Sulfate-Calcium-Magnesium Water Intake on Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD)-Related Outcomes via Impacting Intestinal Permeability (IP), IP-Related Systemic Inflammation, and Oxidative Stress.

Background/Objectives: Fonte Essenziale®, a mineral water rich in bicarbonate, sulfate, calcium, and magnesium, has shown potential in modulating the gut-liver axis and microbiota in hepatic steatosis. However, its long-term effects on intestinal permeability (IP), systemic inflammation (SI), and oxidative stress-key factors in Metabolic dysfunction-associated steatotic liver disease (MASLD)-remain unexplored. Methods: Eighty-seven MASLD patients were randomized into two groups: group A received Fonte Essenziale® (400 mL/day, fasting) plus a controlled nutritional regimen for 12 months, follow…

Diuretic-Induced Hypomagnesemia May Play a Key Role in the Development of Calcium Pyrophosphate Arthritis.

INTRODUCTION: The objective of this study is to determine the prevalence of hypomagnesemia in patients with calcium pyrophosphate (CPP) arthritis and the role of diuretics on inducing hypomagnesemia. METHODS: This was an observational, cross-sectional, prospectively recruited, case-control study. Cases were confirmed by both chondrocalcinosis in plain X-ray and CPP crystals in synovial fluid. Serum creatinine, magnesium, calcium, phosphate, 25-OH-vitamin D, parathyroid hormone, ferritin and transferrin saturation were measured. Diuretic use, class, and dosage were also recorded. RESULTS: A t…

Pure silicate fragment in a recurrent stone former of calcium oxalate.

Long-time oral intake of magnesium-silicate-containing drugs is thought to be a causative factor inducing silicate urolithiasis. Besides, magnesium seems to play an anti-urolithogenic part in the formation of calcium oxalate stones. We report a recurrent calcium oxalate former who had been treated with magnesium aluminometasilicate antacid for gastric ulcer for approximately 17 years. One of the fragments found during extracorporeal shock wave lithotripsy was identified as 100% silicate. Deposition of silica was also found on other fragments. A large dose of magnesium (given in a part of the d…

Hypomagnesemia, Hypocalcemia, and Tubulointerstitial Nephropathy Caused by Claudin-16 Autoantibodies.

BACKGROUND: Chronic hypomagnesemia is commonly due to diarrhea, alcoholism, and drugs. More rarely, it is caused by genetic defects in the effectors of renal magnesium reabsorption. METHODS: In an adult patient with acquired severe hypomagnesemia, hypocalcemia, tubulointerstitial nephropathy, and rapidly progressing kidney injury, similarities between the patient's presentation and features of genetic disorders of renal magnesium transport prompted us to investigate whether the patient had an acquired autoimmune cause of renal magnesium wasting. To determine if the patient's condition might b…

Diet and adjuvant-induced arthritis in the rat.

Rats fed a high carbohydrate or a high protein diet deficient in copper, magnesium or zinc, or in the three trace elements together, failed to develop adjuvant-induced arthritis, yet a deficiency of both copper and magnesium in the diets exhibited no such inhibition. On the other hand, only a magnesium deficiency reduced the severity of the dextran anaphylactoid reaction.

Impaired consciousness due to hypermagnesemia associated with stercoral colitis: report of a rare case.

BACKGROUND: Hypermagnesemia is a rare electrolyte abnormality that is difficult to diagnose because its symptoms are nonspecific. In addition to magnesium administration, renal dysfunction is often a major risk factor associated with the condition; severe intestinal dysfunction is also a known risk factor. However, no cases of hypermagnesemia were observed in the absence of magnesium administration. CASE PRESENTATION: A 75-year-old woman with cognitive impairment presented to the emergency department with impaired consciousness. The patient was comatose and hypotensive and had a markedly dist…

Mechanical properties of bio-cementation materials in pre-precipitation mixing process.

Urease-producing bacteria (UPB) could be used to cement loose sand particles. The UPB would produce free ammonia and carbon dioxide during the process of hydrolyzing urea, and part of the free ammonia would be discharged into the air to cause certain pollution to the atmospheric environment. The carbon dioxide could react with alkaline oxide to form carbonates and improved the strength in GGBS comparing with medium containing different concentrations of urea. By adding hydrogen phosphate ions and magnesium salts, free ammonia could be converted into environmentally friendly magnesium ammonium …

Hypomagnesemia-induced cardiomyopathy.

Magnesium is the fourth most abundant cation in the body and the second most plentiful intracellularly. Magnesium is crucial to mitochondrial integrity, oxidative phosphorylation, protein synthesis, nucleic acid stability, membrane permeability, and neuro-muscular excitability. In addition, magnesium deficiency induces other electrolyte disturbances including hypocalcemia, hypokalemia, and hypophosphatemia. Because it is not routinely measured, many physicians fail to remember the significance of this element. Reported here is a patient with bulimia who presented with a magnesium deficiency wh…

[Effects of hypomagnesemia].

INTRODUCTION: The clinical consequences of plasmatic magnesium variations are underecognized in clinical practice. The dosage of plasmatic magnesium is underused and not reliable. Moreover, hypomagnesemia is often associated with other metabolic disorders (hypocalcemia, hypokaliemia), which are responsible for several symptoms. CASE REPORT: We report an 85-year-old man who presented with repeated bronchospasm, confusion, and abdominal and muscular pain, attributed to low magnesium serum level. We then review pathophysiology, various etiologies, clinical features, diagnostic challenge and trea…

Investigation of hypomagnesaemia prevalence and underlying aetiology in a hospitalised cohort of dogs with ionised hypocalcaemia.

OBJECTIVES: Calcium is the most abundant mineral in the body and plays a critical role in a wide range of physiological processes. Low concentrations of ionised calcium, the most metabolically available form of calcium, have been linked to an increased risk of adverse clinical outcomes in dogs. Magnesium plays an important role in parathyroid hormone function. The objective of this study was to define the prevalence and aetiology of hypomagnesaemia in a hospitalised cohort of dogs with ionised hypocalcaemia (IHC). METHODS: A total magnesium reference interval was established using serum bioch…

Hospital-acquired dysmagnesemia and mortality in critically ill patients: data from MIMIC-III database.

This study aimed to determine the relationship between hospital-acquired dysmagnesemia and in-hospital mortality in critically ill patients. A retrospective cohort study was conducted on critically ill patients who had normal serum magnesium levels on admission. Data were extracted from the Multiparameter Intelligent Monitoring in Intensive Care III database. The normal range of serum magnesium was 1.6-2.6 mg/dL. In-hospital serum magnesium levels were categorized based on the occurrence of hospital-acquired hypomagnesemia and hypermagnesemia. Hospital-acquired hypomagnesemia and hypermagnesem…

Dental phenotype in Jalili syndrome due to a c.1312 dupC homozygous mutation in the CNNM4 gene.

Jalili syndrome denotes a recessively inherited combination of an eye disease (cone-rod dystrophy) and a dental disorder (amelogenesis imperfecta), which is caused by mutations in the CNNM4 gene. Whereas the ophthalmic consequences of these mutations have been studied comprehensively, the dental phenotype has obtained less attention. A defective transport of magnesium ions by the photoreceptors of the retina is assumed to account for the progressive visual impairment. Since magnesium is also incorporated in the mineral of dental hard tissues, we hypothesized that magnesium concentrations in de…

[Behavior of selected bio-elements in women with osteoporosis].

The purpose of this study was to evaluate the concentration of calcium, magnesium, zinc and copper in serum, urine and hair in women with osteoporosis, and to find out whether deficiency of these bioelements correlates with BMD. The concentration of calcium, magnesium, zinc and copper was assessed in 80 women aged 40-68 years. The women had been menopausal for 9.3 years and had never undergone hormone replacement, drugs therapy or mineral supplementation. The bone mass density (BMD) in lumbar spine L2-L4 was measured in 80 postmenopausal women using dual energy X-ray absorptiometry. According …

De novo HNF1 homeobox B mutation as a cause for chronic, treatment-resistant hypomagnesaemia.

UNLABELLED: 29-year-old female presenting with an 8-year history of unexplained hypomagnesaemia, which was severe enough to warrant intermittent inpatient admission for intravenous magnesium. Urinary magnesium was inappropriately normal in the context of hypomagnesaemia indicating magnesium wasting. Ultrasound imaging demonstrated unilateral renal cysts and computed tomography of kidneys, ureters and bladder showed a bicornuate uterus. Referral to genetic services and subsequent testing revealed a de novo HNF1B deletion. LEARNING POINTS: HNF1B loss-of-function mutations are one of the most co…

Treatment of hypomagnesemia.

Serum magnesium concentration is determined by the interplay of intestinal absorption and renal excretion. Hypomagnesemia can occur as a result of insufficient magnesium intake, increased gastrointestinal or renal loss, or redistribution from extracellular to intracellular compartments. A number of drugs are known to cause hypomagnesemia, including proton pump inhibitors (PPIs). We report the case of a patient with symptomatic hypomagnesemia due to short bowel syndrome and PPI therapy. Investigations revealed low 24-hour urinary magnesium excretion and secondary hypocalcemia. PPI treatment was…
