BACKGROUND: The deltoid ligament is a key component of ankle fracture stability. Clinical tests to assess deltoid ligament injury have low specificity. In supination external-rotation (SER) type-IV ankle fractures, there is either a medial malleolus fracture or deltoid ligament injury. These injuries are often considered unstable, requiring surgical stabilisation. We look to identify the anatomical basis for this instability. This study investigates the anatomical basis for such instability by re-creating SER type ankle fractures in a standardised cadaveric study model, investigating the anato

BACKGROUND: Tibial tubercle avulsion fractures are rare, they represent less than 1% of all physeal fractures. Compared to monolateral tibial tubercle avulsion fractures, bilateral occurrence is even rarer. The purpose of this study is to report about the so far largest group of bilateral avulsion fractures and to compare them to unilateral fractures as well as to current literature.
METHOD: All patients who suffered from bilateral tibial tubercle fractures between January 2009 and March 2019 were included. All medical records and radiographs were reviewed and a clinical follow-up was perform

INTRODUCTION: The three-column concept has enhanced understanding of proximal tibial fracture morphology and surgical planning. However, its role as a prognostic indicator remains underexplored. This study evaluates whether column involvement predicts early functional outcome and complications.
MATERIALS AND METHODS: A prospective study of 30 patients with proximal tibial fractures treated with open reduction and internal fixation was conducted. Fractures were classified into single-, two-, and three-column injuries. Functional outcomes were assessed using the Modified Rasmussen score at foll

Subcondylar fracture of the mandible accounts for 25-35% of all mandibular fractures. In the past, most subcondylar fractures were managed non-surgically. The traditional method of fixation for subcondylar fractures uses two miniplates; however some bench studies have reported that trapezoidal plates are superior. The aim of this study was to compare the outcomes of subcondylar fractures fixed either with two non-parallel straight miniplates or with one trapezoidal plate. A randomized clinical trial was designed and implemented. Fifty-two consecutive patients with subcondylar fractures were re

Acromial and scapular spine fractures after reverse total shoulder arthroplasty occur predominantly as a result of bony insufficiency secondary to patient and intra-operative technical factors. The spectrum of the pathology can range from a stress reaction to an undisplaced or displaced fracture. Prompt diagnosis of these fractures requires a high suspicion in the postoperative patient with a clinical presentation of acute onset of pain along the acromion or scapular spine and/or deterioration of shoulder function. Conventional shoulder radiographs are frequently unreliable in identifying thes

CASE: We present the case of a 24-year-old woman who sustained a left midshaft clavicle fracture with acute subclavian artery compression, subclavian vein laceration, and complete brachial plexus palsy after a motor vehicle collision. The patient underwent urgent open reduction internal fixation of the clavicle and repair of the subclavian vein. Two years later, she underwent opponensplasty and flexor digitorum profundus tendon transfers. She regained satisfactory hand function, allowing her to return to full-duty work.
CONCLUSION: Clavicle fractures with associated acute neurovascular and br

A 43-year-old man with a 1-year history of bilateral hip pain, worsened by standing and walking, presented with a limping gait. x-rays showed diffuse osteoporosis (osteoblastic-lytic), and a bone scan revealed symmetric uptake in the costochondral junctions (rosary beads), femoral neck, proximal tibia, and feet, with no visualization of the kidneys, suggesting a metabolic disorder or neoplasm. Computed tomography revealed osteolytic lesions in multiple bones. Suspecting metabolic bone disease or metastasis, laboratory tests were conducted. Magnetic resonance imaging confirmed bilateral femoral

Anterior tibial tuberosity fracture avulsion is an uncommon injury. A concomitant patellar rupture is even more scarcely encountered. We report the case of a 14-year-old male patient who suffered bilateral anterior tuberosity fractures with concomitant bilateral complete patellar tendon rupture. Adolescence, athletic activity, and high BMI may have contributed to this concomitant bilateral injury. Both lesions were treated in a one-stage repair surgery, performing an open reduction and internal fixation of the tibial tuberosity with a cannulated cortical screw and a primary tendon suture follo

CASE: A 10-year-old, postmenarchal girl presented to the emergency department with a closed, displaced, intercondylar T-type distal humerus fracture. Open reduction and internal fixation was performed 3 days following initial presentation. The patient healed but experienced elbow stiffness in the 7 months following the procedure. Implant removal and capsular release were performed at that time. At the 31-month follow-up, the patient reported satisfactory elbow functionality.
CONCLUSION: There is limited literature available discussing optimal management and associated outcomes of intercondyla

Clavicular fractures are some of the most common bone injuries in the paediatric population, yet the rates of nonunion are very low under 18 years. To the best of our knowledge, posttraumatic nonunion of the clavicle in a paediatric population is rarely reported. We report the case of an 11-year-old girl who presented with a nondislocated fracture of the midshaft to the proximal third of the right clavicle. Initial conservative treatment by sling immobilization demonstrated radiologically confirmed healing at 3 months. However, at 1-year follow-up, she presented with painful nonunion. Diagnost

Radial head arthroplasty is indicated for displaced comminuted radial head fractures that cannot be managed reliably with open reduction and internal fixation and that have an associated elbow dislocation. Replacement also is indicated in patients with comminuted radial head fractures that have or are likely to have a disruption of the medial col-lateral, lateral collateral, or interosseous ligaments. Biomechanical studies have demonstrated that metallic implants restore elbow stability similar to the native radial head. The early and midterm clinical experience with metallic radial head arthr

Between February 1996 and July 2000 eight patients (ages 35-74 years) with nine severely impacted humeral head fractures were operated on with the aim of preserving the humeral heads. Patients were treated surgically with open reduction, autologous bone grafting, and internal fixation with a screw-wiring technique. There were no complications and all patients were satisfied with the results. After 10-63 months there was no evidence of nonunion, arthritis, avascular necrosis, or loosening of implants. The mean Constant score was 83 (66-97) points. Results of this study support the observation t

In non-articular fractures of the base of the base of the first metacarpal, this technique permits firm fixation, and immediate mobilization, with an excellent result. It consists of the insertion of two Kirschner wires, on either side, from the neck to the base of the first metacarpal, while reducing the fracture without opening. Two front and side view X-Rays control the correct reduction and position of the wires, which are cut under the skin closure. When making the two lateral incisions, it is very important to avoid injuring the two dorsal branches of the radial nerve. The Kirschner wire

Sternal fractures are uncommon but clinically relevant injuries, most often resulting from blunt thoracic trauma in motor vehicle collisions. While most isolated fractures can be treated conservatively, surgical stabilisation may be required in cases of displacement, instability, or polytrauma. Modern plating techniques offer biomechanical advantages over traditional wire fixation, but indications remain variably defined. We report the case of a 33-year-old male polytrauma patient involved in a high-impact motor vehicle accident. He sustained a displaced sternal fracture with manubriosternal d

Complex intra-articular injuries like proximal tibial fractures need validated patient-reported functional outcomes. Functional outcomes of 30 adults undergoing locking plate ORIF for closed proximal tibia fractures were assessed via KOOS at 1.5, 3 and 6 months postoperatively (September 2022-February 2024). The cohort consisted of middle-aged males (76.7%, mean age 44.33 ± 13.19 years) with the most common fracture patterns being Schatzker Type-5 (46.7%) and Type-6 (30%). Patients experienced bone union at 15.03 ± 1.61 weeks without complications. Overall KOOS scores improved significantly fr

BACKGROUND: Shoulder injuries caused by trauma are common, including clavicle fractures. Even so, bipolar segmental fracture of the clavicle is extremely rare and seldom reported. Therefore, there is still a controversial issue about how to treat these bipolar segmental clavicle fractures.
CASE SUMMARY: A 56-year-old security guard arrived at our emergency room after falling on his electric bicycle. There was no loss of consciousness or other pain. He had no numbness in his fingers. X-rays and 3D computed tomography revealed that the patient's right shoulder had a bipolar segmental clavicle f

Bilateral acetabular fractures following epileptic seizures are a rare but known occurrence in adults, with an 18.5% mortality rate. These fractures occurring post epileptic seizures have not been previously documented in children. We report a case of a 13-year-old boy who presented to hospital via ambulance following two violent generalised tonic-clonic seizures in a postictal state, metabolically acidotic and a low haemoglobin. Acute abdomen was suspected and the patient underwent a CT scan which showed bilateral acetabular fractures with central dislocations of both femoral heads and free f

PURPOSE: The aim of this study was to evaluate our treatment methods of proximal humeral physeal injuries retrospectively and elucidate the relationship between the trauma mechanism, the radiographic injury pattern, the consequent therapy and the functional outcome, and to further deduct and verify prognostic criteria.
METHODS: At our Department of Trauma Surgery, 303 children and adolescent patients with fractures of the proximal humeral epiphysis were treated from 1992 to 2009. 72 cases were diagnosed as physeal fractures according to the Salter-Harris classification and were included in ou

The results following nonoperative treatment of displaced, medial end clavicle fractures is often unsatisfactory; but no study has yet reported the outcome of operative fixation of these fractures. This study reports the results of open reduction and internal fixation on displaced, medial end clavicle fractures, in five adult patients (aged 25-52 years, mean 43) including 1 patient with a nonunion. The mean follow-up was 3.3 years (8 months-10.3 years). All fractures had united clinically and radiologically. No complications occurred, and no revision surgery was required. VAS pain scores avera
