[Perforator-based chimeric anterolateral thigh flap for head and neck reconstruction after en bloc resection].

PURPOSE: To discuss and report the operative techniques for harvesting perforator-based chimeric flap in anterolateral thigh region and the advantages for head and neck reconstruction after en bloc resection. METHODS: A retrospective review was performed of perforator-based chimeric anterolateral thigh (ALT) flap for head and neck reconstruction since December of 2007 to March of 2011. 66 perforator-based chimeric flaps were harvested including a skin paddle and a muscular flap supplied by one mother pedicle-descending branch of lateral circumflex femoral artery(d-LCFA). 32 flaps were used fo

A novel approach to the treatment of lower extremity lymphedema by transferring a vascularized submental lymph node flap to the ankle.

OBJECTIVE: Vascularized groin lymph node flaps have been successfully transferred to the wrist to treat postmastecomy upper limb lymphedema. This study investigated the anatomy, mechanism and outcome of a novel vascularized submental lymph node (VSLN) flap transfer for the treatment of lower limb lymphedema. METHODS: Bilateral regional submental flaps were dissected from three fresh adult cadavers for histological study. A unilateral submental flap was dissected in another six fresh cadavers after latex injection. The VSLN flap was transferred to the ankles of seven lower extremities in six p

Half forehead reconstruction with a single rotational scalp flap for dermatofibrosarcoma protuberans treatment.

BACKGROUND: Dermatofibrosarcoma protuberans (DFSP) is a soft tissue neoplasm of intermediate to low-grade malignancy. Although metastasis rarely occurs, DFSP has a locally aggressive behavior with a high recurrence rate. In the head and neck area, resection involving a wide margin of healthy tissue can be difficult because of functional and cosmetic considerations. We describe a novel reconstructive method for half forehead defects with an innovative single local wide scalp flap following excision of DFSP with a 3 cm margin of healthy tissue. METHODS: Two patients underwent wide resection of

[Regaining dignity: the experience and process of reconstructing normal defecation functions through an anal sphincter-saving operation].

BACKGROUND: Resection through an anal sphincter-saving (ASS) operation may affect a low rectal cancer patient'snormal defecation function. The long and complex reconstruction process following such a procedure makes understanding the patient rehabilitation experience necessary in order to develop an effective educational strategy that addresses patient needs. PURPOSE: This study investigated the defecation reconstruction experience of low rectal cancer patients after ASS. METHODS: Researchers used a case study method and purposive sampling. Participants were followed for a period of 6 to 8 m

Intraoral multistage mandibular angle ostectomy: 10 years' experience in mandibular contouring in Asians.

In Asia, a round face rather is more acceptable than a square face. Intraoral mandibular angle ostectomy is a common aesthetic procedure for correcting a prominent mandibular angle. However, an operation of sheer straight-lined prominent mandibular angle resection would sometimes create extramandibular angles or palpable bone steps in the margin of mandibular body and produce unnatural lower one-third facial appearance, especially for a square and disproportional mandibular angle even extending to the middle mandibular body. This article describes the method of multistage mandibular angle oste

Successful treatment of nonunion after sliding genioplasty.

Because chin is a major component of the lower face, underprojection, overprojection, and asymmetry of chin have been addressed via various surgical approaches. Sliding genioplasty is a popular technique to resolve this problem. This technique is rarely known to have postoperative complications, including nonunions. We experienced 1 case of nonunion after sliding genioplasty and treated it successfully.A 25-year-old woman who underwent sliding genioplasty 10 years ago at a local clinic visited our institute owing to displeasing appearance of the chin. Ten months ago, nonunion was diagnosed, an

Posterior pedicle lateral nasal wall flap: new reconstructive technique for large defects of the skull base.

BACKGROUND: Indications for expanded endoscopic approaches continue to grow, resulting in larger and more complex skull base defects. Reconstructive developments, however, have lagged our extirpative capabilities. As the complexity of clinical scenarios continues to escalate, challenging our current reconstructive strategies, we are compelled to develop alternative techniques to prevent cerebrospinal fluid leaks and protect neurovascular structures. In this article we show the anatomic basis for a new posterior pedicled flap from the lateral wall of the nose (Carrau-Hadad [C-H] flap) for the r

Esophageal lung: presentation, management, and review of literature.

Esophageal lung is an extremely rare type of bronchopulmonary foregut malformation where a main stem bronchus anomalously arises from the esophagus instead of the trachea. Less than 20 cases have been reported, and most of these are in association with esophageal atresia. Most cases are treated with detachment of the esophageal bronchus with repair of the esophagus and resection of the hypoplastic lung. Described herein are the presentation, diagnostic workup, and management of 2 such cases of esophageal lung.

Surgical aspects of a lower face, mandible, and tongue allotransplantation.

Face transplantation is a novel treatment for the reconstruction of massive facial defects. To date 13 cases have been performed. The technical aspects of a composite lower face transplantation including the tongue, floor of the mouth, and most of the mandible are detailed. The transplantation was performed in August 2009 in an HIV-positive, postoncologic patient. A preparatory surgery for nerve identification was performed. Facial composite tissue was procured after cardiac cessation. Revascularization was performed to the right subclavian artery with an internal shunt between the internal ca

The role of fabricated chimeric free flaps in reconstruction of devastating hand and forearm injuries.

Devastating hand and forearm injuries almost exclusively need free flap transfer if reconstruction is attempted. Early active and passive motion is only possible with aggressive, early, and comprehensive reconstruction. Despite recent advances in compound flaps, in selected cases it might be wise to harvest several smaller flaps and microsurgically combine them to one "chain-linked" flap "system." Four microsurgically fabricated chimeric free flaps were used in four patients for complex hand and forearm injuries. The combinations were sensate anterolateral thigh (ALT) flap plus sensate extende

Visual subunits of the hand: proposed guidelines for revision surgery after flap reconstruction of the traumatized hand.

The posttraumatic hand deformity remains a difficult challenge for reconstructive surgeons. Much has been written about the relative benefits of fasciocutaneous, fascial, and muscle flap reconstruction; however, the literature is devoid of guidelines for secondary surgery after flap reconstruction to improve the appearance of the hand. We introduce the concept of visual subunits of the hand in an effort to better guide restoration of natural hand form; thenar, hypothenar, midpalm, distal palm, dorsal hand, volar forearm, and dorsal forearm subunits are proposed. The visual subunit principle ha

Importance of the reconstruction of the extensor retinaculum after injury.

We reconstructed an extensive soft tissue defect of the dorsum of the left wrist after an injury using a free anterolateral thigh flap and a tendon graft, and intending to substitute the extensor retinaculum for the fascia in this flap. As a result, maximum function of the extension mechanism was restored to the reconstructed hand.

Umbilical cord inverting technique: a simple method to utilize the umbilical cord as a biologic dressing for sutureless gastroschisis closure.

A sutureless gastroschisis closure provides a cosmetically appealing outcome. The umbilical cord is usually used as a covering material in a sutureless closure because it is a native tissue. However, during the staged closure with a silo placement, special attention is required to keep the umbilical cord moist. The authors report a simple technique to preserve the feasibility of the umbilical cord as a biologic dressing during the silo placement in staged gastroschisis closures.

The homodigital neurovascular antegrade island flap for fingertip reconstruction in children.

We describe a homodigital neurovascular island flap for fingertip reconstruction in children and review the appearance and function of the reconstructed fingertips. Eleven children, with a mean age of 4 years (range: 2-7),who had sustained a fingertip injury with total or subtotal pulp loss were treated with a homodigital neurovascular island flap between 2007 and 2009. The flaps were harvested based on a single digital neurovascular bundle without further shortening of the distal phalanx. The average follow-up period was 15 months (range: 10-32 months). The clinical outcome evaluations includ

A 2-stage ear reconstruction for microtia.

OBJECTIVE: To introduce our 2-stage reconstruction of microtia method, which results in a natural-looking contour of the reconstructed ears, one of the most demanding challenges in facial plastic surgery. METHODS: In the first stage, the 3-dimensional cartilage framework is fabricated. The skin flap and retroauricular fascial flap are elevated in the mastoid area. Then the framework is wrapped by the fascial flap from behind and covered by the skin flap from front. In the second stage the crus, the tragus, and the conchal cavity are reconstructed. So almost all of the fine structures of ear a

Treatment of midline cleft of the mandible-A 2-stage approach.

Midline clefts of the lower lip and mandible are rare congenital anomalies. They may present as complete or incomplete in nature along with associated bifid tongue and ankyloglossia. Some may present with cleft of lower lip, cleft of mandible, fissured tongue, absent hyoid bone, cleft of manubrium sterni, or an extremely rare anomaly known as Tessier cleft no. 30. We present this unique case of midline cleft of the lower lip (incomplete) and mandible (complete) with initial soft-tissue surgery for the lower lip. The hard-tissue surgery for this case is deferred at a later date, and the reasons

A toenail flap based on the fibro-osseous hiatus branch for fingernail reconstruction.

This study included two parts: 1) cadaver dissection to elucidate the perfusion of toenail flaps by the fibro-osseous hiatus branch (FHB), and 2) clinical application of the toenail flap for reconstruction of a fingernail defect. Four second toes of two fresh Korean cadavers were dissected. The plantar digital artery (PDA) and terminal segment branch (TSB) were ligated, and red latex was injected distally into the ligated PDA. Perfusion of the dye into the toenail bed through the FHB was observed. From Oct 2004 to Sep 2009, eight toenail flaps based on the FHB pedicle with or without the dista
