Minimally invasive plate osteosynthesis for proximal humerus fractures: functional results of treatment.

Proximal humeral fractures can safely and effectively be treated with minimally-invasive plate osteosynthesis (MIPO). Twenty-one patients treated with MIPO for 2-, 3-, and 4-part proximal humerus fractures were treated at a mean 6.8 days (range, 1-24 days) after injury and followed for a mean of 24 months (range, 5-38 months). All fractures healed by 8 weeks postoperatively, with reductions "good" in 18 (86%) of patients and "fair" in 3 (14%). There were no infections or nerve or vascular injuries. One patient had loss of reduction that healed but required hardware removal. The neck-shaft angl

Complications of biceps tenodesis based on location, fixation, and indication: a review of 1526 shoulders.

BACKGROUND: Long head of the biceps tendon (LHBT) tenodesis is predominantly performed for 2 reasons: anterior shoulder pain (ASP) or structural reasons (partial tear, dislocation). METHODS: Between 2006 and 2014, all cases of primary LHBT tenodesis performed at an integrated health care system were retrospectively reviewed. Complications were analyzed by tenodesis location (below or out of the groove [OOG] vs leaving tendon in the groove [ITG]), fixation method (soft tissue vs implant), and indication (preoperative ASP vs structural). RESULTS: Among 1526 shoulders, persistent ASP did not di

Spinal Accessory Nerve Meningioma at the Foramen Magnum with Medullar Compression: A Case Report and Literature Review.

BACKGROUND: Meningiomas that arise from the cranial nerve are rare. We present a case with an intradural extramedullary tumor at the foramen magnum originating from the spinal accessory nerve. CASE DESCRIPTION: The patient was a 69-year-old woman with dizziness and pain in the bilateral shoulder for 2 years. Neurologic examination revealed spinal accessory nerve palsy (difficult in raising the shoulder, deficit of 3/5) on the left side without further deficits. Magnetic resonance imaging showed medullar compression because of a left intradural extramedullary foramen magnum lesion dorsolateral

Electrodiagnostic localization of traumatic upper trunk brachial plexopathy.

Eighteen patients having traumatic upper trunk brachial plexopathy ("the stinger"), a common football injury, were investigated electrodiagnostically. Proximal nerve conduction was determined by stimulating the supraclavicular fossa and at the C5 root and recording from muscles supplied by the long thoracic, suprascapular, musculocutaneous, axillary, lateral pectoral and thoracodorsal nerves. The accessory nerve was stimulated in the lateral posterior triangle and the evoked potential recorded from the upper trapezius muscle. Median and ulnar nerves were also tested, sensory and motor fibers b

Editorial Commentary: Arthroscopically Assisted Lower Trapezius Transfer: Promising Technique Still Searching for the Optimal Indications or Panacea for All Irreparable Rotator Cuff Tears?

Arthroscopic-assisted lower trapezius transfer (LTT) has gained in popularity as a technique to restore function and improve pain in large-to-massive irreparable posterosuperior rotator cuff tears. Surgeons who have adopted this technique have variable indications. While most agree that there is a limit of arthropathy, other factors appear to be contentious. Age, teres minor integrity, and degree of external rotation deficit remain debatable in terms of indications and contra-indications for LTT. The biomechanical narrative around LTT is sound and has been proven. Surgeons have recently raised

Biomechanical analysis of wheelchair athletes with paraplegia during cross-training exercises.

CONTEXT: Extreme conditioning programs (ECPs), such as CrossFit®, are a relatively new method of fitness with rapid growth in individuals with paraplegia. However, it is unknown if wheelchair users are at an additional risk of musculoskeletal injury during these exercises. Biomechanical characterization is necessary to determine the safety and efficacy of ECPs as an exercise modality for wheelchair users with paraplegia. OBJECTIVE: To characterize the three-dimensional (3-D) thorax and upper extremity joint kinematics of paraplegic wheelchair athletes during exercises commonly prescribed as p

Risk factors for degenerative, symptomatic rotator cuff tears: a case-control study.

BACKGROUND: Despite the considerable public health burden of rotator cuff tears, there is no consensus on risk factors associated with symptomatic rotator cuff tears. In this study, a large data source was used to identify factors associated with symptomatic rotator cuff tears. We defined cases of rotator cuff tears as those verified by imaging or operative reports and controls as symptomatic shoulders without rotator cuff tears as verified by imaging or operative reports. METHODS: We performed a case-control study of patients with and without symptomatic rotator cuff tears by use of the Vand

A genome-wide association study for shoulder impingement and rotator cuff disease.

BACKGROUND: The purpose of the study was to identify genetic variants associated with rotator cuff disease by performing a genome-wide association study (GWAS) for shoulder impingement using the UK Biobank (UKB) cohort and then combining the GWAS data with a prior GWAS for rotator cuff tears. The loci identified by the GWAS and meta-analysis were examined for changes in expression following rotator cuff tearing using RNA sequencing. METHODS: A GWAS was performed using data from UKB with 3864 cases of shoulder impingement. The summary statistics from shoulder impingement and a prior study on r

An anatomical study of the fetal superior capsule of the glenohumeral joint.

INTRODUCTION: The superior part of the glenohumeral joint capsule has an intimate relationship with the tendons of the rotator cuff and the tendon of the long head of the biceps. One of the strategies currently proposed in the event of a massive cuff rupture is to reconstruct this superior capsule. The main objective of this anatomical study was to describe the superior joint capsule of the embryonic glenohumeral joint and its relationship to the tendons of the rotator cuff. HYPOTHESIS: The hypothesis was that this structure was an anatomical entity, morphologically identifiable from the embr

Diagnostic Accuracy of Musculoskeletal Ultrasound on Long Head Biceps Tendon Pathologies.

Clinical Scenario: Pathologies of the long head of the biceps brachii (LHB) tendon are a source of shoulder pain in many people. It is important to have a reliable assessment of the LHB tendon to make an accurate diagnosis and provide the correct treatment or referral if necessary. Shoulder ultrasound is very accurate in the diagnosis of rotator cuff tears. However, its ability to detect pathologies of the LHB tendon is still unclear. Clinical Question: In patients with shoulder pain, can musculoskeletal ultrasound accurately diagnose LHB tendon pathologies? Summary of Key Findings: Four high-
