Subgroups of older adults with osteoarthritis based upon differing comorbid symptom presentations and potential underlying pain mechanisms.

INTRODUCTION: Although people with knee and hip osteoarthritis (OA) seek treatment because of pain, many of these individuals have commonly co-occurring symptoms (for example, fatigue, sleep problems, mood disorders). The purpose of this study was to characterize adults with OA by identifying subgroups with the above comorbid symptoms along with illness burden (a composite measure of somatic symptoms) to begin to examine whether subsets may have differing underlying pain mechanisms. METHODS: Community-living older adults with symptomatic knee and hip OA (n = 129) participated (68% with knee O

Circadian biomarkers, circadian genes and bipolar disorders.

Bipolar disorders are associated with circadian deregulations both during acute mood episodes and during euthymic periods, suggesting that these circadian rhythms may represent trait markers of the disease. Several arguments demonstrate that deregulations of circadian rhythms may be part of the pathophysiology of bipolar disorders. Abnormal quantitative and qualitative circadian disturbances have been repeatedly showed in bipolar patients, both during euthymic periods and acute phases, using different assessment tools such as actigraphy, polysomnography, and blood melatonin monitoring. [Diagnosis of Asperger syndrome in infancy and childhood].

I discussed, in this chapter, the possibilities and limitations of clinical diagnosis for Asperger syndrome (AS) in infancy and early childhood. For early diagnosis or simply noticing the possibility of AS, clinicians should be sensitive to the auxiliary conditions of AS such as motor dysfunction and sensory/sleep disturbances during the early stage of life, as well as the incipient representations of the main AS symptoms in the diagnostic criteria, such as lack of reciprocal social interaction or joint attention. Future prospective study with large cohort, or creation of the new methods for c

Sleep disturbances following mild traumatic brain injury in childhood.

OBJECTIVE: To examine objective and subjective reports of sleep disturbance in school-aged children who had sustained mild traumatic brain injury (TBI) at least 6 months prior to the study. METHODS: Eighteen children aged 7-12 years with a history of mild TBI (GCS 13-15. LOC < 15 min) were compared to 30 children with orthopedic injuries using actigraphy and parental and self-report sleep questionnaires. RESULTS: Parents reported greater sleep disturbance in the mild TBI group. No significant differences were found in parental ratings of daytime sleepiness, child-reported sleep difficulties,

Optimization of levodopa therapy.

While there is no single correct starting dose for levodopa therapy, many individuals can be started on either the 25/100 or controlled-release formula, following the general rule not to attempt to titrate carbidopa-levodopa to the point of "normality," which can lead to toxicity. The physician should also determine the proper use of any adjunctive medications; such combined therapy has become the standard approach to treatment. Following the initial period of therapy, emerging difficulties require a reassessment of therapeutic approaches, such as dosage adjustment or introduction of a dopamin

Prominent somnolence and cerebellar syndrome in subclavian artery thrombosis--a case report.

A patient suffering from acute left subclavian artery thrombosis presented with an unusual syndrome of prominent somnolence, right cerebellar signs, and left upper extremity ischemia. Clinical, angiologic, and electrophysiologic studies confirmed a brain stem and reticular formation dysfunction due to vertebrobasilar ischemia. Urgent thromboembolectomy led to neurologic improvement and satisfactory revascularization of the involved upper limb.

Long-term neurological implications of somnolence syndrome in children with acute lymphocytic leukemia.

A longitudinal study of 49 children with acute lymphocytic leukemia (ALL) assessed the long-term effects of central nervous system (CNS) prophylaxis on brain function. From 10 to 12 electroencephalograms (EEGs) were done before and at intervals during after 30 months of treatment that included 2,400 rads of CNS irradiation plus intrathecal methotrexate therapy. None of the children had CNS leukemia, and all remained in first complete remissions. All 49 had abnormally slow EEG background frequencies during the four-year study, and 29 (60%) developed somnolence syndrome six to eight weeks after

Polygraphic study of periodic breathing and hypersomnolence in a patient with severe hypothyroidism.

A 67-year-old woman with primary hypothyroidism, who had Pickwickian syndrome as well, was studied electrophysiologically. A polygraphic study with simultaneous recordings of electroencephalography, actograms of the abdominal walls, the mentum and the anterior neck, and electromyogram of the intercostal muscles revealed a cyclic appearance of apnea in the sleep phase and gasping preceding arousal, which, together with macroglossia and sleep in the sitting position, suggested a cyclic obstruction of the upper airway. The obstruction appeared responsible for both alveolar hypoventilation and dis

Sleep Disorders.

Sleep disorders are frequent and can have serious consequences on patients' health and quality of life. While some sleep disorders are more challenging to treat, most can be easily managed with adequate interventions. We review the main diagnostic features of 6 major sleep disorders (insomnia, circadian rhythm disorders, sleep-disordered breathing, hypersomnia/narcolepsy, parasomnias, and restless legs syndrome/periodic limb movement disorder) to aid medical practitioners in screening and treating sleep disorders as part of clinical practice.

Adverse childhood experiences and risk of chronic sleep disturbances: A population-based cohort study from a Chinese national survey.

BACKGROUND: Adverse childhood experiences (ACEs) have long-lasting effects in Chinese older adults. Using China Health and Retirement Longitudinal Study (CHARLS) data, we examined ACEs' impact on chronic sleep disturbances combining poor sleep quality and abnormal duration in China's aging population. METHODS: We analyzed 20,450 CHARLS participants (2014 baseline). After excluding those with missing ACEs data (n = 11,006), incomplete sleep data, or aged <45 years and with psychiatric conditions, the final sample comprised 5,533 individuals. ACE exposure was categorized into: Class 1 (0 ACEs,

Childhood sleep behaviors and polycystic ovary syndrome risk in adolescence: the Raine study cohort longitudinal study.

BACKGROUND: Sleep disturbances are common in women with polycystic ovary syndrome (PCOS), but no prospective studies have explored whether childhood-adolescence sleep disturbances are associated with later PCOS risk. Understanding behavioral markers may support early identification and intervention. OBJECTIVE: To examine whether poor sleep behaviors from childhood to adolescence are prospectively associated with a diagnosis of PCOS at age 14 years. MATERIALS AND METHODS: Sleep behavior data (Child Behavior Checklist) from a longitudinal study of 226 female participants of the Raine Study coh

Longitudinal association of self-reported restless sleep and duration with motoric cognitive risk syndrome in Chinese older adults: Evidence from CHARLS.

BACKGROUND: Motoric cognitive risk syndrome is an established risk factor for dementia in older adults. Although self-reported sleep complaints and sleep duration have been cross-sectionally associated with MCR, prospective evidence remains limited. METHODS: Data were obtained from the 2011, 2013, and 2015 waves of the China Health and Retirement Longitudinal Study. We included older adults aged 60 and above. Restless sleep was assessed using a single self-reported item and categorized as rare (≤2 days/week), occasional (3-4 days/week), and frequent (5-7 days/week) restless sleep. Self-report

Exploring Breast Cancer Survivors' Preferences for Text Messaging-Based Mobile Health Interventions Targeting Sleep and Physical Activity: Qualitative Study.

BACKGROUND: Sleep disturbances and low physical activity are common among breast cancer (BC) survivors and are associated with increased morbidity and mortality. Given the increased access to technological devices and the growing popularity of SMS text messaging-based mobile health interventions, these tools have the potential to both address sleep disturbances and promote physical activity in a scalable and cost-effective way. To understand and make effective use of these tools, it is important to consider the preferences of BC survivors with sleep disturbances, including how SMS text messagi
