Twice Daily Prazosin and Valproic Acid in the Treatment of Flashbacks in PTSD.

Posttraumatic stress disorder (PTSD) is a debilitating stress disorder occurring in the context of a traumatic event and is characterized by intrusive and avoidance symptoms, negative alterations in cognition and mood, and arousal and reactivity changes. Despite its representation throughout literature, the pathophysiology of PTSD remains incompletely understood, thus contributing to broad, variable, and at times, experimental treatment options. The authors present the first documented case of the rapid and successful management of PTSD using valproic acid and twice daily dosing of prazosin ai

Nexus of despair: A network analysis of suicidal ideation among veterans.

The objective of this study was to estimate a network model of risk and resilience factors of suicidal ideation among veterans. Two network models of suicidal ideation among Operation Iraqi Freedom/Operation Enduring Freedom/Operation New Dawn veterans (N = 276) incorporated key disorders, traumatic stress, and resilience constructs to contextualize suicidal ideation. Childhood trauma was positively connected with suicidal ideation and harassment and inversely connected with social support and distress tolerance. This exemplifies long-lasting associations between childhood trauma and re-victim

Diagnostic classification of fear of childbirth: why specific phobia may not be enough.

BACKGROUND: Fear of childbirth (FoB) is a common experience during pregnancy which can cause clinically significant distress and impairment. To date, a number of investigations of FoB have assumed that clinically significant FoB is best understood as a type of specific phobia. However, preliminary evidence suggests that specific phobia may not be the only diagnostic category under which clinically significant symptoms of FoB are best described. AIM: The current study is the first to investigate which DSM-5 diagnostic categories best describe clinically significant symptoms of FoB. METHOD: Pr

Associations Between Lifetime Panic Attacks, Posttraumatic Stress Disorder, and Substance Use Disorders in a Nationally Representative Sample.

Objective: Rates of lifetime substance use disorder (SUD) are high among people with lifetime posttraumatic stress disorder (PTSD). Panic attacks are also prevalent among trauma survivors and people with SUD, yet studies on PTSD/SUD have rarely examined comorbid panic. This potentially creates additional barriers to effective treatment for people with PTSD/SUD, in that panic may be under-diagnosed among people with PTSD/SUD and consequently attenuate treatment outcome. Additionally, research on PTSD/SUD often combines people with alcohol use disorder (AUD) and people with drug use disorders (D

Obsessive-compulsive disorder has a reduced placebo (and antidepressant) response compared to other anxiety disorders: A meta-analysis.

BACKGROUND: Previous studies have indicated that obsessive-compulsive disorder (OCD) might have a reduced placebo response compared to other anxiety-related disorders including generalized anxiety disorder, panic disorder, post-traumatic stress disorder, and social anxiety disorder. No previous analysis has directly compared antidepressant and placebo responses between OCD and these conditions. METHOD: We analyzed pre-post change scores within drug and placebo groups as well as between-groups change scores (i.e., drug compared to placebo) for all FDA-approved antidepressants for the treatment

Nicotine dependence mediates the relations between insomnia and both panic and posttraumatic stress disorder in the NCS-R sample.

Although a growing literature has demonstrated elevations in insomnia symptoms among persons with either panic disorder (PD) or posttraumatic stress disorder (PTSD) relative to people without psychopathology, comparably little is known about processes underlying these associations. In recognition of this important gap in the literature, this study tested nicotine dependence as a partial mediator of the relations between insomnia symptoms and both PD and PTSD among a nationally representative sample of 5,692 (3,311 females; M(age)=43.33, SD=16.55) adults from the National Comorbidity Survey-Rep

Paradoxical and less paradoxical effects of thought suppression: a critical review.

The process of consciously trying to avoid certain thoughts is referred to as thought suppression. Experimental research has documented that thought suppression may have paradoxical effects in that it leads to an increased frequency of the to-be-suppressed thought intruding consciousness. It has also been claimed that suppression has disruptive effects on episodic memory (i.e., a less paradoxical effect). The present article critically evaluates studies on the paradoxical and less paradoxical effects of thought suppression. More specifically, the issue of whether thought suppression plays a ca

Mental health outcomes following emergent versus elective colorectal surgery.

INTRODUCTION: Emergency surgery carries risk for psychological adverse events, yet procedure-specific mental-health outcomes remain poorly characterized. This study evaluated the association between surgical acuity (emergent vs elective), ostomy formation and new-onset postoperative mental-health conditions following colorectal surgery. METHODS: A retrospective cohort study of patients undergoing emergent or elective colorectal surgery from 2015 to 2025 in the TriNetX research-network was performed. Patients with pre-existing mental-health diagnoses were excluded. Analyses were stratified a p

Placebo response in trials with patients with anxiety, obsessive-compulsive and stress disorders across the lifespan: a three-level meta-analysis.

QUESTION: Randomised controlled trials assessing treatments for anxiety, obsessive-compulsive and stress-related disorders often present high placebo response rates in placebo groups. Understanding the placebo response is essential in accurately estimating the benefits of pharmacological agents; nevertheless, no studies have evaluated the placebo response across these disorders using a lifespan approach. STUDY SELECTION AND ANALYSIS: We searched MEDLINE, PsycINFO, Embase, Cochrane, websites of regulatory agencies and international registers from inception to 9 September 2022. The primary outc

The Effect of Wildfire Exposure: Neurological Outcomes, Mental Health, and Epigenetic Insights.

Background/Objectives: Wildfires are increasing in frequency and intensity worldwide, leading to widespread exposure to wildfire smoke and associated environmental stressors. While the respiratory and cardiovascular effects of wildfire smoke are well established, the potential neurological and mental health consequences have received growing attention. This narrative review synthesizes evidence from animal and human studies examining the effects of wildfire exposure on neurological function, behavior, and mental health, and explores the potential role of epigenetic mechanisms. Methods: A struc

Structure and correlates of the Moral Injury Exposure and Symptom Scale for Civilians (MIESS-C) in a community sample with high trauma exposure.

Background: Moral injury (MI) is a clinical phenomenon that can emerge in the aftermath of engaging in or witnessing an event that transgresses an individual's deeply held moral beliefs. Research on MI assessments has yielded conflicting results regarding optimal structure, and there are very few comprehensive examinations of the clinical correlates of MI in diverse community samples. Objective: We examined psychometric properties of the Moral Injury Exposure and Symptoms Scale for Civilians (MIESS-C) and investigated clinical correlates of this measure. When Trauma Crosses Generations: Mechanisms, Clinical Patterns and Therapeutic Implications of Transgenerational Trauma-A Systematic Review.

BACKGROUND: Transgenerational trauma (TT)/Intergenerational trauma (IT) is the transmission of the effects of traumatic experiences of parents to their children, who have not themselves experienced traumatic events. This transmission occurs through neurobiological and metabolic changes and the environment in which they were raised. METHODS: A systematic review was conducted in accordance with the PRISMA 2020 guidelines. PubMed and Google Scholar databases were searched from 2005 to 2025. Studies focusing on adult offspring, exposure to ancestral trauma, biological markers (DNA methylation, co

Trauma in Young Children and Their Families.

The early period of life form pregnancy to preschool is often overlooked in discussions of the impact of trauma. Early childhood is a period of high sensitivity to experience, both positive and negative. Young children may be at higher risk of experiencing potentially traumatic events (PTEs) AND the possibility- but not destiny- of long-term negative implications. This article describes patterns of exposure to PTEs, the associated clinical presentations, and the opportunities for health care systems to deliver effective, trauma-informed care and protect against potential secondary traumatizati

The Anesthesiologist's Role in Trauma-Informed Care: An Update.

Trauma-informed care continues to follow the 4R principles (realize the widespread nature of trauma, recognize what unresolved trauma looks like, respond by changing practices and protocols, and resist retraumatization) but has expanded to include 6 core guiding principles that include safety, trust, peer support, mutuality, empowerment, and the understanding of an individual's cultural context. Additionally, it is now recommended that trauma-informed practices be offered as universal precautions to all individuals, regardless of trauma history, focusing on trauma prevention. [The use of psychedelics in psychiatry].

Over the past five years, the literature on the psychiatric use of psychedelic and related treatments has expanded substantially. Several randomized controlled trials and pooled analyses have been conducted in mood disorders, treatment-resistant depression, and post-traumatic stress disorder. Trial methodology, however, remains a fundamental source of uncertainty, and despite the popularity of the field, the clinical evidence is still weak. Psilocybin produces consistent and often large improvements in major depressive disorder, yet in the most rigorous trials, the predefined primary endpoint

Management of trauma-related nightmares in PTSD: Can doxazosin serve as a pragmatic alternative to prazosin?

OBJECTIVES: Trauma-related nightmares (TRNs), core features of post-traumatic stress disorder (PTSD), partially or fully replicate the traumatic event and are often associated with nocturnal awakenings and significant emotional distress. In light of the imminent withdrawal of prazosin in France, this review explores the potential of doxazosin as a pharmacological alternative while emphasizing the central role of psychotherapeutic interventions in TRNs management. METHODS: Following a brief overview of TRNs and existing therapeutic strategies, both pharmacological and non-pharmacological, this

Mental health of women with HIV: a qualitative meta-synthesis.

BACKGROUND: Women living with human immunodeficiency virus (HIV) experience significant psychological burdens due to biological and gendered factors such as pregnancy, childbirth and breastfeeding. However, a life course-based understanding of their mental health, adaptation and recovery remains limited. This study aimed to synthesise and analyse qualitative research on the mental health experiences of women living with HIV. METHODS: A qualitative meta-synthesis was conducted following Sandelowski and Barroso's methodology. A comprehensive search of six electronic databases employed keywords
