[Pharyngocele].

Pharyngocele is a lateral pharyngeal wall herniation located in the pyriform recess or at the vallecula. It is a rare condition and the patient complains of dysphagia, lateral cervical mass which increases in size with the Valsalva maneuver, and other symptoms of the upper digestive way. The elective diagnostic study is pharyngoesophageal swallow. We present the case report of a patient who had acute respiratory failure due to supraglottic mass occupation. The explorations are discussed and the case report is compared with those few reported in the literature.

[Longitudinal radiologic study of the esophagus, in an endemic area of Chagas disease, in a period of 13 years].

A radiological study of the oesophagus of a cohort of patients was carried during a 13 year period in the municipality of Mambaí Goiás. Barium swallow findings were recorded on 70mm film using a portable machine. Of 731 patients examined 382 (52.3%) were seropositive for T. cruzi. The sexes were equally divided. The incidence of detectable megaoesophagus was 7.9% among the cohort and 14.2% in the seropositive individuals. Progression of the disease was noted during this longitudinal study in 21.7% of males and 16.6% of females.

Kinematic pharyngeal transit times in myopathy: evaluation for dysphagia.

Measurement of kinematic pharyngeal transit times, a new videofluoroscopy technique, provides useful quantitative data to supplement the qualitative data previously available from videofluoroscopy swallowing studies. Kinematic pharyngeal transit times have not previously been reported for subjects with myopathy. This study demonstrates the use of quantitative kinematic pharyngeal transit times for dysphagia evaluation in 15 patients with myopathy. The successful treatment of dysphagia by cricopharyngeal myotomy is reported in two patients with limb-girdle syndrome.

[Boerhaave syndrome in pyloric hyperplasia].

Boerhaave's-syndrome is a rare illness, usually caused when sudden intense vomiting leads to a spontaneous rupture of the healthy esophagus. Alcoholism and neurological diseases are considered to be the main predisposing factors. Because the complaint is so rare the casuistic of Boerhaave's-syndrome is described in connection with pyloric stenosis. In order to confirm the diagnosis the following practice is recommended: During gastrografin swallowing the pylorus should be examined in order to remedy the possible cause of the complaint when the operative therapy is carried out.

[Nd:YAG laser treatment of cancer of the esophagus and cardia].

In 1988, Nd:YAG laser treatment was employed as palliative therapy for 23 patients with cancer of the oesophagus and cardia. After treatment, half of the patients could ingest light or normal diets and the ability to swallow improved in a total of 3/4 of the patients. Perforation of the oesophagus occurred in two patients and aspiration in two other patients. It is concluded that, in a great number of patients with inoperable cancer of the oesophagus and cardia, laser treatment is better than other palliative therapeutic measures. The indications are discussed.

Tetanus: an uncommon cause of dysphagia.

A 53-year-old woman was examined at our medical center because of progressive dysphagia of 14 days' duration and a severe inability to open her mouth and swallow saliva. A barium esophagogram showed no obstruction, but pooling of barium in the hypopharynx suggested a neuromuscular disorder. The clinical diagnosis of tetanus was confirmed by electromyography. With appropriate therapy, the patient recovered during a period of 6 weeks. This case illustrates both an uncommon cause of dysphagia and an uncommon initial manifestation of tetanus.

A study of sustained action pseudoephedrine in allergic rhinitis.

A single capsule of sustained release pseudoephedrine (SUDAFED S.A., Burroughs Wellcome Co.) was shown by objective and subjective measurements to be superior to placebo in relieving nasal congestion associated with allergic rhinitis. The drug had no discernible effect on (1) the degree of wetness perceived in the mouth or nose, (2) a complex of symptoms which included sneezing, coughing, sniffing, swallowing, itching of eyes and nose or (3) number of nose blows. The study was marked by an absence of serious adverse reactions.

Calcified gastric leiomyoma--a case report.

A 62 year-old man was admitted with the chief complaint of a epigastric pain and difficulty of swallowing. Examination of the upper gastrointestinal tract revealed a calcified mass causing a smooth filling defect at the lesser curvature of the cardia. At laparotomy, a localized tumor was detected beneath the serosa of the stomach. The excised tumor was 4 x 4 x 1.6 cm and the pathological diagnosis was calcified leiomyoma probably due to cytoplasmic degeneration. In this paper, details of the case are described and the related literature is reviewed.

Pharyngolaryngectomy, total extrathoracic esophagectomy and gastric transposition.

Total pharyngolaryngectomy, extrathoracic esophagectomy and immediate reconstruction using transposed stomach were performed without operative mortality in 16 patients using a two team approach. The intact transposed stomach appears to be a highly reliable esophageal substitute. Significant palliation was achieved since all were able to swallow by mouth until their death. Results in terms of survival were disappointing but, possibly, may be improved by better patient selection and the judicious use of adjuvant chemotherapy and radiation.

Fibreoptic endoscopic palliative intubation of inoperable oesophagogastric neoplasms.

Palliative intubation for inoperable malignant strictures at the cardia was done on 16 occasions in 13 patients using fiberoptic endoscopy. Preliminary dilatation was performed, and the Celestin tube was mounted on an introducer and passed over a guide wire inserted with a fiberoptic endoscope. Only one death resulted from the procedure and all the 12 patients who left hospital were swallowing satisfactorily on discharge. The method provided a simple and relatively safe means of relieving dysphagia and improving nutrition.

The G syndrome. A four-generation family study.

A male infant with hypertelorism, hypospadias, swallowing difficulties with tendency to regurgitation and cough, high arched palate, and a delicate voice, consistent with the G syndrome, is reported. In the family the same symptoms in addition to cleft lip and palate were known in several family members through four generations. In the females only slight manifestations of the syndrome were found, and in the males variable expression of symptoms was observed. Autosomal dominant inheritance is likely, but X-linked inheritance cannot be ruled out.

Pediatric Dysphagia: Through The Thick and Thin.

The article provides an overview of dysphagia in children, emphasizing the complexities of the swallowing process and its disruptions, which can lead to oropharyngeal or esophageal dysphagia. It discusses the overlapping clinical symptoms in young children and highlights the importance of comprehensive gastroenterological evaluations, including imaging and endoscopic techniques. The article also stresses the necessity of a multidisciplinary aerodigestive approach, especially for complex cases, and underscores the need for age-appropriate diagnostic and therapeutic strategies to effectively man

Foreign bodies in the esophagus in children and adults - a five-year experience.

Incidents of foreign body ingestion and entrapment in the esophagus are a common pathology that brings patients to emergency departments. The main affected groups are children, adults over 80 years old and people with mental problems as well as patients with an anatomical predisposition to retain larger bites of food in the esophagus - conditions after surgical interventions on the esophagus, after either unintentional swallowing acids and bases or suicide attempt, growing mass causing narrowing of the esophagus in a sudden area and others.

Fixed drug combination (levosulpiride and rabeprazole)-induced atypical Parkinsonian's disorders with associated anxiety and low-lying depression.

We hereby describe a rare case of levosulpiride-induced atypical parkinsonism presenting with sluggish movements, atypical kinetic tremors (tremors with voluntary movement), periorbital tremors, dystonia, difficulty in speech and coordination, postural imbalance, with additional features of difficulty in swallowing and drooling with associated recent onset psychiatric disturbances such as anxiety and low-lying depression. The dechallenge of levosulpiride and medications for associated anxiety and low-lying depression caused a complete remission of the disease within 2 ½ months.

[Multiple factors contributing to dysphagia : Extended tasks for the interprofessional team in the intensive care unit].

A stay in the intensive care unit (ICU) can be associated with complications and lead to secondary problems, known as post intensive care syndrome (PICS). They can prolong inpatient stays, increase ICU mortality and lead to long-term problems. One secondarily acquired problem in intensive care is dysphagia. This article highlights various influencing factors on dysphagia. Some require an extended interprofessional treatment concept that goes beyond strictly speech and swallowing therapist treatment.

Magnetic Appendix: An Uncommon Indication for Appendectomy.

Foreign object ingestions are a common occurrence in pediatrics, often necessitating endoscopic or surgical intervention. The ingestion of multiple magnets poses an increased risk for serious complications. Our article presents a case of a five-year-old boy who swallowed two pennies and four magnets. The latter failed to pass spontaneously and were lodged in the appendiceal orifice resulting in a challenging and unsuccessful endoscopic retrieval and hence required laparoscopic exploration, appendectomy, and partial cecal resection.

Esophageal Achalasia: Diagnostic Evaluation.

A precise diagnosis is key to the successful treatment of achalasia. Barium swallow, upper endoscopy and high-resolution manometry provide the necessary information about a patient's anatomy, absence of other diseases, and type of achalasia (I, II, III). High-resolution manometry also has prognostic value, the best results of treatment being obtained in type II achalasia according to the Chicago classification. Abdominal CT scanning and endoscopic ultrasound might be warranted if an underlying malignancy is suspected.

Dysphagia in the Older Patient.

Dysphagia in older adults is a challenging problem and necessitates a team approach. The key to effective management is recognition. Patients tend to dismiss their symptoms as normal aging; therefore, early diagnosis depends on the diligence of the primary care doctors. No diagnostic technique can replace the benefits of a thorough history, with a detailed understanding of nutritional status and aspiration risk. Although one of the main goals in management is to ensure safe swallowing, the impact of a nonoral diet on the quality of life of patients should not be underestimated.
