Results of accelerated postoperative rehabilitation using novel "suture frame" repair of Achilles tendon rupture.

The management of Achilles tendon rupture is a much-debated subject. In recent years, there has been much interest in early postoperative mobilization. We present the results of our Achilles tendon repair technique and accelerated rehabilitation program. The technique we propose uses the strength of a 1-loop polydioxanone "suture frame" to enable restoration of the tendon length, immediate positioning of the foot in a near-plantigrade position, and an accelerated rehabilitation program. We followed up 15 cases of Achilles tendon rupture treated with this technique. The initial follow-up was a

Early controlled passive motion regime for zone II extensor tendon injury--a case report.

Zone II extensor tendon injuries are often associated with sharp laceration and saw injuries. These injuries are usually been immobilized for 4 to 6 weeks with static splint followed with gradual increment in ROM at distal interphalangeal (DIP) joint. Zone II injuries are usually associated with stiff DIP joint. Any effects taken to improve the DIP joint flexion aggravates the extensor lag at DIP joint. We report a dynamic passive mobilization regime for treating zone II extensor tendon injuries. We found dynamic passive mobilization program was effective in managing these injuries in preventi

Two-incision versus one-incision repair for distal biceps tendon rupture: a cadaveric study.

HYPOTHESIS: The purpose of this study was to accurately map the insertional footprint of the distal biceps tendon and to test our hypothesis that a superior anatomic repair can be achieved by a 2-incision technique when compared with a 1-incision technique. METHODS: We randomly assigned 20 cadaveric arms to 1 of 2 groups: 1 incision (group I) or 2 incision (group II). The bicipital tuberosity was exposed through either a single anterior incision (group I) or a posterolateral approach (group II). A guide pin was placed into the tuberosity to mark the axis for creating a virtual bone tunnel. Ea

Surgical repair of the distal biceps brachii tendon: a comparative study of three surgical fixation techniques.

PURPOSE: Several fixation techniques exist to repair the distal biceps brachii tendon. We aimed to evaluate the clinical and functional outcomes after distal biceps tendon repair using three different fixation techniques. METHODS: A total of 54 patients were included in the study and were available for follow-up (mean, 30.7 months; SD 18.6). Surgery was performed using a single-incision approach with Corkscrews or Mitek suture anchors or with a two-incision exposure using transosseous sutures. The patients were evaluated with range of motion (ROM) measurements for flection/extension and prona

Comparison of functional outcomes of two anterior cruciate ligament reconstruction methods with hamstring tendon graft.

OBJECTIVE: The aim of this study was to compare the effects of Endobutton post-fixation and femoral (TransFix) transfixation in ACL reconstruction on lower extremity muscle strength, joint position sense, and knee stability. METHODS: Subjects who had undergone ACL reconstruction with hamstring tendon using Endobutton post-fixation (n=20, mean age: 26.5 years) or femoral transfixation (n=20, mean age: 29.9 years) were recruited to an ACL rehabilitation program. Twelve months after surgery, quadriceps and hamstring torque values were recorded using an isokinetic dynamometer. Computerized coordi

Extended healing validation of an artificial tendon to connect the quadriceps muscle to the Tibia: 180-day study.

Whenever a tendon or its bone insertion is disrupted or removed, existing surgical techniques provide a temporary connection or scaffolding to promote healing, but the interface of living to non-living materials soon breaks down under the stress of these applications, if it must bear the load more than acutely. Patients are thus disabled whose prostheses, defect size, or mere anatomy limit the availability or outcomes of such treatments. Our group developed the OrthoCoupler™ device to join skeletal muscle to prosthetic or natural structures without this interface breakdown. In this study, the

Determination of optimal screw positioning in flexor hallucis longus tendon transfer for chronic tendoachilles rupture.

BACKGROUND: Neglected ruptures of the tendoachilles pose a difficult surgical problem. There are no data to determine the optimal positioning of the FHL tendon to the calcaneus. METHODS: Two computer programmes (MSC.visualNastran Desktop 2002™ and Solid Edge(®) V19) were used to generate a human ankle joint model. Different attachment points of FHL tendon transfer to the calcaneus were investigated. RESULTS: The lowest muscle force to produce plantarflexion (single stance heel rise) was 1355 N. Plantarflexion increased for a more anterior attachment point. The maximum range of plantarflexion

Postoperative MR imaging of the foot and ankle: tendon repair, ligament repair, and Morton's neuroma resection.

This review article describes the postoperative magnetic resonance (MR) findings relating to surgery after tendon repair, ligament repair, and Morton's neuroma resection. The normal postoperative tendon is commonly thickened, showing signal changes that are most pronounced 3 to 6 months after surgery. Two years after tendon suture, the signal intensity should be low on T2-weighted images. The focus of the postoperative MR imaging after ankle repair is to detect the normal condition after the various surgical procedures (e.g., Broström, Watson-Jones, Evans, or Chrisman-Snook). The repaired liga

Treatment of distal biceps tendon ruptures using a single-incision technique and a Bio-Tenodesis screw.

No consensus exists in the literature on the optimal operative treatment method for distal biceps tendon ruptures. It is our hypothesis that a single-incision technique with a poly-L-lactide Bio-Tenodesis screw (Arthrex, Inc, Naples, Florida) is a safe and effective method for operative management of distal biceps tendon ruptures, with success and complication rates comparable to previous reports in the literature. This article describes a prospective case series of 29 consecutive patients (30 operations) managed by the same surgeon over 34 months. Average follow-up was 19.6 months. Two patien
