An antegrade method for repositioning a prematurely released double J ureteric stent is described. The technique is applicable when the proximal pigtail is still within the renal parenchyma or in the perinephric space. A custom-made Nitinol gooseneck snare was used in this case, but suitable alternatives should be readily available in a standard interventional suite and these are described.

In children the use of internal ureteral stents has the added morbidity of requiring general anesthesia for stent retrieval. Magnetically tipped ureteral stents were developed to allow easy retrieval without the need for anesthesia or cystoscopy. For pyeloplasty we generally do not use a stent or nephrostomy tube but we treated 7 children 2 1/2 months to 11 years old in whom we believed there was an indication for stenting and in whom we used a magnetically tipped stent. The catheters were removed with a magnetically tipped catheter retriever without the need for anesthesia in 6 of the 7 cases

Catheterization is a safe and effective nursing intervention that is taught to almost every nursing student in the country. Most of the complications students are encouraged to prevent include: infection, skin irritation, trauma due to failing to release the bubble when removing the catheter, and obstruction. Rarely are additional problems encountered. Case studies can often alert us to the possibility of rare and unanticipated complications. The following is an example of just such a case study.

We reviewed the results of ureteral stent use with extracorporeal shock-wave lithotripsy (ESWL) in 3,096 patients with renal calculi less than 3 cm in diameter. The 2,595 patients with indwelling ureteral stents required lower total power (shocks x voltage) and less radiation and had a lower secondary procedure rate but a higher retreatment rate than the 501 patients without stents. Between 1981 and 1990, 151 patients with lower ureteral calculi received 163 applications of Zeiss basket (ZB) transurethrally. A male aged 73 years in poor cardio-pulmonary condition and with urinary retention due to prostatic infravesical obstruction was treated with implantation of a new type of plastic urethral catheter. Prostatectomy was planned after improvement in the cardio-pulmonary condition. This period was prolonged and, five months after implantation of the urethral catheter, massive incrustation in the proximal intravesical part had formed. The catheter was found to be firmly embedded in the prostatic part of the urethra and cystotomy proved necessary to remove it.

There is a certain volume of urine left within the bladder which cannot be drained by a catheter. A dilutional method was used to calculate this residual volume in 15 men; 10 had acute retention secondary to benign prostatic hyperplasia and 5 had permanent indwelling catheters for neurogenic urinary incontinence. A new, original self-retaining intraurethral catheter (IUC), 30-60 mm long, was inserted into the urethra and bladder neck in 34 patients with urinary retention due to benign prostatic hyperplasia. The placement of the IUC is easily done under local anesthesia in the outpatient clinic. Before insertion, all the patients had had an indwelling catheter for weeks or months. In 29 patients (85.3%) the IUC was left in place from 2-38 weeks (mean 13.8). These patients voided satisfactorily and were fully continent.
