Executive Functioning in Children and Adolescents with ADHD and Disruptive Behavior Disorders.

OBJECTIVE: The goal was to compare the impairment on executive functions in children and adolescents with attention deficit hyperactivity disorder (ADHD) alone and with comorbid oppositional defiant disorder (ODD), conduct disorder (CD), or both (ADHD+ODD+CD). DESIGN: A total of 162 patients were diagnosed with the Mini International Neuropsychiatric Interview for Children and Adolescents (MINI-KID), and the results of their performances in Behavior Rating Inventory of Executive Function (BRIEF) and the Tower of London (ToLo) were compared. RESULTS: Patients with only ADHD showed less impair

Perceptions of waived juvenile defendants across mental health diagnoses and demographic characteristics.

Justice involved youth who present with diagnosable mental health issues are commonly prosecuted in criminal courts. Limited research has examined how jurors perceive and respond to transferred juveniles with mental health issues. For the current study, 252 mock jurors were randomly assigned to read one of six profiles (i.e., White male, White female, Black male, Black female, Latino, and Latina) and report culpability, deserved punishment, behavior regulation, and dangerousness for juveniles diagnosed with conduct disorder, major depressive disorder, attention deficit hyperactivity disorder,

Personality Profile of Male Adolescents With Tourette Syndrome: A Controlled Study.

Tourette syndrome is a neurodevelopmental disorder characterized by multiple tics and commonly associated with behavioral problems, especially obsessive-compulsive disorder and attention-deficit hyperactivity disorder (ADHD). The presence of specific personality traits has been documented in adult clinical populations with Tourette syndrome but has been underresearched in younger patients. We assessed the personality profiles of 17 male adolescents with Tourette syndrome and 51 age- and gender-matched healthy controls using the Minnesota Multiphasic Personality Inventory-Adolescent version, al

Treatment of aggressive ADHD in children and adolescents: conceptualization and treatment of comorbid behavior disorders.

Primary care physicians who treat attention-deficit/hyperactivity disorder (ADHD) may expect to encounter oppositional defiant disorder (ODD) in about half of patients with ADHD. Up to 20% of patients with ADHD may meet criteria for conduct disorder (CD), and a higher percentage will exhibit aggressiveness or other symptoms of CD without meeting full diagnostic criteria. Primary care physicians self-report more competence in managing ADHD alone than when it is accompanied by comorbid ODD or CD, even though the diagnostic and treatment considerations are similar. The empirical literature on nor

A Tourette Syndrome/ADHD-like Phenotype Results from Postnatal Disruption of CB1 and CB2 Receptor Signalling.

Cannabinoid receptor 1 (CB1) signalling is critical for weight gain and for milk intake in newborn pups. This is important as in humans, low birth weight increases the risk for attention-deficit hyperactivity disorder (ADHD). Moreover, some children with ADHD also have Tourette syndrome (TS). However, it remains unclear if insufficient CB1 receptor signalling may promote ADHD/TS-like behaviours. Here, ADHD/TS-like behaviours were studied from postnatal to adulthood by exposing postnatal wild-type CB1 and Cannabinoid receptor 2 (CB2) knockout mouse pups to SR141716A (rimonabant), a CB1 receptor

New gene targets related to schizophrenia and other psychiatric disorders: enzymes, binding proteins and transport proteins involved in phospholipid and fatty acid metabolism.

Phospholipids make up about 60% of the brain's dry weight. In spite of this, phospholipid metabolism has received relatively little attention from those seeking genetic factors involved in psychiatric and neurological disorders. However, there is now increasing evidence from many quarters that abnormal phospholipid and related fatty acid metabolism may contribute to illnesses such as schizophrenia, bipolar disorder, depression and attention deficit hyperactivity disorder. To date the possible specific proteins and genes involved have been relatively ill-defined. This paper reviews the main pat

Anxiety Disorders Among Children and Adolescents.

Anxiety disorders represent the most common psychiatric illnesses affecting children and adolescents. Youths who suffer from anxiety disorders typically experience impairment in social, family, and educational domains of functioning. Despite the prevalence of youth anxiety disorders, identifying anxiety as the underlying cause can be a challenge. This article summarizes recent changes in diagnostic criteria in DSM-5, reviews core features of anxiety, and discusses how to recognize anxiety among youths. It also provides recommendations on how to differentiate anxiety from other diagnoses, with

Second-Generation Antipsychotics and Real-World Outcomes in Youth With On- and Off-Label Indications.

OBJECTIVE: Second-generation antipsychotic (SGA) treatment has increased in youth globally. However, the effects of these agents on real-world outcomes are largely unknown, especially for non-approved indications. METHOD: This study included a national cohort of incident SGA recipients 7 to 17 years of age in Sweden from 2007 to 2020. Youth were followed from 1 year before SGA initiation through up to 1 year afterward, and in recurrent treatment analyses, up to 13 years after initiation. Within-individual comparisons were used to examine the risk of psychiatric hospitalization, self-harm, acc

Risk Factors for Prolonged Postoperative Opioid Use After Spinal Fusion for Adolescent Idiopathic Scoliosis.

BACKGROUND: Opioids are commonly used after posterior spinal instrumented fusion (PSIF) for adolescent idiopathic scoliosis (AIS). Prescription opioids use can potentially lead to misuse, abuse, dependence, and overdose death. Prolonged opioid use has not been extensively studied in the postoperative AIS population. The purpose of this study is to identify risk factors associated with prolonged opioid use after PSIF for AIS. METHODS: A large insurance database was queried for AIS patients undergoing PSIF. Patients with prolonged postoperative opioid use were defined as those receiving new pre

Substance use disorders and chronic itch.

Chronic pruritus is one dermatologic manifestation of an underlying substance use disorder. Recent literature has uncovered similarities between the general neurologic mechanisms of addiction and chronic itch, largely involving activation of the dopaminergic reward circuits within the brain and imbalances between mu and kappa opioid receptor activation. It is likely that the use of specific drugs, like central nervous system stimulants and opioids, results in further activation and imbalances within these pathways, perpetuating both addiction and pruritus simultaneously. Opioid users often pre

Medial Frontal Hyperactivation in the Developing Obsessive-Compulsive Disorder Brain: An Adaptive Response Rescued by Medication-Related Reduction of Limbic Interference?

The capacity to control emotion and behavior is an important human adaptation. The development of cognitive control strategies is a critical aspect of children's social development and protects against psychopathology. Dysfunctions in inhibitory control play an important role in the development of neurodevelopmental disorders, such as obsessive-compulsive disorder (OCD), attention-deficit/hyperactivity disorder, and Tourette's disorder. Inhibitory control is not a unitary construct and consists of motor response inhibition (i.e., inhibition of pre-potent and automatic motor responses or cancel

The neuropsychiatry of Gilles de la Tourette syndrome: The état de l'art.

Gilles de la Tourette syndrome (GTS) is a chronic tic disorder characterised by the presence of multiple motor and vocal tics with onset during development. Tics are the most common hyperkinetic symptoms in childhood and co-morbid behavioural conditions (especially obsessive-compulsive disorder, attention-deficit and hyperactivity disorder, affective symptoms, and impulsivity) are present in the majority of patients. Although GTS is no longer considered a rare medical curiosity, its exact pathophysiology remains elusive. Recent research on the brain correlates of the subjective 'urge to tic' h

Diagnostic and clinical characteristics of early-manifesting females with Duchenne or Becker muscular dystrophy.

Manifestations of Duchenne and Becker muscular dystrophy (DBMD) are present in up to 40% of heterozygous females, but there are few reports of females who exhibit skeletal muscle symptoms in childhood. From the Muscular Dystrophy Surveillance Tracking and Research Network, a multi-site population-based surveillance network for dystrophinopathy, nine symptomatic female heterozygotes with onset of symptoms prior to age 9 years were identified. The median age at diagnosis was 8.3 years, and the median interval from first symptoms to diagnosis was 1.35 years. Of the nine female heterozygotes, four

Tic disorders: from pathophysiology to treatment.

Tic disorders are stereotypic behaviours, more frequent than once believed, and therefore likely to be encountered by primary care physicians. Tics usually begin in childhood and are the clinical hallmark of Tourette Syndrome (TS), the most common cause of tics. TS is a relatively common neurobehavioural disorder with a spectrum of manifestations that wax and wane during its natural course. The pathophysiology of tics, at molecular and cellular level, is still unknown, whereas structural and functional neuroimaging studies have shown the involvement of the basal ganglia and related cortico-stria

Psychopathic traits and conduct problems in community and clinic-referred samples of children: further development of the psychopathy screening device.

This study examined the structure of psychopathic traits in 2 samples of children. The nonreferred community sample included 1,136 children recruited from elementary schools in 2 school districts in the southeastern United States. The clinic sample included 160 children referred to an outpatient mental health clinic serving the same geographic region. In both samples, parent and teacher ratings of psychopathic traits were subjected to a principal-axis factor analysis, and the congruence of the factor structure across samples was examined using confirmatory factor analysis. In both samples, 1 d

Community-based multiple-gate screening of children at risk for conduct disorder.

The present study employed a multiple-gate screening procedure to identify children at risk for the development of conduct disorder. Measures of cross-setting disruptive behavior and parent discipline practices were administered in sequential fashion to screen a population of 7,231 children attending suburban elementary schools. Convergent validity of the respective gating measures was confirmed by significant correlations with adjustment constructs. Analyses of covariance performed between positive screens, negative screens, and low-risk comparison children on adjustment constructs at each ga

Recognition of facial emotions among maltreated children with high rates of post-traumatic stress disorder.

OBJECTIVE: The purpose of this study is to examine processing of facial emotions in a sample of maltreated children showing high rates of post-traumatic stress disorder (PTSD). Maltreatment during childhood has been associated independently with both atypical processing of emotion and the development of PTSD. However, research has provided little evidence indicating how high rates of PTSD might relate to maltreated children's processing of emotions. METHOD: Participants' reaction time and labeling of emotions were measured using a morphed facial emotion identification task. Participants inclu

Adolescent with tourette syndrome and bipolar disorder: a case report.

Tourette syndrome consists of multiple motor tics and one or more vocal tics. Psychopathology occurs in approximately 90% of Tourette syndrome patients, with attention-deficit/hyperactivity, mood, and obsessive-compulsive disorders being common. Additionally, Tourette syndrome and bipolar disorder may be related in some individuals. However, it is unclear why bipolar disorder may be overrepresented in Tourette syndrome patients, and more research is needed. Herein, we report the case of a 15-year-old boy diagnosed with both Tourette syndrome and bipolar disorder, whose symptoms improved with a

The underdiagnosis of neuropsychiatric Lyme disease in children and adults.

Lyme Disease has been called "The New Great Imitator," a replacement for that old "great imitator" neurosyphilis. This article reviews the numerous psychiatric and neurologic presentations found in adults and children. It then reviews the features of Lyme Disease, which makes it almost uniquely hard to diagnose, including the complexity and unreliability of serologic tests. Clinical examples follow that illustrate those presentations of this disease that mimic attention deficit hyperactivity disorder (ADHD), depression, and multiple sclerosis.
