Association Between Pregnancy and Symptomatic Hemorrhage in Women With Brainstem Cavernous Malformations and Bleeding in the Preceding Year: A Prospective Multicenter Study.

BACKGROUND: The association between pregnancy and the risk of symptomatic hemorrhage (SH) in brainstem cavernous malformations remains uncertain. We aimed to quantify the risk of pregnancy-associated SH in women with brainstem cavernous malformations. METHODS: We analyzed women with brainstem cavernous malformations in a prospective registry (2016-2022) experiencing pregnancy after enrollment. Follow-up was divided into pregnancy-associated segments (pregnancy plus ≤6 weeks postpartum or postabortion) and nonpregnant segments. Primary analysis used case-crossover models; propensity score-matc

Atypical longitudinal development of speech-evoked auditory brainstem response in preschool children with autism spectrum disorders.

Language impairment is common in children with autism spectrum disorders (ASDs). Previous research has shown that this disability may be, in part, due to atypical auditory processing of speech stimuli. However, how speech sounds are processed in children with ASD remains largely unknown. The present study assessed the developmental pattern of auditory information processing at the level of the brainstem in preschool children with ASD using speech-evoked auditory brainstem response (speech-ABR). Children with ASD (N = 15) and of typical developing (TD) (N = 20), both of preschool age, were enro

Auditory brainstem, middle and late latency responses to short gaps in noise at different presentation rates.

OBJECTIVE: The effects of rate on auditory-evoked potentials (AEP) to short noise gaps (12 ms) recorded at high sampling rates using wide-band filters were investigated. DESIGN: Auditory brainstem (ABR), middle latency (MLR), late latency (LLR) and steady-state (ASSR) responses were simultaneously recorded in adult subjects at four gap rates (0.5, 1, 5 and 40 Hz). Major components (V, Na, Pa, Nb, Pb, N1 and P2) were identified at each rate and analysed for latency/amplitude characteristics. Gap responses at 40 Hz were recovered from Quasi-ASSRs (QASSR) using the CLAD deconvolution method. ST

Clinical tolerance of corticospinal tracts in convection-enhanced delivery to the brainstem.

OBJECTIVE: Convection-enhanced delivery (CED) has been explored as a therapeutic strategy for diffuse intrinsic pontine glioma (DIPG). Variables that may affect tolerance include infusate volume, infusion rate, catheter trajectory, and target position. Supratentorial approaches for catheter placement and infusate distribution patterns may conflict with corticospinal tracts (CSTs). The clinical relevance of these anatomical constraints has not been described. The authors report their experience using CED in the brainstem as it relates to anatomical CST conflict and association with clinical tol

Bottlenose dolphin (Tursiops truncatus) auditory brainstem responses to frequency-modulated "chirp" stimuli.

Previous studies have demonstrated that increasing-frequency chirp stimuli (up-chirps) can enhance human auditory brainstem response (ABR) amplitudes by compensating for temporal dispersion occurring along the cochlear partition. In this study, ABRs were measured in two bottlenose dolphins (Tursiops truncatus) in response to spectrally white clicks, up-chirps, and decreasing-frequency chirps (down-chirps). Chirp durations varied from 125 to 2000 μs. For all stimuli, frequency bandwidth was constant (10-180 kHz) and peak-equivalent sound pressure levels (peSPLs) were 115, 125, and 135 dB re 1 μ

Electrode Selection and Speech Understanding in Patients With Auditory Brainstem Implants.

OBJECTIVES: The objective of this study was to evaluate whether speech understanding in auditory brainstem implant (ABI) users who have a tumor pathology could be improved by the selection of a subset of electrodes that were appropriately pitch ranked and distinguishable. It was hypothesized that disordered pitch or spectral percepts and channel interactions may contribute significantly to the poor outcomes in most ABI users. DESIGN: A single-subject design was used with five participants. Pitch ranking information for all electrodes in the patients' clinic maps was obtained using a pitch ran

Cochlear and brainstem audiologic findings in normal hearing tinnitus subjects in comparison with non-tinnitus control group.

While most tinnitus cases have some degree of hearing impairment, a small percent of the patients admitted to Ear, Nose and Throat Clinics or Hearing Evaluation Centers are those who complain of tinnitus despite having normal hearing thresholds. Present study was performed in order to better understanding of the probable causes of tinnitus and to investigate possible changes in the cochlear and auditory brainstem function in normal hearing patients with chronic tinnitus. Altogether, 63 ears (31 ears with tinnitus and 32 ears without tinnitus) were examined. The prevalence of transient evoked o

Time course of diffusion imaging in acute brainstem infarcts.

PURPOSE: To study the time course of diffusion imaging at the lesion site in brainstem infarcts. MATERIALS AND METHODS: Sequential MR scans were acquired from 24 patients with brainstem infarcts. Diffusion-weighted images (DWI), T(2)-weighted images (T(2)w), maps of apparent diffusion coefficient, and maps of fractional anisotropy were generated from each MR scan. A trend function was fitted to these measurements to model an objective, general time course of the studied parameters. RESULTS: Apparent diffusion coefficient (ADC) continuously decreased over time until a transition time around 4

SPET assessment of auditory cortex stimulation and tonotopic spatial distribution in auditory brainstem implant.

Activation of the auditory cortex by multifrequency acoustic stimuli has been evaluated using Single Photon Emission Tomography in a case of auditory brainstem implant after activation of 6 and 11 electrodes. Before implantation, no activation of the auditory cortex has been observed after acoustic stimulation. Following auditory brainstem implant, the stimulation of 11 electrodes showed an activation value, in terms of blood flow increase, of the contralateral temporal cortex similar to that obtained with 6 electrodes (47.70 vs. 43.76%), but a significantly stronger activation was present in

Hearing habilitation with auditory brainstem implantation in two children with cochlear nerve aplasia.

Patients with aplasia and hypoplasia of the cochlear nerve have no chance of having their hearing restored by stimulating the periphery of the auditory system using the traditional cochlear implant. A possible approach to auditory rehabilitation may be direct electrical stimulation of the cochlear nuclei with an auditory brainstem implant (ABI). Recently, two children, aged 4 and 3 years, respectively, with bilateral severe cochlear malformations and cochlear nerve aplasia received an ABI. The present paper reports the technique and the preliminary results of this experience. The classic retro

Multichannel auditory brainstem implantation: the Australian experience.

The multichannel auditory brainstem implant (ABI) provides the potential for hearing restoration in patients with neurofibromatosis type 2 (NF2). Programmes for auditory brainstem implantation have been established in two Australian centres. Eight patients have been implanted under the protocol of an international multi-centre clinical trial. Three patients had ABI insertion at the time of first side tumour removal, four at second side tumour removal and one after previous bilateral surgery where there was some residual tumour. The translabyrinthine approach was used in all cases. Successful p

Binaural auditory processing in multiple sclerosis subjects.

In order to relate human auditory processing to physiological and anatomical experimental animal data, we have examined the interrelationships between behavioral, electrophysiological and anatomical data obtained from human subjects with focal brainstem lesions. Thirty-eight subjects with multiple sclerosis were studied with tests of interaural time and level discrimination (just noticeable differences or jnds), brainstem auditory evoked potentials and magnetic resonance (MR) imaging. Interaural testing used two types of stimuli, high-pass (> 4000 Hz) and low-pass (< 1000 Hz) noise bursts. Abn

[A case of Moebius syndrome--electrophysiological studies of facial nerve and brainstem].

A five-year old boy was the product of a 40 week pregnancy by vertex presentation complicated only by threatened abortion at approximately 8 weeks gestation. Apgar score was 5 after one minute. At birth he was noted to have a generalized hypotonia associated with facial diplegia, small mandible, weak suck and swallow reflexes. Admission examination revealed small mandible, mask-like facial expression and mild mental retardation. Cranial nerve examination showed bilateral blepharoptosis and facial nerve palsies. Pupil reflexes were normal, but corneal reflexes were impaired bilaterally. Diplopi

Prognostic value of auditory brainstem response for neurologic outcome in patients with neonatal indirect hyperbilirubinemia.

To investigate the value of the auditory brainstem response as a reliable test for the neurologic prognosis of infants with neonatal indirect hyperbilirubinemia, auditory brainstem response studies were performed in 22 infants. The patients were followed up until 12 months of age. Two patients demonstrated pathologic auditory brainstem response consistent with auditory neuropathy but had no neurologic finding except a lack of speech at 12 months of age. Two other patients had neurologic sequelae, one showing severe dyskinetic cerebral palsy, the other mild hypotonia and motor retardation, but

Multimodality evoked potentials in aortoarteritis.

Brainstem auditory evoked potentials (BAEPs) have been used to detect early and even asymptomatic brain ischemia. There is a need to detect subclinical brain and/or spinal cord ischemia in patients with aortoarteritis so that ischemic deficits can be prevented by early intervention. We studied multimodality (Visual evoked potentials--VEPs, Somatosensory evoked potentials--SEPs and BAEPs) evoked potentials (EPs) in 10 neurologically asymptomatic subjects with aortoarteritis. EP latencies were normal in all the patients studied. The possible value of EP latencies in detection of early brain and/

Computed tomography and magnetic resonance imaging of the brainstem.

The brainstem is a fundamental central nervous system component, essential for life. It comprises the midbrain, pons, and medulla oblongata. It is connected superiorly to the diencephalon, inferiorly to the spinal cord, and posteriorly to the cerebellum. The brainstem is pivotal in permitting transit for ascending and descending tracts, integrative functions, such as consciousness and respiratory cycle, and transmitting action potentials to cranial nerves. Lesions in this region can lead to diverse neurologic deficits. Imaging the brainstem presents challenges due to the proximity of critical
