Epidemiological aspects of Trichuris and trichuriasis in Caribbean communities.

The review argues for a reappraisal of the health significance of the human whipworm, Trichuris trichiura. Infections with this geohelminth are at least as prevalent as Ascaris lumbricoides in many localities, and are associated with significant morbidity. Infection may result in severe trichuriasis syndrome or, more frequently, in a chronic colitis associated with growth stunting. Under-reporting of the chronic manifestations of disease has resulted in a gross under-estimation of the health impact of trichuriasis. Furthermore, estimation of the population dynamical parameters of T. trichiura

Community perceptions of appropriate sources of emergency care.

Two hundred eighteen residents in a suburban community northwest of Chicago were interviewed using standard telephone survey techniques. Respondents lived in an area served by two hospitals and two freestanding emergency centers (FECs) that had been in operation for three years prior to the survey. Those surveyed were asked to react to three common medical emergencies of graded levels of severity, small laceration, injured ankle, and myocardial infarction, to identify self-referral patterns of this community. Respondents were questioned about their knowledge and perception of the FECs and thei

[Community-acquired pneumonias: importance of pneumococcal pathology].

The frequency and the various etiologies of community acquired pneumonia are not well established. In adults, it is believed that half of the cases are caused by viruses and Mycoplasma pneumoniae. The pneumonia in these patients is limited and treated on an ambulatory basis. In patients who need to be hospitalized, the main etiology of pneumonia is S. pneumoniae. Despite effective antibiotic treatment, pneumococcal pneumonia still remains a life-threatening infection, especially in bacteremic patients presenting with underlying predisposing factors such as renal insufficiency, liver cirrhosis,

Aetiology and outcome of severe community-acquired pneumonia.

Between January 1972 and December 1981, 50 patients with severe community-acquired pneumonia were admitted to the intensive care unit of a district general hospital. A causal pathogen was identified in 41 cases (82%). Streptococcus pneumoniae (16 cases), Legionella pneumophila (15 cases) and Staphylococcus aureus (5 cases) were the commonest. Assisted ventilation was required in 44 patients, of whom 25 died (57%). All 5 patients with staphylococcal pneumonia and 12(75%) with pneumococcal pneumonia died. Only 5 (33%) with Legionnaires' disease died. Mortality was significantly associated with a

Demographics, stress, and depression in a community health screening.

Stress has been found to a significant factor in the development of physical illness. The relationship between demographic factors such as gender and age and their influence on stress and illness has not been extensively investigated. The present study investigated the relationship among life events, affective disorders, life satisfaction, and stress symptoms in a cross-sectional sample of 463 ambulatory visitors to a voluntary health screening. Age relationships were found to be strongest for life satisfaction, stress symptoms, and life stresses, whereas gender was the major factor in depress

Recurrent thyrotoxicosis and papilledema in a patient with communicating hydrocephalus.

A patient with communicating hydrocephalus had recurrent papilledema during episodes of thyrotoxicosis. Increased intracranial pressure was documented and, as reflected by the severity of the papilledema, fluctuated in proportion to the thyroid hormone levels.

Mallory-Weiss syndrome. Experience in a community hospital.

The Mallory-Weiss syndrome is characterized by repeated bouts of retching and/or vomiting followed by the sudden onset of hematemesis or melena. Bleeding arises from linear, nonperforating mucosal lacerations at the cardia, cardioesophageal junction, distal esophagus, or a combination of these sites. Hiatal hernia is often a coexisting finding. Severity of hemorrhage can vary from mild to severe (100 to 2,000 ml). The clinical course is usually benign. The diagnosis can be suspected from the history and confirmed by upper gastrointestinal endoscopy. In the majority of patients, medical managem

An outbreak of community-acquired Legionnaires' disease pneumonia.

In October 1981, an outbreak of 29 cases of community-acquired pneumonia occurred among adult residents of Johnson County, Iowa. Retrospective study revealed 12 cases (41 per cent) had laboratory evidence of Legionnaires' disease (LD). No significant differences in clinical or epidemiological features were found between LD cases and the other pneumonias in the outbreak. All LD cases received erythromycin; one case died for a case-fatality rate of 8 per cent. The outbreak's focus could not be identified.

An epidemiological study of leprosy in a rural community of Pondicherry.

In a rural area, endemic for Leprosy a total population survey had been undertaken in 6 villages covering a population of 5667. Out of the 5281 persons examined, 191 were found to be suffering from leprosy giving a prevalence rate of 36.16/1000. The prevalence was highest in children aged between 5 and 14 years., i.e., 41.61/1000. Among adults the highest prevalence was seen among those aged above 45 years, i.e., 45.66/1000. Both the sexes were found to be equally affected in this area. Tuberculoid leprosy was the most common type found with a prevalence rate of 23.29/1000, followed by Indeter

Chemoprophylaxis for Neisseria meningitidis in an isolated Arctic community.

Community chemoprophylaxis with a regimen of sequential minocycline/rifampin (adults) or rifampin alone (children [less than 12 years of age]) was undertaken in a remote Arctic community one year after an outbreak of meningitis due to Neisseria meningitidis serogroup B. Nasopharyngeal carriage rates of N. meningitidis before prophylaxis were 32.4% in Inuit (Eskimos) and 6% in Caucasians, with maximal carriage (44.8%) in adolescents. Serogroup B accounted for 63.9% of all isolates before prophylaxis. One week after prophylaxis, the nasopharyngeal carriage rates were 0.8% in Inuit who had receiv

Hospital study of adult community-acquired pneumonia.

The cause of primary pneumonia was diagnosed in 124 of 127 consecutive adult patients admitted to hospital with community-acquired illness. Pneumococcal infection was found in 96 (76%) patients and legionnaries' disease was the second commonest infection identified (15%). Other bacterial infections were uncommon. 11 patients had atypical pneumonia, including 7 with psittacosis. There were several mixed infections and most of the 11 patients with viral infections also had bacterial pneumonia. 19 patients died (15%) and mortality was associated with increasing age, the presence of coexisting dis
