Idiopathic Spinal Epidural Hematoma: A Near Miss of a Rare Entity.

Spinal epidural hematomas (SEH) are rare, and cases with a spontaneous etiology are even more infrequent. Management of spontaneous SEH varies, with surgical or conservative approaches determined by the severity of deficits and symptom resolution. Adverse prognostic factors may include thoracic segment location, anticoagulation use, severe neurologic deficits at admission, sphincter dysfunction, and rapid progression. We report a patient with a sudden onset of bilateral lower limb weakness and reduced urinary output. Magnetic resonance imaging was conducted and indicated an epidural hematoma e

Reversal of cognitive, behavioral, and language impairments after the left frontal arachnoid cyst fenestration in a pediatric patient.

BACKGROUND: Arachnoid cysts (ACs) are cerebrospinal fluid-containing cysts located between the surface of the brain or spinal cord and arachnoid layer of the leptomeninges. ACs have been known to cause cognitive, language, and behavioral deficits and currently there is no standard treatment paradigm. Surgical indications include papilledema, increasing growth with mass effect causing neurological deficit, or rapid head growth, however, cognitive symptoms related to mass effect may not always be considered. CASE DESCRIPTION: We present a 3-year-old male with an AC of the left anterior fossa ca

Intra-articular fractures of the distal humerus-a review of the current practice.

Intra-articular fractures of the distal humerus are complex injuries that can considerably limit elbow function if not treated appropriately. Surgical management is indicated for most intra-articular distal humerus fractures with the goal of restoring elbow range of motion and function. Open reduction and internal fixation (ORIF) with plates and screws has been the preferred surgical option. Double plating is recommended for bicolumnar fractures and plates can be applied either parallel or orthogonal to each other. Surgical approach for ORIF of the distal humerus can be performed through an ol

Medical management of osteonecrosis of the hip: a review.

Osteonecrosis or avascular necrosis (AVN) of the hip is a progressive disease mainly affecting adults in their third, fourth or fifth decade of life. Studies into the natural history of the disease suggest that femoral head collapse occurs within 2-3 yrs with associated degenerative changes and at that stage arthroplasty is the most reliable treatment option. Therefore prevention of femoral head collapse is highly desirable in this young patient group. In early stage disease, before femoral head collapse (Ficat and Arlet stage 1-3) core decompression of the femoral head is currently the most w

[Surgical therapy of lumbar disk herniation in 6 815 cases].

OBJECTIVE: To explore the surgical therapy for lumbar disc herniation (LDH). METHODS: Under local anesthesia by blocking the posterior branches of spinal nerves, 3 surgical approaches were adopted in 6 815 LDH cases, namely discectomy by lateral and interlamina access, double fenestration and enhanced decompression for broad central LDH, and removal of nucleus pulposus with small incision and fenestration. RESULTS: Follow-up study was conducted in 5 324 cases for the duration varying from 6 months to 14 years, averaging 6.3 years. Excellent results were achieved in 4 260 cases (80.02%), good

Bacterial Contamination of Autologous Cranial Bone Flaps: Microbiological Analysis and In Vitro Antibiotic Decontamination.

OBJECTIVE: Bacteria present on extracted bone flaps during decompressive craniectomy inhibit bone flap reimplantation. An adapted treatment with antibiotic mixture for decontamination of allografts could be used in neurosurgery for preparation of bone flaps for autologous cranioplasty. METHODS: Eleven bone flaps were analyzed using standardized bacterial cultures and droplet digital polymerase chain reaction. Growth of Cutibacterium acnes, Staphylococcus aureus, and other Staphylococci species were detected. Bone flaps were treated with antibiotic mixtures for 24 hours before evaluating the a

Decompression of Axillary Vein: An Essential Adjunct for Advanced Lymphedema.

BACKGROUND: In advanced lymphedema, lymphovenous anastomosis (LVA) can be a solution based on using the pressure gradient between the high-pressure lymphatics and the low-pressure veins. If the vein pressure is high, the effect of surgery will be less optimal. This study evaluated the effect of axillary vein perivascular scar release on LVA. METHODS: This is a retrospective study of 40 upper limb stage 2 and 3 lymphedema patients divided into 2 groups with an average follow-up of 33 months (minimum of at least 12 months): the scar release group ( n = 25) and the control group ( n = 15). Pre- and postoperative MRI analysis of central decompression in MIS fusion with lumbar stenosis.

OBJECTIVES: Mini-invasive approaches have grown substantially these last decades in spinal surgery, notably for lumbar decompressions and fusion, with advantages over open approaches in terms of morbidity reduction. However, to our knowledge, no study has measured on MRI the amount of central decompression obtained by MIS approach. The goal of this study was to precisely measure the decompression of central stenosis by unilateral MIS approaches. METHODS: The files of 42 patients that had a MIS lumbar fusion with central decompression for central stenosis were reviewed. All patients had a pre-

Minimally Invasive Interlaminar Decompression with a 10-mm Endoscope and Microscope in Cases of Adult Degenerative Scoliosis.

BACKGROUND: Adult degenerative (de novo) scoliosis (ADS) usually occurs due to degenerative changes and is accompanied by progressive low-back pain and/or symptomatic lumbar stenosis. Interlaminar decompression is considered an effective treatment of lumbar stenosis, but some surgical contraindications to traditional open surgery limit its application in elderly patients with many disorders. A 10-mm endoscope has been used in the treatment of stenosis in individuals with ADS and its safety and efficacy should be assessed. OBJECTIVE: The objective was to conduct a retrospective analysis to com

Treatment of L5-S1 Spondyloptosis with Multiple Pedicle Defects Through a Combined Anterior and Posterior Approach.

BACKGROUND: An anterior dislocation of the L5 vertebral body from the sacrum and decent into the pelvis is considered as spondyloptosis. The surgical treatment of L5-S1 spondyloptosis remains challenging and controversial. We present a case of lumbosacral spondyloptosis with multiple pedicle defects treated through a combined anterior and posterior approach. CASE DESCRIPTION: A 38-year-old female patient suffering from L5-S1 spondyloptosis with multiple pedicle defects without any history of trauma presented with lower back pain and bilateral lower leg radiating pain. Use of Rituximab After Orbital Decompression Surgery in Two Grave's Ophthalmopathy Patients Progressing to Optic Neuropathy.

BACKGROUND: While orbital decompression can alleviate optic nerve compression and prevent further vision loss in dysthyroid optic neuropathy (DON), it cannot relieve inflammatory symptoms. Very high doses of intravenous glucocorticoids (GCs) are the first-line therapy for DON; however, the effective rate is only 40% and might be much lower in patients who fail high-dose GC pulse therapy and progressed to DON. The results of two case series studies indicated that rituximab treatment had a much better curative effect compared to very high doses of intravenous GCs, but some patients required urge

Relief of Anteriorly Translocated Pulmonary Artery Compression With a Nuss Bar.

A concomitant anterior translocation of the right pulmonary artery (RPA) can be used in patients with airway compression by a dilated RPA associated with congenital heart disease having a large left-to-right shunt or aortic arch anomaly. However a chest wall deformity and mechanical compression of the anteriorly translocated RPA could develop after the operation. In this situation simple RPA angioplasty is not sufficient. Therefore we adopted a technique to perform a concomitant Nuss procedure in an effort to resolve this problem. After 8 months of follow-up a widely patent RPA and a normal ap

Greater Proptosis Is Not Associated With Improved Compressive Optic Neuropathy in Thyroid Eye Disease.

PURPOSE: Despite the paucity of supporting data, it has generally been held that proptosis in thyroid eye disease (TED) may provide relative protection from compressive optic neuropathy (CON) by producing spontaneous decompression. The objective of this study was to investigate this phenomenon in patients with bilateral TED-CON. METHODS: We retrospectively reviewed the charts of 67 patients (134 orbits) with bilateral TED-CON at Columbia-Presbyterian Medical Center. Significant asymmetric proptosis (Hertel) was defined as ≥ 2 mm. Significant asymmetric CON was defined first, as the presence o
