Extensor tendon rupture associated with osteoarthritis of the distal radioulnar joint.

Non-rheumatoid osteoarthritis of the distal radioulnar joint can cause extensor tendon rupture. We analysed the radiographic morphology of the distal radioulnar joint to identify the risk factors for this complication. Forty-one wrist X-rays of 37 patients with extensor tendon rupture caused by distal radioulnar joint osteoarthritis were evaluated retrospectively for the severity of osteoarthritis by the Kellgren/Lawrence scoring system. Measurements were obtained from posteroanterior views. All but one wrist had severe osteoarthritic changes exceeding grade 3. The radiographic features that w

Skeletal traction and delayed percutaneous fixation of complicated supracondylar humerus fractures due to delayed or unsuccessful reductions and extensive swelling in children.

The functional and radiological results of 13 supracondylar humerus fractures of 13 children were evaluated to determine the efficiency of delayed percutaneous fixation of the fractures after a period of skeletal overhead traction. All the fractures were complicated by delayed reduction, extensive swelling or unsuccessful reduction manipulations. As the unfavourable preexisting factors subsided under skeletal traction, anatomical reduction and fixation of fractures by percutaneous pinning were performed under general anesthesia. The average follow-up period was 21 months. Bauman and lateral ca

Flexion-type supracondylar elbow fractures in children.

Flexion-type supracondylar elbow fractures are uncommon in children. Minimally displaced fractures were treated in plaster cast in 7 children, and 22 children with a displaced fracture were treated by closed reduction and percutaneous pinning. Results after an average follow-up of 6.3 years showed that the treatment of choice yielded excellent or good results in 86.2% of the patients. All patients were satisfied with the end result and had normal use of their elbow.

Rehabilitation in extremity fractures.

General principles in the rehabilitation of a patient with an extremity fracture include: treat the patient, not the x-ray; move all joints not immobilized; prevent disuse atrophy; use gravity to assist in mobilizing a joint; stabilize proximal joints to avoid reverse action of biarticular muscles; permit early protected weight bearing until adequate joint mobility is achieved; appropriately instruct the patient in a home program, and avoid all stretching.

Dual-Approach AI for Pediatric Supracondylar Fractures: Multiclass Radiograph Classification with Explainable AI and Diagnostic Meta-analysis of AI-Based Computational Approaches.

RATIONALE AND OBJECTIVES: Pediatric supracondylar fractures (SCFs) are the most common elbow injury in children, yet radiographic diagnosis remains challenging due to complex developmental anatomy, with initially missed fracture rates of 17-77%. Prior artificial intelligence (AI) studies have been limited to binary classification frameworks without Gartland subtype differentiation, and no diagnostic test accuracy meta-analysis specific to supracondylar fractures exists. This study aimed to develop the first multiclass deep learning model for SCF detection and Gartland classification and to sys

Description of outcomes of upper-extremity musculoskeletal disorders in workers highly exposed to repetitive work.

PURPOSE: The outcomes of workers with upper-extremity musculoskeletal (UEMS) symptoms or disorders were evaluated over a 3-year period in a population highly exposed to repetitive work. METHODS: Data were derived from a survey focused on the health effects of repetitive tasks, with a standardized physical examination by an occupational physician, in 1993 to 1994 and again in 1996 to 1997. All workers with UEMS symptoms or disorders in 1993 to 1994 were included. We classified 3-year outcomes into 3 categories in 1996 to 1997: neither UEMS disorder nor symptoms, UEMS symptoms only, and UEMS di

Expanding traditional tendon-based techniques with nerve transfers for the restoration of upper limb function in tetraplegia: a prospective case series.

BACKGROUND: Loss of upper extremity function after cervical spinal cord injury greatly affects independence, including social, vocational, and community engagement. Nerve transfer surgery offers an exciting new option for the reanimation of upper limb function in tetraplegia. The aim of this study was to evaluate the outcomes of nerve transfer surgery used for the reanimation of upper limb function in tetraplegia. METHODS: In this prospective case series, we consecutively recruited people of any age with early (<18 months post-injury) cervical spinal cord injury of motor level C5 and below, w

Modified Partial Radial to Axillary Nerve Transfer.

BACKGROUND: Nerve transfers are routinely performed in patients with brachial plexus injuries because these patients have limited alternative solutions secondary to their severe injury with substantial functional limitations5,6. Nerve transfers offer distinct advantages over other surgical options, as they are able not only to bypass the zone of injury but also to decrease regeneration time because of the proximity of the motor end plate to the repair site1,2. It is for this latter reason that a nerve transfer should be considered for an isolated axillary nerve injury, in which a full recovery

Restoration of reaching and grasping movements through brain-controlled muscle stimulation in a person with tetraplegia: a proof-of-concept demonstration.

BACKGROUND: People with chronic tetraplegia, due to high-cervical spinal cord injury, can regain limb movements through coordinated electrical stimulation of peripheral muscles and nerves, known as functional electrical stimulation (FES). Users typically command FES systems through other preserved, but unrelated and limited in number, volitional movements (eg, facial muscle activity, head movements, shoulder shrugs). We report the findings of an individual with traumatic high-cervical spinal cord injury who coordinated reaching and grasping movements using his own paralysed arm and hand, reani

Inflammatory cytokines and matrix metalloproteinases in the synovial fluid after intra-articular elbow fracture.

BACKGROUND AND HYPOTHESIS: Post-traumatic elbow contracture remains a common and challenging complication with often unsatisfactory outcomes. Although the etiology is unknown, elevated or abnormal post-fracture synovial fluid cytokine levels may result in the migration of fibroblasts to the capsule and contribute to capsular pathology. Thus, the purpose of this study was to characterize the cytokine composition in the synovial fluid fracture hematoma of patients with intra-articular elbow fractures. METHODS: The elbow synovial fluid fracture hematoma of 11 patients with intra-articular elbow

Surgical technique for the treatment of C5 and C6 root avulsion.

C5 and C6 root avulsion is generally treated by neurotization of musculocutaneous (M.C.) nerve by spinal accessory (S.A.) or intercostal nerve, and neurotization of supra-scapular nerve by spinal accessory. For the last few years, permanent paralysis of C5 and C6 root has been treated by neurotization of musculocutaneous nerve by one or two fascicles of the ulnar or median nerve, and axillary nerve by a few fascicles of the radial nerve. Eighteen patients with M.C. nerve paralysis were treated by end-to-end suture of one or two fascicles of the ulnar nerve. Patients were followed for 4 years.
