Lower extremity reconstruction for limb salvage and functional restoration - The Combat experience.

Evolution in extremity injury treatment often occurs during major conflicts, with lessons learned applied and translated among military and civilian settings. In recent periods of war, improvements in protective equipment, in-theater damage control resuscitation/surgery, delivery of antibiotics locally/systemically, and rapid evacuation to higher levels of medical care capabilities have greatly improved combat casualty survivability rates. Additionally, widespread application of lower extremity tourniquets also has prevented casualties from exsanguination, thus reducing hemorrhagic-related dea

A case-control pilot study of stress fracture in adolescent girls: the discriminative ability of two imaging technologies to classify at-risk athletes.

UNLABELLED: Since stress fractures are common among adolescent athletes, it is important to identify bone assessment tools that accurately identify risk. We investigated the discriminative ability of two imaging technologies to classify at-risk athletes. Findings suggested that peripheral quantitative computed tomography (pQCT) has the ability to distinguish differences in bone structure in injured vs. uninjured limbs. INTRODUCTION: Given the high stress fracture (SFX) prevalence among adolescent girls, an understanding of the most informative assessment tools to identify SFX risks are requir

Factors Contributing to Morbidity after Combined Arterial and Venous Lower Extremity Trauma.

The purpose of this study was to evaluate the impact of management of venous injury on clinical outcomes in patients with combined lower extremity arterial and venous trauma. Patients with common and external iliac, common and superficial femoral, and popliteal artery injuries were identified. Patients who underwent vein repair and those who received vein ligation were compared. The analysis was repeated for those patients who required secondary intervention for their arterial injury and those who did not require secondary intervention. Seventy patients were identified with both arterial and v

On-site management of medical encounters during obstacle adventure course participation.

Obstacle adventure courses (OAC) are increasing in popularity. Although injuries are not uncommon, there is scant medical literature documenting the morbidity and mortality associated with these events. This manuscript describes a case series. Event demographics, medical coverage, and injuries/illnesses documented during four OAC events in British Columbia, Canada, are discussed - Tough Mudder™ (2012 and 2013) and Warrior Dash© (2011 and 2012). The patient presentation rate across all events ranged from 26.53 to 37.40 per 1,000 participants. Ambulance transfer rates were low (range = 0 to 5 pe

Fascia-only anterolateral thigh flap for extremity reconstruction.

INTRODUCTION: The ability to use the anterolateral thigh (ALT) flap as a vascularized fascial flap, without skin or muscle, was first documented by Koshima et al in 1989. The authors mention the possibility of using the fascia alone for dural reconstruction. Despite its description more than 20 years ago, little literature exists on the application of the ALT flap as a vascularized fascial flap. In our experience, the ALT flap can be used as a fascia-only flap for thin, pliable coverage in extremity reconstruction. METHODS: After approval from the institutional review board, the medical recor

Reconstruction of distal lower extremities defect using the free peroneal artery perforator vessel based flap.

Soft tissue defects of the distal lower extremities are challenging. The purpose of this paper is to present our experiences with the free peroneal artery perforator flap for the reconstruction of soft tissue defects of the distal lower extremity. Nine free peroneal artery perforator flaps were used to reconstruct soft tissue defects of the lower extremities between April 2006 and October 2011. All flaps were used for distal leg and foot reconstruction. Peroneal artery perforator flaps ranged in size from 2 cm × 4 cm to 6 cm × 12 cm. The length of the vascular pedicle ranged from 2 to 6 cm. Restoration of altered somatosensory cortical representation with spinal cord stimulation therapy in a patient with complex regional pain syndrome: a magnetoencephalography case study.

OBJECTIVES: Development of effective chronic pain treatment strategies has been hampered by the lack of an objective pain biomarker. Magnetoencephalography (MEG) has demonstrated cortical disorganization corresponding to the affected limb of complex regional pain syndrome (CRPS) patients and spinal cord stimulation (SCS) can acutely treat CRPS in a reversible and adjustable fashion. In order to better define a potential MEG-sensitive biomarker for chronic pain, our goal was to study the effects of therapeutic SCS on cortical disorganization in patients with unilateral limb CRPS. METHODS: Two

Clinical outcomes of isolated lower extremity or foot burns in diabetic versus non-diabetic patients: a 10-year retrospective analysis.

INTRODUCTION: The incidence of diabetes mellitus (DM) in the United States is expected to increase from 8 per 1000 in 2008 to 15 per 1000 by 2050 [20]. As a result, DM patients will constitute a large proportion of Burn Center admissions, with burns typically due to contact burn or scalding. Peripheral vascular disease (PVD) and peripheral neuropathy (PN) are far more common in DM patients, particularly in those with poorly controlled disease, and are often associated with worse outcomes than non-diabetic (nDM) burn patients. This study sought to analyze whether the outcome of isolated leg and

The incidence of acute venous injury as a result of proximity penetrating trauma screened with colour flow duplex ultrasound.

INTRODUCTION: The incidence of acute deep venous thrombosis as a result of penetrating proximity extremity trauma (PPET) to the thigh has been demonstrated to be 16% in a single report. The purpose of the current study is to demonstrate the incidence and clinical significance of venous injury as a result of proximity trauma to the thigh in a large cohort screened with colour flow duplex (CFD) ultrasound and to identify factors predictive of defining a wound in proximity to a major vascular structure. PATIENTS AND METHODS: A prospective observational study was conducted from January 1st, 2010

Distally based fasciocutaneous sural flap for foot reconstruction: a retrospective review of 10 years experience.

BACKGROUND: The treatment of soft tissue defects of the foot is a problem mainly connected to the thickness of the coverage tissues, to the poor circulation, and to the frequent involvement of muscle, tendon, and bone. The authors present their experience with the sural flap, also in some particular cases. MATERIALS AND METHODS: The authors treated 33 patients for small- and medium-size defects of the foot, caused by work, home, and road accidents, and by venous or diabetic ulcers. In all cases, the flap was cut in its fasciocutaneous variant; an extension of the sole portion of fascia was ad

Damage control resuscitation for vascular surgery in a combat support hospital.

BACKGROUND: Hemorrhage from extremity wounds is a leading cause of potentially preventable death during modern combat operations. Optimal management involves rapid hemostasis and reversal of metabolic derangements utilizing damage control principles. The traditional practice of damage control surgery favors a life over limb approach and discourages elaborate, prolonged vascular reconstructions. We hypothesized that limb preservation could be successful when the damage control approach combines advanced resuscitative strategies and modern vascular techniques. METHODS: Trauma Registry records a

The use of a hands-free crutch in patients with musculoskeletal injuries: randomized control trial.

Unilateral musculoskeletal below-knee injuries occur with great frequency. Patients who cannot bear weight on an injured limb usually mobilize themselves with standard crutches. When the patient also has an upper limb injury, however, mobilization might be impossible, and can result in a lengthy in-patient stay. A randomized control trial was conducted on 80 patients to share our experience with the innovative 'hands-free crutch', and to discuss the potential of this device for more frequent use in orthopaedic surgery. We present its value in facilitating early discharge in patients with both

[Generators of walking movements in humans: spinal mechanisms of their activation].

The paper is focused on spinal generation of walking movements in patients afflicted with loss of supraspinal control consequent to back trauma. The author cites literature on methods of initiation of walking movements with pharmacological and proprioceptive stimulation. On his own experimental investigations with epidural electrical stimulation of the spinal marrow dorsal surface he proves existence of walking generators in the human. There is evidence that the walking pattern in extensors is formed through amplitude modulation of monosynaptic reflexes, whereas in flexors it is established th

Developing prediction rules and evaluating observation patterns using categorical clinical markers: two complementary procedures.

Substantial uncertainty often remains at the time that important diagnostic or therapeutic decisions must be made, despite the availability of multiple clinical indicators. Multiple indicators may be used to define observation patterns that are associated with the presence or absence of disease. Clinical prediction rules based on groups of observation patterns have been used to quantify probabilities and reduce error rates for some medical problems, but efficient use of multiple indicators remains a major challenge in medical practice. Medical outcomes and clinical observations are frequently
