Artificial Intelligence-Driven MRI for Cervical Nodal Metastasis Detection in Oral Squamous Cell Carcinoma: A Hierarchical Meta-Analysis of Diagnostic Accuracy.

BACKGROUND: Artificial intelligence (AI) applied to magnetic resonance imaging (MRI) may improve detection of cervical lymph node metastases in oral squamous cell carcinoma (OSCC) but is heterogeneous. METHODS: A systematic review identified observational studies (from 2000) evaluating AI-based MRI in adults with histopathologically confirmed OSCC. Risk of bias was assessed with QUADAS-AI. Diagnostic performance was synthesized using a hierarchical bivariate model. Publication bias and certainty of evidence were assessed using Deeks' test and GRADE. RESULTS: Twelve studies were included; sev

Tumor draining lymph nodes in non-small cell lung cancer: underrecognized role in biomarker discovery and therapeutic innovation.

Recent advances have furthered our understanding of the role of the tumor draining lymph node (TDLN) in the immune response to thoracic malignancies. This review synthesizes the rapidly expanding evidence that tumor draining lymph nodes (TDLNs) are not passive conduits of metastasis but dynamic immunologic organs that shape anti-tumor immunity in non-small cell lung cancer (NSCLC). Across cytokine, cellular, genomic, transcriptomic, and metabolic domains, the TDLN microenvironment becomes progressively remodeled towards immune suppression. These changes influence tumor growth and early metasta

Impact of R0/R1 Margin Status on Overall Survival in Node-Positive Resected Pancreatic Ductal Adenocarcinoma: Meta-Analysis.

BACKGROUND: Microscopic residual disease (R1)and node-positive disease in resected pancreatic ductal adenocarcinoma (PDAC) are associated with worse survival. Some have suggested that obtaining an R0 resection is no longer relevant when patients have node-positive disease. A meta-analysis confirming this hypothesis is lacking. PATIENTS AND METHODS: A systematic review including studies with at least 50 patients with resected node-positive PDAC was conducted in PubMed, Embase, and Web of Science (inception to March 2025). Patients with node-negative disease were excluded. Primary outcome was o

Clinicopathological Factors and Nomogram Development for Predicting Lymph Node Metastasis in Locally Advanced Rectal Cancer.

BACKGROUND: Research on lymph node metastasis (LNM) in locally advanced rectal cancer (LARC) remains significantly underexplored. This study investigates the clinicopathological factors associated with LNM in LARC and develops a predictive nomogram for clinical application. METHODS: A retrospective analysis was performed on 1270 patients with LARC who underwent radical surgery between 2018 and 2023. Univariate and multivariate logistic regression analyses were conducted to identify independent predictors of LNM. A nomogram integrating these predictors was constructed and internally validated

De-escalation strategies for axillary management at primary surgery in early breast cancer: insights and implications for medical oncology practice.

The recognition that axillary surgery in early breast cancer primarily serves as a staging tool rather than a curative treatment, along with the shift from axillary lymph node dissection (ALND) to sentinel lymph node biopsy (SLNB) as the standard procedure for clinically node-negative patients, has paved the way for further de-escalation of axillary treatment. This strategy aims to reduce treatment-related morbidity while preserving survival outcomes. Multiple studies have provided compelling evidence that ALND can be safely omitted in cases of limited SLN involvement. Moreover, even the omiss

Lymphangitic breast cancer in explanted lungs with interstitial lung disease: an unexpected finding.

A 60-year-old woman with end-stage fibrosing interstitial lung disease (ILD) underwent bilateral lung transplantation. Systematic histological analysis of the explanted lungs revealed extensive lymphangitic carcinomatosis and hilar lymph node metastases from a previously undiagnosed breast carcinoma. Retrospective imaging review identified a suspicious mammographic finding that had not been further investigated. The patient was later diagnosed with metastatic breast cancer and passed away at 12 months post-transplant. This case emphasizes the challenging diagnosis of neoplasia in end-stage lun

An Interferon-Response Transcriptomic Signature of Lymphovascular Invasion in Prostate Cancer.

Lymphovascular invasion is an adverse pathologic feature in prostate cancer, but its independent molecular drivers remain unclear due to strong confounding by tumor grade and stage. We performed a confounder-adjusted transcriptomic analysis of 403 TCGA-PRAD samples. Differential expression was adjusted for Gleason score and pathological T stage. A transcriptional profile associated with LVI was derived and tested in multivariable logistic and Cox proportional hazards models for biochemical recurrence-free survival, with bootstrap internal validation. After multivariable adjustment, 129 genes w

Pelvic lymph node dissection in prostate cancer: current evidence, controversies, and future directions.

BACKGROUND: Pelvic lymph node dissection (PLND) remains controversial in the management of prostate cancer. Although it provides the most accurate pathological staging, its therapeutic value beyond staging has long been debated due to conflicting evidence and concerns regarding procedure-related morbidity. OBJECTIVE: To critically evaluate the contemporary role of PLND, particularly extended pelvic lymph node dissection (ePLND), in prostate cancer management in the context of modern imaging, risk stratification tools, and evolving oncologic endpoints. EVIDENCE ACQUISITION: A narrative review

Lack of oncological significance of prophylactic splenectomy for type 4 gastric cancer invading the greater curvature: a multicenter retrospective study.

The necessity of prophylactic splenectomy for 4 gastric cancer with greater curvature invasion remains unclear, as this subtype was excluded from the pivotal JCOG0110 trial. This study aimed to evaluate the short-term and long-term outcomes of splenectomy specifically in this population. We analyzed patients with clinically diagnosed advanced (cT2 or deeper) type 4 gastric cancer invading the greater curvature who underwent radical total gastrectomy at 11 institutes between 2005 and 2015. To minimize selection bias, inverse probability of treatment weighting (IPTW) was performed. Patients with

Mucinous adenocarcinoma of minor salivary gland with IPMN-like features.

This salivary mucinous adenocarcinoma with intraductal papillary mucinous neoplasm-like features demonstrates regional lymph node metastases and GNAS amplification. The findings suggest these lesions represent low-grade invasive mucinous neoplasms with secondary ductal extension rather than purely intraductal entities, supporting their classification within the mucinous adenocarcinoma spectrum.

Targeted axillary dissection in the primary surgery setting for node-positive breast cancer: A systematic review.

INTRODUCTION: Targeted axillary dissection (TAD) improves the accuracy of axillary staging in patients undergoing neoadjuvant chemotherapy but its role in patients with low volume nodal disease undergoing primary surgery is unclear. This study aimed to summarise the evidence of upfront TAD in the primary surgical setting in patients with node-positive breast cancer. METHODS: Searches were performed in PubMed, Embase, and Scopus to identify studies reporting the outcomes of upfront TAD in patients with node-positive breast cancer. Primary outcomes were to report procedural technique, the rate

Structure-specific involvement in advanced cervical lymph node extranodal extension predicts prognosis in nasopharyngeal carcinoma: A multi-center study.

OBJECTIVE: To further investigate the prognostic value of advanced cervical lymph node (CLN) extranodal extension (ENE) involving different structures and its potential synergistic effects with other nodal features in nasopharyngeal carcinoma (NPC). METHODS: A total of 1,373 non-metastatic NPC patients from three centers between 2011 and 2021 were enrolled. For advanced CLN ENE, involved structures, maximum axial diameter (MAD), necrosis, and bilaterality were recorded. Adjusted hazard ratios (AHRs) and random survival forests were used to identify the high-risk structure group. Kaplan-Meier

Craniofacial extramedullary plasmacytoma with hard palate and bilateral cervical lymph node involvement: a diagnostic challenge and literature review.

BACKGROUND: Extramedullary plasmacytoma (EMP) is a rare plasma cell tumor primarily affecting extramedullary sites without involving the bone marrow. While 80–90% of cases occur in the head and neck regions, craniofacial EMP involving the hard palate and spreading to bilateral cervical lymph nodes is exceptionally rare. CASE PRESENTATION: A 59-year-old female presented with a one-year history of a nasal mass on the right anterior side. Her medical history showed basal cell carcinoma and oropharyngeal tumors. Physical examination revealed a mulberry-like mass in the right nasal vestibule. Compu

Prognostic Significance of Isolated Tumor Cells and the Role of Immunohistochemistry in Nodal Evaluation in Breast Cancer: A SEER-Based Analysis and Reappraisal.

PURPOSE: The prognostic significance of isolated tumor cells (ITCs) in lymph nodes remains uncertain, and immunohistochemistry (IHC) is not consistently used for their detection. We aimed to evaluate the clinical relevance of ITCs and assess whether omission of IHC leads to nodal understaging by comparing outcomes across node-negative patients with and without IHC-confirmed negativity. MATERIALS AND METHODS: In this retrospective population-based cohort study, we analyzed SEER data from 2010 to 2015, including 127,494 women with invasive breast cancer. Nodal status was classified as N0, N0(i-
