Parathyroid function was studied in two infant sisters with primary hypomagnesemia while they were both hypomagnesemic and hypocalcemic. In one of the infants, plasma immunoreactive parathyroid hormone (iPTH) was elevated, the calcemic response to exogenous parathyroid hormone (PTH) was absent, and the phosphaturic response was normal. Restoration of serum magnesium with i.v. magnesium corrected the hypocalcemia, with no further rise of plasma iPTH. In the other infant, plasma iPTH was undetectable, and exogenous PTH produced both phosphaturic and calcemic responses. Normalization of serum mag…

Genes that render bacteria resistant to tetracycline-derived antibiotics are tightly regulated by repressors of the TetR family. In their physiologically relevant, magnesium-complexed form, tetracyclines induce allosteric rearrangements in the TetR homodimer, leading to its release from the promoter and derepression of transcription. According to earlier crystallographic work, recognition of the tetracycline-associated magnesium ion by TetR is crucial and triggers the allosteric cascade. Nevertheless, the derivative 5a,6-anhydrotetracycline, which shows an increased affinity for TetR, causes p…

Side effects of antacid therapy are dose dependant and compound related. High dose antacid intake may lead to fluid retention in the body depending on the sodium content of the different antacid preparations. Sodium bicarbonate ingestion provokes metabolic alkalosis and alkaliuria, the "nonsystemic calcium and magnesium containing antacids" cause these changes too, but to a lower degree. Urinary pH elevation favours the precipitation of calcium and magnesium salts, predisposing to renal stone formation. In patients with renal insufficiency the calcium and magnesium absorption may lead rapidly …

BACKGROUND: Among persons with type 1 diabetes mellitus (T1DM) low concentrations of magnesium have been reported. Previous (small) studies also suggested a relation of hypomagnesemia with (poor) glycaemic control and complications. We aimed to investigate the magnitude of hypomagnesemia and the associations between magnesium with parameters of routine T1DM care in a population of unselected outpatients.
METHODS: As part of a prospective cohort study, initially designed to measure quality of life and oxidative stress, data from 207 patients with a mean age of 45 [standard deviation (SD) 12] y…

The hemodynamic and the endocrine disturbances in congestive heart failure (CHF) impose major changes in electrolyte balance with a retention of sodium and concomitant losses of potassium and magnesium from the body. These changes are of great importance for the development of cardiac dysrhythmias, a diminished glucose tolerance and for the well-being of the patient. The use of conventional diuretics imposes further burdens on the already deranged electrolyte balance. On long-term therapy with conventional diuretics in CHF we observed that approximately 50% of the 297 patients studied had pota…

Hypomagnesemia and hypermagnesemia could have serious implications and possibly lead to progress from a mild form to a severe outcome of Covid-19. Susceptibility of subjects with low magnesium status to develop and enhance this infection is possible. There is little data on the magnesium status of patients with Covid-19 with different degrees of severity. This study was conducted to evaluate prevalence of dysmagnesemia in a prospective Covid-19 cohort study according to the severity of the clinical manifestations and to identify factors associated. Serum magnesium was measured in 300 of 549 pa…

Magnesium ions are essential to all living cells. As the second most abundant intracellular cation, magnesium has a crucial role in fundamental metabolic processes such as DNA and protein synthesis, oxidative phosphorylation, enzyme function, ion channel regulation, and neuromuscular excitability. After presenting an overview of magnesium homeostasis, we review the etiologies of hypomagnesemia, with an emphasis on hereditary causes.

OBJECTIVE: Since 2010, the American College of Obstetrics and Gynecology have released three committee opinions to recommend and reaffirm the utility of magnesium sulfate for neuroprotection and later for tocolysis to achieve antenatal steroid course completion in preterm labor. We sought to determine changes in antenatal magnesium sulfate exposure and other tocolytic agents for pregnancies resulting in neonatal intensive care unit (NICU)-admitted preterm infants.
STUDY DESIGN: Using the Pediatrix Clinical Data Warehouse, we evaluated all inborn infants delivered between 22 and 33 weeks' gest…

The administration of thyroxine produces a significant increase in the copper, magnesium, iron and zinc contents of liver, with a concomitant decrease of heart iron. Magnesium in heart tissues was also increased. The administration of antithyroid drugs produces a significant decrease in the concentration of the same metals in the liver with concomitant increases in kidney copper and calcium, and in heart magnesium and calcium. Brain magnesium was found to be significantly decreased after antithyroid treatment and increased to a lesser extent after thyroxine administration. Thyroidectomy did no…

BACKGROUND: It has been suggested that magnesium can be used to reduce serum calcium levels seen with hyperparathyroidism during pregnancy, thus reducing maternal and fetal risk.
CASE: A young woman presented at 32 weeks' gestation with abdominal pain from pancreatitis caused by hyperparathyroidism from a parathyroid adenoma. She was started on magnesium sulfate tocolysis for preterm labor. During treatment, serum parathyroid hormone was undetectable, but serum calcium and vitamin D-1,25 were elevated. When magnesium was discontinued, her vitamin D-1,25 was suppressed and the parathyroid horm…

Background/Objectives: Hyperkalemia is a common complication in patients receiving maintenance hemodialysis (HD). Conventional cation-exchange resins, including sodium polystyrene sulfonate (SPS) and calcium polystyrene sulfonate (CPS), effectively lower serum potassium but may non-selectively bind magnesium. Sodium zirconium cyclosilicate (SZC) is a highly selective potassium binder with minimal affinity for magnesium. This study investigated changes in serum cation profiles and nutritional status after switching from conventional cation-exchange resins to SZC in patients undergoing maintenan…

Magnesium plays a critical role in many cell functions. Hypomagnesemia may occur because of decreased intake or absorption, internal redistribution or increased loss of this element through either renal or nonrenal routes. Manifestations of magnesium deficiency include alterations in calcium, phosphate and potassium homeostasis along with cardiac disorders such as malignant ventricular arrhythmias refractory to conventional therapy, enhanced sensitivity to digoxin and, possibly, coronary artery vasospasm and sudden death. Other features of magnesium deficiency include a host of neuromuscular a…

Three children with familial hypomagnesemia from infancy were treated perorally with magnesium for 9 to 12 years. Their somatic and intellectual development have since been normal. Without therapy, the serum magnesium fell from subnormal (about 0.5 mmoles/liter) to very low values (0.2 to 0.3 mmoles/liter) within 1 to 4 wk. We observed a secondary fall in serum calcium and potassium and an increase in sodium and phosphate although serum concentrations of PTH, calcitonin, and 25-OH-vitamin D in the blood remained normal. Balance studies confirmed the presence of a defect in the intestinal absor…

Sodium, potassium, calcium, magnesium content was determined in the blood plasma, erythrocytes and the 24-hour urine. There was revealed an increased sodium content in erythrocytes, a fall of plasma potassium level, a reduction of urinary potassium excretion and of potassium-magnesium coefficient of the urine; a tendency to increase in the calcium and magnesium level was seen in erythrocytes. Only calcium content in erythrocytes became normal after the treatment.

In the cardiovascular field, coating containing copper used to catalyze NO (nitric oxide) production on non-degradable metal surfaces have shown unparalleled expected performance, but there are few studies on biodegradable metal surfaces. Magnesium-based biodegradable metals have been applied in cardiovascular field in large-scale because of their excellent properties. In this study, the coating of copper loaded in silk fibroin is fabricated on biodegradable ZE21B alloy. Importantly, the different content of copper is set to investigate the effects of on the degradation performance and cell be…

Hypomagnesemia with secondary hypocalcemia is a rare autosomal recessive disorder which manifests in early infancy with generalized seizures, other symptoms of neuromuscular irritability, and growth disturbances. Homozygous mutations in the magnesium transporter gene, transient receptor potential melastatin 6 (TRPM6), cause the disease. Here, we present an 8-month-old Turkish boy with a novel mutation of TRPM6. The patient, son of first-degree cousins, was hospitalized because of recurrent seizures and mild hypotonia. He had seizures since the newborn period and he had been treated with phenob…

Hypermagnesemia is a relatively uncommon but potentially life-threatening electrolyte disturbance characterized by elevated magnesium concentrations in the blood. Magnesium is a crucial mineral involved in various physiological functions, such as neuromuscular conduction, cardiac excitability, vasomotor tone, insulin metabolism, and muscular contraction. Hypomagnesemia is a prevalent electrolyte disturbance that can lead to several neuromuscular, cardiac, or nervous system disorders. Hypermagnesemia has been associated with adverse clinical outcomes, particularly in hospitalized patients. Prom…

The prevalence of hypomagnesemia was studied in neonates and children. The specimens were selected randomly from those submitted to the clinical chemical laboratory for blood test. A serum magnesium concentration less than 0.74 mmol/L was considered hypomagnesemic. A total serum magnesium determinations of 910 patients showed that 188 (21.7% prevalence rate) patients contained low serum magnesium levels. Frequently encountered hypomagnesemia was found among neonates with clinical conditions as diarrhea 41 (21.8%), premature births 24 (12.8%), neonatal hepatitis 20 (10.6%) and respiratory distr…

The aim of this study was to assess hair magnesium, iron, copper, and zinc levels in breast-fed and formula-fed infants. Our findings showed that concentration of the assayed macro- and trace elements, except copper, were higher in breast-fed children and declined with age, faster in formula-fed children. The frequent occurrence of magnesium deficiency indicates that pregnant women should receive magnesium supplementation.
