Prevalence of congenital anomalies in infants with in utero exposure to antiretrovirals.

BACKGROUND: Although use of efficacious interventions, including antiretrovirals (ARVs), has dramatically reduced the rate of mother-to-child transmission of human immunodeficiency virus, the safety of in utero ARV exposure remains of concern. METHODS: Data regarding 1112 infants enrolled in the International Maternal Pediatric Adolescent AIDS Clinical Trials Group protocol P1025 born between 2002 and 2007 were analyzed for this study. Congenital anomalies were classified based on the Metropolitan Atlanta Congenital Defects Program guidelines. Associations between congenital anomalies and tim

Noncardiac malformations in congenital heart disease: a retrospective analysis of 305 pediatric autopsies.

Congenital heart disease (CHD) is one of the important groups of birth defects and contributes significantly to infant mortality. Extracardiac anomalies occur in 15-45% of cases with CHD. In this retrospective study, autopsies of cases born alive and diagnosed as CHD between 1977-2002 at Hacettepe University Ihsan Doğramaci Children's Hospital, Pediatric Pathology Department, were investigated. In this period, a total of 3320 autopsies were performed and the incidence of CHD was 9.1%. The most commonly encountered CHD was ventricular septal defect (VSD) (15.3%). In 45.9% of cases, one or more

Stopping nicotine exposure before surgery. The effect on spinal fusion in a rabbit model.

STUDY DESIGN: A double-blind, prospective, randomized study using a validated rabbit model of intertransverse process fusion. OBJECTIVES: To determine the effect of stopping prolonged nicotine exposure on autogenous bone graft incorporation in a rabbit lumbar spinal fusion model. SUMMARY OF BACKGROUND DATA: There is a growing body of evidence that systemic nicotine impairs healing of spinal fusions and fractures. However, it remains to be determined whether, if nicotine increases the nonunion rate of spinal fusion surgery, stopping nicotine exposure before surgery will negate this inhibitory

Diaphragmatic rupture due to blunt trauma: morbidity and mortality in 42 cases.

We present an analysis of 42 cases of acute rupture of the diaphragm by blunt trauma. There were 31 men (74%) and 11 women (26%); the mean age was 32.8 years +/- 2.4 SEM. At admission, hypovolemic shock was present in 45% of the cases, pelvic fracture in 36%, and severe respiratory distress in 21%. Diaphragmatic rupture was suggested before operation by unilateral elevation of the diaphragm, supradiaphragmatic densities, and displacement of abdominal organs into the thorax, as shown by chest films and GI series in 18 cases (43%). The left hemidiaphragm was injured in 24 cases (57%), the right

Ultrasound for the diagnosis of gout-the value of gout lesions as defined by the Outcome Measures in Rheumatology ultrasound group.

OBJECTIVE: To evaluate ultrasound for diagnosing gout using consensus-based Outcome Measures in Rheumatology ultrasound definitions of gout lesions. METHODS: Ultrasound was performed in patients with clinically suspected gout. Joints (28) and tendons (26) were binarily evaluated for the Outcome Measures in Rheumatology gout lesions-double contour (DC), tophus, aggregates and erosions. Ultrasound assessment was compared with two reference standards: (i) presence of MSU crystals in joint/tophus aspirate (primary outcome) and (ii) ACR/EULAR 2015 gout classification criteria (secondary outcome).

Preparticipation Physical Evaluation.

Preparticipation physical evaluations are often a challenge for physicians. A recent study examined the PPE in clinical practice among pediatricians and family physicians in the state of Washington. Unfortunately, many physicians in this study perceived significant barriers to effective performance of the PPE. These barriers included uncertainty about how to perform the PPE, the relative importance of each PPE component, and the lack of a standardized approach and time for appropriate performance of the PPE. Although these concerns are shared by physicians beyond the borders of Washington, tho

Diagnostic scope in out-of-hours primary care services in eight European countries: an observational study.

BACKGROUND: In previous years, out- of-hours primary care has been organised in large-scale organisations in many countries. This may have lowered the threshold for many patients to present health problems at nights and during the weekend. Comparisons of out-of-hours care between countries require internationally comparable figures on symptoms and diagnoses, which were not available. This study aimed to describe the symptoms and diagnoses in out-of-hours primary care services in regions in eight European countries. METHODS: We conducted a retrospective observational study based on medical rec

Musculoskeletal symptoms among mobile hand-held device users and their relationship to device use: A preliminary study in a Canadian university population.

The study aims were, in a population of university students, staff, and faculty (n = 140), to: 1) determine the distribution of seven measures of mobile device use; 2) determine the distribution of musculoskeletal symptoms of the upper extremity, upper back and neck; and 3) assess the relationship between device use and symptoms. 137 of 140 participants (98%) reported using a mobile device. Most participants (84%) reported pain in at least one body part. Right hand pain was most common at the base of the thumb. Significant associations found included time spent internet browsing and pain in th

Should I Stay or Should I Go? Identifying Intrinsic and Extrinsic Factors in the Decision to Return to Duty Following Lower Extremity Injury.

INTRODUCTION: Rehabilitation research of wounded service members (SMs) commonly focuses on physical ability to return to duty (RTD) as a measure of successful recovery. However, numerous factors or barriers may influence a SM's ability and/or desire to RTD after lower extremity musculoskeletal trauma. SMs themselves as well as the clinical care team that works with them daily, often for years at a time, both offer unique perspectives on the influential factors that weigh into decisions to RTD. The purpose of this study was to identify the intrinsic and extrinsic factors patients and clinicians

Electroactive fibrous scaffolds for tendon repair: From bioelectric microenvironment engineering to motion-driven and remotely activated stimulation.

Tendon injuries rank among the most prevalent musculoskeletal disorders. Their clinical treatment remains challenging due to the inherently poor self-healing capacity and highly ordered hierarchical architecture of native tendon tissues. In recent years, electroactive fiber-based biomaterials have attracted growing attention in the field of tendon regeneration, attributed to their unique capability to mimic the fibrous microstructure of natural tissues while providing therapeutic electrical stimulation throughout tissue repair. In this review, recent advances in electroactive fibrous scaffolds

Pregnant versus non-pregnant healthy subjects - a prospective longitudinal musculoskeletal ultrasound study concerning the spectrum of normality.

UNLABELLED: Pregnancy induces profound changes in the maternal body and the normality spectrum may differ compared to non-pregnant subjects. AIMS: The main objective was to establish the presence, frequency and distribution of articular and peri-articular abnormal findings in healthy women, pregnant and non-pregnant, using clinical examination (CE) and musculoskeletal ultrasound (MSUS). Second objective was to identify which joint/tendon set would show fewer modifications in both groups of healthy subjects and would remain stable throughout pregnancy. MATERIAL AND METHODS: A prospective cros

[Treatment duration of trauma and orthopedic patients in an emergency department].

INTRODUCTION: The majority of emergency patients are admitted to hospital via the emergency department. Overcrowding in emergency departments results in dissatisfied patients, increased complication rates, and negative medicoeconomic consequences. To overcome these problems, sufficient personnel strength should be available depending on treatment duration and the patients' characteristics. MATERIALS AND METHODS: First, trauma and orthopedic patients were classified into six categories: ABT (history, findings, and therapy), RABT (X-ray and ABT), WABT (wound care and ABT), WRABT (wound care and

Effects of toxic exposure to molds and mycotoxins in building-related illnesses.

The authors studied 100 patients who had been exposed to toxic molds in their homes. The predominant molds identified were Alternaria, Cladosporium, Aspergillus, Penicillium, Stachybotrys, Curvularia, Basidiomycetes, Myxomycetes, smuts, Epicoccus, Fusarium, Bipolaris, and Rhizopus. A variety of tests were performed on all, or on subgroups of, these patients. Sensitivities and exposures were confirmed in all patients by intradermal skin testing for individual molds (44-98% positive), and by measurement of serum antibodies. Abnormalities in T and B cells, and subsets, were found in more than 80%
