Notes on currently used public health measures for sexually transmitted diseases.

Traditional public health measures for sexually transmitted diseases (STDs) have been to create laws that allow the society to investigate and treat assumed cases of STD. Such laws often include STD that in many communities are rare, but left out very common ones, e.g., genital chlamydial and human papillomavirus infection. However, such laws do not often stress the rights of infected persons to receive counseling, confidentiality, and help to notify the partner(s) if asked for. Although, free health care may be provided at venereal (STD) clinics, this is not true for many other types of clini

On the absence of correlation between responses to noxious heat, cold, electrical and ischemic stimulation.

Is a person's response to one noxious stimulus similar to his/her responses to other noxious stimuli? This long-investigated topic in pain research has provided inconclusive results. In the present study, 2 samples were studied: one using 60 healthy volunteers and the other using 29 patients with coronary artery disease. Results showed near-zero correlations between measures of heat, cold, ischemic, and electrical laboratory pains, as well as between these laboratory pains and an idiopathic pain, the latency to exercise-induced angina in the patients. Power analyses showed that the sample size

Relationships among airway reactivity, pupillary alpha-adrenergic and cholinergic responsiveness, and age.

Healthy adult volunteers (n = 122), who denied personal history of lung disease or family history of cystic fibrosis or asthma, took no interfering medications, and had forced expiratory volume in 1 s greater than or equal to 80% predicted, underwent methacholine challenge and pupillary reactivity testing. Pupil diameter measured in dark and light test conditions declined with age (Pearson's r = -0.54 and -0.36). Pupillary alpha-adrenergic responsiveness (expressed as the concentration of phenylephrine required to dilate the pupil 1 mm) was significantly correlated with age. Older subjects req

Perceptions of stressful life events and depression: a test of attributional models.

A brief review of tests of the attributional model of depression suggests that there is only weak or inconsistent support for the predicted causal ascriptions by depressed persons for negative events. Moreover, in very few studies have researchers actually tested the causal predictions of the model. Finally, because many of the studies were conducted with normal college students, the generality of the model as applied to different populations needs to be investigated. We addressed three questions: (a) the validity of the hypothesized independent and direction relation between each of the dimen

Academic Orthopaedic Surgeons Prefer Peer-Reviewed Guidelines Over Artificial Intelligence and Social Media for Medical Information.

Background and objective Patients increasingly seek recommendations for improved health and wellness, as well as medical advice, through social media and large language models (LLMs), yet the quality of these sources varies considerably. Effective clinical communication requires an understanding of how patients engage with online medical content, as these sources may shape expectations even before a clinical encounter. However, to date, no study has assessed how orthopaedic surgeons themselves use these same resources. Hence, this study aimed to evaluate orthopaedic surgeons' self-reported use

Feasibility and Acceptability of Web-Based Structured Oral Examinations for Postgraduate Certification: Mixed Methods Preliminary Evaluation.

BACKGROUND: The COVID-19 pandemic disrupted postgraduate certification examinations globally. The Colleges of Medicine of South Africa continued hosting certification examinations through the pandemic. This was achieved by effecting a rapid transition from in-person to web-based certification examinations. OBJECTIVE: This formative evaluation explored candidates' acceptability of web-based structured oral examinations (SOEs) hosted via Zoom (Zoom Communications Inc). We also reported the audiovisual quality and technical challenges encountered while using Zoom and candidates' overall experien

Prognosis of diastolic and systolic orthostatic hypotension in older persons.

BACKGROUND: Orthostatic hypotension (OH) predicts mortality in hypertensive persons with diabetes mellitus, but no increase in mortality has been found among random samples of home-dwelling persons with OH. We examined the risks of nonvascular and vascular deaths according to different definitions of OH among home-dwelling elderly persons. SUBJECTS AND METHODS: The study population consisted of all persons aged 70 years or older living in 5 rural municipalities (N=969), of whom 833 (86%) participated. Orthostatic tests were successfully carried out in 792 persons by nurse examiners. Orthostat

Discontinuing Surveillance of Low-Risk Branch-Duct Intraductal Papillary Mucinous Neoplasms in Routine Practice: An External Application of the "Trivial" Criteria and the Resources it Would Release in Fourteen Health Systems.

BACKGROUND: Surveillance of low-risk branch-duct intraductal papillary mucinous neoplasms is usually lifelong. Criteria to stop it after 5 years of stability in older patients have not been tested outside the center that devised them, nor has their economic value been measured. OBJECTIVE: To test these criteria in an independent cohort and measure the lesions missed and the resources released. METHODS: Single-center cohort of patients with a low-risk lesion at diagnosis was followed for at least 12 months with cross-sectional imaging at baseline and during surveillance. The outcome was any n

Circumstantial variables preceding firearm suicide among females with and without mobility disability in the USA: comparative analysis using data from the National Violent Death Reporting System.

BACKGROUND: Mobility disability impacts approximately 12% of the US population; females are overrepresented among persons with mobility disability. Those with mobility disability are at increased risk of suicide compared with their non-disabled counterparts. Suicide using a firearm has increased among females in the last two decades. This study aims to describe and explore significant circumstantial variables (eg, socio-demographic, health indicators) preceding firearm suicide among females with mobility disability as compared with females without mobility disability. METHODS: This is a secon

Latent tuberculosis infection screening in patients with inflammatory bowel disease: a nationwide retrospective cohort study in South Korea comparing IGRA alone versus a combination of TST and IGRA.

BACKGROUND/AIMS: We aimed to evaluate if using the interferon-gamma release assay (IGRA) alone is effective for latent tuberculosis infection (LTBI) screening in preventing active tuberculosis in patients with inflammatory bowel disease (IBD) before initiating anti-tumor necrosis factor alpha (anti-TNF-α) therapy, compared to using both the tuberculin skin test and IGRA. METHODS: Using South Korea's Health Insurance Review and Assessment Service, we selected IBD patients treated with anti-TNF-α agents for ≥ 1 year who underwent LTBI screening between 2018 and 2021. We compared the 1-year inci

Obstructive sleep apnea and polysomnographic predictors of neuropsychological performance two years after injury in a prospective cohort of adults with traumatic brain injury.

Objective: Obstructive sleep apnea (OSA) has been associated with structural and functional brain changes and cognitive impairment in sleep clinic samples. Persons with traumatic brain injury (TBI) are at increased risk of OSA compared to community samples, and many experience chronic cognitive disability. However, the impact of OSA on cognitive outcome after TBI is unknown. The purpose of this study was to examine the relationship between polysomnographic sleep parameters on neuropsychological performance 2 years following TBI. We hypothesized that oxygen desaturation, sleep fragmentation, an
