Endonasal flap reconstruction in sinonasal malignancy.

Sinonasal malignancies are a broad, yet rare, class of head and neck cancers with a poor prognosis. Surgical resection is the mainstay of tr … roviding a rich and reliable blood supply. The nasoseptal flap has become the workhorse of anterior skull base reconstruction given its relative ease of harvest, reliability, low donor site morbidity and success: CSF leaks rates have decreased from > 20% to < 5% with the use of the nasoseptal flap. This review thoroughly discusses the history, use, and techn …

Cervical spine chordoma: Imaging features and diagnostic challenges of a rare C6 localization.

Chordomas are rare malignant tumors arising from remnants of the embryonic notochord. They are slow-growing but locally aggressive. They most frequently occur in the sacrococcygeal and skull base regions, while cervical spine involvement is rare. Their proximity to critical neurovascular structures makes diagnosis and management challenging. We report the case of a 62-year-old man presenting with a progressive left cervical mass, dysphonia, and a foreign body sensation in the lower neck. Clinical examination revealed a firm, immobile cervical swelling with mild displacement of adjacent larynge…

CSF1R+ macrophage and osteoclast depletion impairs neural crest proliferation and craniofacial morphogenesis.

Despite a wealth of knowledge about the mechanisms underlying craniofacial morphogenesis during gestation, the roles of fetal macrophages and osteoclasts during this process remain less well characterized. Here, we used the pharmacological inhibitor PLX5622 to disrupt colony stimulating factor 1 receptor (CSF1R) signaling, which is essential for macrophage and osteoclast proliferation, differentiation and survival. Prenatal PLX5622 exposure in mouse resulted in ∼50% depletion of CSF1R+ macrophages, with complete loss of osteoclasts. While there were no notable changes in craniofacial nerve or

Spontaneous cerebrospinal fluid (CSF) rhinorrhea originating from clival defects is an exceptionally rare condition, with only a limited num … ho were successfully treated using an endoscopic endonasal transsphenoidal approach with multilayer skull base reconstruction. Postoperative outcomes were favorable, with no recurrence during follow-up. These cases highlight the diagnostic pitfalls of clival CSF leaks, underscore the importance of meticulous high-resolution imaging, and confirm that early en …

Follicular Thyroid Carcinoma with Cavernous Sinus Metastasis and Perineural Spread: A Rare Manifestation of Radioiodine-refractory Disease.

Cavernous sinus metastasis from follicular thyroid carcinoma (FTC) is exceedingly rare and typically indicates advanced, radioiodine-refractory disease. We report a 37-year-old man with minimally invasive FTC who was treated with total thyroidectomy, neck dissection, and cumulative radioiodine therapy totaling 720 mCi for persistent iodine-avid disease. Four years later, he developed progressive left-sided palsies of cranial nerves III, IV, and VI. Imaging revealed a cavernous sinus mass with orbital apex extension and perineural spread. Serum thyroglobulin was markedly elevated (>500 ng/mL),

Craniofacial Mature Teratoma: A Case Report.

A congenital teratoma is a rare malformation. In this case report, we present a case of a one-month-old female patient with a craniofacial teratoma that has an intracranial origin from the skull base located anterior to the sella turcica, extending to the hard palate and associated with cleft palate. The patient presented to our department with a large tumor protruding from the oral cavity that did not cause an immediate respiratory obstruction, but there were feeding difficulties noted. The diagnosis was established based on clinical and radiological aspects and confirmed by a preoperative in…

Thoracoabdominal chordoma in a pediatric patient. A rare entity.

INTRODUCTION: Chordoma is a rare, slow-growing notochordal neoplasm typical of adults. Less than 5% of the cases occur in children, where they are located at the skull base. Treatment involves surgical resection with or without radiotherapy. CLINICAL CASE: 14-month-old patient with a left dorsal lumbar mass, pain, and limited mobility in the lower limbs. MRI showed a left paravertebral mass infiltrating the spinal canal (T11-L1), in contact with the parietal pleura, along with abdominal extension. Chordoma diagnosis was established based on percutaneous biopsy. Full resection without safety m…

Nasopharyngeal Carcinoma with Spinal Cord Metastasis and Secondary Syringomyelia: A Case Report.

Nasopharyngeal cancer with central nervous system metastases is rare. True metastasis to the distal regions of the central nervous system, especially the spinal cord, is incredibly uncommon, although tumor invasion to intracranial locations through the skull base can be prevalent. We report on a 45-year-old male who had been suffering from progressive unsteady gait and numbness of lower limbs for 3 weeks. The numbness eventually ascended to the thigh area and the patient required a wheelchair. His muscle power was normal. Magnetic resonance imaging showed multiple enhancing nodular lesions in …

An accessory sphenoidal foramen of the middle cranial fossa detected on computed tomography.

BACKGROUND: The skull base depicts significant morphological variability, which is frequently described due to its neurosurgical significance. The middle cranial fossa's accessory foramen has rarely been described. MATERIALS: A 53-year-old female patient's computed tomography (CT) scan was further investigated for its unusual morphology. RESULTS: On the left-sided middle cranial fossa, an accessory sphenoidal foramen (ASF) was observed, located 3.3 mm posterior to the foramen rotundum (FR) and 5.5 mm anterior to the foramen ovale (FO). Extracranially, the ASF opened into the infratemporal fo…

Pediatric Giant-Cell Tumor of Bone With Secondary Aneurysmal Bone Cyst Causing Orbital Apex Syndrome.

Giant-cell tumors of bone (GCTBs) are benign, expansile lesions usually found in the long bones of young adults. The authors report a pediatric skull base GCTB causing cranial nerve deficits and vision loss. The patient presented with blurry vision, left eye misalignment, headaches, nausea, and vomiting. Imaging revealed a large heterogenous mass invading the anterior cranial fossa floor. Initial histopathology suggested an aneurysmal bone cyst (ABC), and the mass was surgically resected. Four months later, recurrence led to genetic testing, which identified a p.G35W H3F3A mutation, confirming…

Frontotemporal Approach for Spheno-Orbital Meningioma and Orbital Compartment Resection: Technical Case Instruction: 2-Dimensional Operative Video.

BACKGROUND AND IMPORTANCE: Spheno-orbital meningiomas (SOMs) pose a challenge to the skull base neurosurgeon because of their variable presentation and involvement of critical structures within the orbit. There is no consensus on optimal management of these patients and how to achieve maximal safe resection. The authors share an illustrative case with an accompanying video to demonstrate their aggressive approach to resect SOMs and their intraorbital components. CLINICAL PRESENTATION: A 75-year-old-woman presented with progressive vision loss and proptosis. Magnetic resonance imaging was cons…

[Malignant Transformation of Benign Meningiomas with or without Radiotherapy].

Most meningiomas are benign, slow growing tumors, which rarely progress to a higher grade. The incidence rate of malignant progression is estimated to be 2.98/1000 patient-year. However, non-skull base location is a significant risk factor for progression. The median time to malignant progression is 4.3 years; however, the cumulative rate of progression approaches a plateau after 10 years. Although radiosurgery does not appear to increase the incidence rate(0.5/1000 patient-year), exact comparisons have been difficult because of differences in study populations. The median time to progression …

A novel therapeutic pathway to the human cochlear nerve.

Traditional approaches to the human cochlear nerve have been impeded by its bony encasement deep inside the skull base. We present an innovative, minimally invasive, therapeutic pathway for direct access to the nerve to deliver novel regenerative therapies. Neuroanatomical studies on 10 cadaveric human temporal bones were undertaken to identify a potentially safe therapeutic pathway to the cochlear nerve. Simulations based on three-dimensional delineation of anatomical structures obtained from synchrotron phase-contrast imaging were analyzed. This enabled the identification of an approach to t…

Dural arteriovenous fistula onyx embolization through a non-visible previously embolized pedicle.

Cerebral dural arteriovenous fistulas (dAVFs) are commonly treated with endovascular embolization, either through arterial or venous routes in single or multi-staged procedures. Treatment outcomes depend on multiple factors, including the patient's clinical status, the anatomy of the malformation, and the operator's experience, but technical success is also highly dependent on choice of neurovascular devices and embolic agents. When transvenous routes are not feasible and the most obvious transarterial routes do not appear safe, deep knowledge of the dynamic fluid properties of liquid embolics

Lateral Rectus Muscle Entrapment in Complex Orbital Fracture in a Child.

A 6-year-old boy presented with a lateral rectus muscle entrapment in a complex orbital fracture sustained during bicycle riding. He was diagnosed with a skull-base fracture with pneumocephalus and zygomatic and inferior orbital wall fractures at another hospital. The zygomatic fracture and pneumocephalus were treated conservatively. Although a detailed ocular motility examination could not be performed due to pain and inability to open the eyelid, the inferior wall fracture was surgically repaired. Postoperatively, persistent extraocular movement limitation prompted a referral to the authors'
