Oesophageal foreign bodies in dogs.

The aim of the paper is to present own experience in diagnosis and endoscopic removal of foreign bodies from the oesophagus in dogs. The study was performed on 22 dogs of different breed and sex, aged 7 months to 17 years, with suspicion of a foreign body in the oesophagus. All the dogs were subjected to a radiography of the oesophagus and oesophagoscopy. The most commonly observed clinical signs were regurgitation and swallowing disorders. The best diagnostic method allowing detection of foreign bodies in the oesophagus was esophagoscopy.

Epidermal inclusion cyst of the soft palate and uvula in an infant.

Congenital epidermal inclusion cysts of the soft palate or the uvula are uncommon mass lesions, and there have been only limited case reports of these in the literature. Although epidermal cysts are benign in nature and develop slowly, mass lesions growing near the soft palate or the uvula can result in velopharyngeal insufficiency, affecting both speech and swallowing. We present such a condition in a 7-month-old male infant with an epidermal inclusion cyst involving both the soft palate and the uvula.

A rare cause of hematemesis in newborn: fibrocystic breast disease of mother.

Hematemesis in a healthy newborn is most often caused by swallowed maternal blood. Maternal blood due to fibrocystic breast disease in human milk has not previously been reported in the literature. We report here a newborn case with hematemesis in which the mother had fibrocystic breast disease, and we want to emphasize this rare entity. Physicians should be aware of this rare condition, and fibrocystic breast disease of the mother should be included in the differential diagnosis of newborns with hematemesis.

[Myasthenia gravis and oropharyngeal disorders: two cases reports].

OBJECTIVE: Description of two atypical clinical cases of myasthenia gravis with pharyngeal dysphagia. CASES REPORTS: A retrospective study from two cases report of myasthenia gravis with pharyngeal dysphagia. CONCLUSION: The decrease of the laryngeal ascent and the desynchronization of the upper oesophageal sphincter opening appears as the main physiopathological mechanisms. The aspirations are frequent. In front of pharyngeal dysphagia, myasthenia have to be evocated. The alteration of the pharyngeal swallowing can be inaugural in this disease. The cholinesterase inhibitors can help for the

Acute appendicitis and ileal perforation with a toothpick treated by laparoscopy.

A 69-year-old man underwent an emergency laparoscopic procedure after the acute appendicitis diagnosis has been established. Laparoscopic exploration showed inflamed appendix and perforation of terminal ileum with a swallowed part of the wooden toothpick. The treatment consisted of typical laparoscopic appendectomy and laparoscopic removal of the foreign body, followed by laparoscopic closure of the perforation site and lavage of the abdominal cavity. The postoperative course was uneventful and the patient was discharged from the hospital on day 3 after the operation.

Congenital alveolar fusion.

Congenital fusion of the jaws is rare. It may be unilateral or bilateral and may involve only the soft tissues or both the hard and soft tissues. This anomaly may be seen separately or in association with other syndromes. Congenital alveolar fusion restricts mouth opening, causing problems with feeding, swallowing, and respiration. Case 1 had membranous bands between the alveoli that required tracheostomy for stabilization, followed by osteotomy for release. Postoperatively, both patients had adequate mouth opening.

Glottis and subglottis: a thoracic surgeon's perspective.

Anatomically, the larynx presents several unique features that account for the difficulties in the surgical management of pathologies, such as benign strictures located in the area. These features include its unique cartilaginous support, its short length, its intrinsic and extrinsic musculatures, and its innervation, all of which are fundamental for the function of breathing, phonation, and swallowing. It is important that the thoracic surgeon operating on the larynx understand this particular anatomy, the limits of surgery, and most importantly the steps to be taken to avoid catastrophic com

Pharyngo-colostomy with supraglottic partial laryngectomy in caustic oropharyngeal stricture.

We present two cases of chronic caustic stricture from the oropharynx to the entire esophagus combined with laryngeal stricture in which the piriform sinuses were stenosed. The restoration of digestive continuity was accomplished by end-to-end pharyngo-colostomy at the level of the cricoid cartilage through the posterior mediastinal route. Supraglottic partial laryngectomy and pharyngoplasty were done simultaneously for the laryngeal and oropharyngeal stricture. The patients were able to swallow a soft blend diet to regular diet with negligible penetration.

Speech and language therapists in palliative care: what do we have to offer?

Speech and language therapists (SLTs) are specialists in the management of communication and swallowing disorders (dysphagia); however, few are members of acute and community specialist palliative care teams in the UK. Although numbers of SLTs working in palliative care are increasing, there is a need for education and research to support service development and promote understanding of their role. This profile piece outlines what SLTs have to offer patients with palliative care needs and describes how their role in palliative care is increasing in the UK.

Actinomycotic abscess of the thyroid gland in an infant.

Actinomycotic abscess of the thyroid gland is rare especially in childhood. In this article, we report the second pediatric case in literature, at age of 18 months. Although spread of actinomycotic infection to the thyroid gland from upper aerodigestive tract by preformed pathways such as piriform sinus fistula seems rational, it was demonstrated neither in current nor in previous cases. Therefore, further work-up such as barium swallow, which is onerous to perform in a child may be reserved for recurrent cases.

Wound botulism among black tar heroin users--Washington, 2003.

During August 22-26, 2003, four injection-drug users (IDUs) in Yakima County, Washington, sought medical care at the same hospital with complaints of several days of weakness, drooping eyelids, blurred vision, and difficulty speaking and swallowing. All four were regular, nonintravenous injectors of black tar heroin (BTH), and one also snorted BTH. This report summarizes the investigation of these cases, which implicated wound botulism (WB) as the cause of illness.

Persistent foreign body sensation and pharyngeal pain due to retention of staples: an interesting sequelae of endoscopic stapling procedure.

The surgical treatment of a pharyngeal pouch with endoscopic stapling diverticulotomy is a relatively new concept. Long-term results and complications are yet to be fully studied. We describe a patient who developed persistent pharyngeal pain and foreign body sensation due to retention of a clump of staples at the cricopharyngeal sphincter. This complication has not been reported before. This case highlights the need for repeat endoscopy rather than a barium swallow X-ray if the patients are symptomatic after stapling procedures.

Successful thoracoscopic surgical treatment of oesophageal cyst.

The authors report a video-thoracoscopically successfully treated case of oesophageal cyst. The symptomatic lesion was diagnosed by swallowing, X-ray, oesophagoscopy, chest CT scan and endoscopic ultrasonography. The benign tumour was removed by a videothoracoscopic method using selective intubation. There were no intraoperative and postoperative complications. The patient was discharged on the fifth postoperative day. The videothoracoscopic technique is safe, involves little pain and permits a rapid return to normal activity. It is a preferred method for removing benign lesions of the oesopha

[Primary adenocarcinoma of the duodenum. Study of three cases and review of the literature].

The authors report three cases of primary adenocarcinoma, two concerning the duodenum and the third concerning the duodenojejunal junction. These rare tumours are characterized by polymorphic and non specific clinical signs. Diagnosis is suspected by upper gastrointestinal fibroscopy and barium swallow, and confirmed by biopsies. Prognosis varies according to histological differentiation and lymph node extension. Whipples operation is the sole curative procedure in the absence of local lymph node involvement. Cases of extensive invasion are managed by bilio-digestive by-pass.

Oral stent as treatment adjunct for oral submucous fibrosis.

Oral submucous fibrosis is a chronic inflammatory disease that results in progressive juxtaepithelial fibrosis of the oral soft tissues that can cause increasing difficulty in chewing, swallowing, speaking, and mouth opening. Many treatment regimens for oral submucous fibrosis have been proposed to alleviate the signs and symptoms of the disorder. In severe cases, surgical intervention is the only treatment modality, but relapse is a major problem. This article describes the use of an oral stent as an adjunct to surgery to prevent relapse.

Supraglottic transection: results of repair and functional outcome.

We present two cases of supraglottic transection, with concomitant neurotemesis of the superior laryngeal nerve. Both cases were repaired primarily and a temporary tracheostomy was used to protect the airway. In both cases subsequent decannulation was achieved within four weeks, following the initial injury. No long-term swallowing or voice disturbance was noted in either case. We recommend that in such cases it is unnecessary to repair the nerves directly, but that careful repair of the wound itself will achieve a functional result.

Opitz BBBG syndrome: new family with late-onset, serious complication.

The Opitz BBBG syndrome is characterized by hypertelorism and (in male patients) hypospadias, in addition to a number of midline abnormalities: posterior laryngeal cleft, stridor, swallowing dysfunction, cardiac defects, imperforate anus, and urinary tract and CNS anomalies. Inheritance is autosomal dominant (McKusick number *145410) with partial male sex limitation in most pedigrees. We report a Dutch family with Opitz BBBG syndrome in which the proband developed late-onset symptoms of a structural laryngeal abnormality.

Multiple cranial neuropathies associated with leprosy.

We report on a Samoan man with dysphagia, voice hoarseness, facial erythema, and edema. Neurologic examination revealed hypesthesia at the site of the facial rash, enlarged auricular nerves, a right facial palsy, decreased gag reflexes, and voice hoarseness. Laryngoscopic examination showed paralysis of the left vocal cord, and a barium swallow revealed a possible compressive lesion. A skin biopsy specimen was diagnostic of tuberculoid leprosy. This patient has an unusual case of leprosy with multiple cranial neuropathies.

Gastrostomy without laparotomy--a percutaneous endoscopic technique: review of 35 cases.

Percutaneous endoscopic gastrostomy (PEG) was performed in 33 patient with head injury, one with laryngeal cancer, and one with gastric volvulus. The gastrostomy tube was prepared from 20 F Foley catheter and a plastic micropipette tip. The complications encountered included peritubal leak in three patients (9%) and abdominal wall hematoma in one patient (3%). There was no procedure-related mortality. We recommend PEG for tube enteral feeding in patients who have lost the swallowing reflex.

[Glossopharyngeal neuralgia].

Glossopharyngeal neuralgia, such less common than trigeminal neuralgia, is characterized by paroxysm of unilateral pain with sudden onset radiating from the oropharynx and base of the tongue, to the ear. Often precipitated by swallowing the glossopharyngeal neuralgia is occasionally accompanied by bradycardia or syncope. Conservative treatment consists of anticonvulsant drugs. Surgical treatment consists of section of the rootlets of the ninth and, in part, tenth cranial nerves. In some selected patients microvascular decompression without section of rootlets is also been made.
