Irreducible Galeazzi Fracture-Dislocations.

BACKGROUND: Fractures of the radial shaft with disruption of the distal radial ulnar joint (DRUJ) or Galeazzi fractures are treated with reduction of the radius followed by stability assessment of the DRUJ. In rare instances, the reduction of the DRUJ is blocked by interposed structures requiring open reduction of this joint. The purpose of this study is to review all cases of irreducible Galeazzi fracture-dislocations reported in the literature to offer guidelines in the diagnosis and management of this rare injury. METHODS: A search of the MEDLINE database, OVID database, and PubMed databas

Magnetic resonance imaging of the shoulder in asymptomatic professional baseball pitchers.

The purpose of this study was to evaluate the magnetic resonance imaging findings in both shoulders of asymptomatic professional baseball pitchers. Fourteen pitchers who were without significant prior injury underwent a blinded clinical assessment and magnetic resonance imaging of both shoulders. All images were interpreted by two experienced musculoskeletal radiologists. The appearance of the rotator cuff tendons was graded, with additional evaluation of the biceps, labrum, and osseous structures. Ten athletes were found to have stable shoulders and painless full range of motion. Clinically,

A new model for experimental tendon adhesions in the chicken.

A minimally invasive model using a manual abrader to induce adhesions in the chicken's central digit is described. The flexor synovial sheath and the profundus tendon were abraded with access through small flaps at the level of the proximal and distal phalanges of the avian long toes. The birds were divided into two groups according to the severity of the induced trauma. Group I birds received an abrasion injury and were euthanized to allow biomechanical testing 5 weeks postoperatively. Group II birds had a more severe abrasion and were euthanized similarly and tested 5 weeks after surgery. Re

Arthroscopic Bicortical Biceps Anchorage: An Arthroscopic-Assisted Technique for Subpectoral Biceps Tenodesis.

Long head of the biceps tendon pathology is a frequent cause of anterior shoulder pain. Tenotomy is technically simple but may lead to cosmetic deformity and muscle cramping, whereas tenodesis better preserves the biceps contour but involves a more complex procedure. Multiple surgical techniques and fixation methods are available. No single technique has emerged as clinically superior, and no gold standard has been established. Among the various techniques and fixation sites, subpectoral placement within cancellous bone has been proposed as a favorable option for long head of the biceps tendon

Thumb boutonnière deformity without rheumatoid arthritis or trauma.

UNLABELLED: Boutonnière deformity of the thumb without rheumatoid arthritis or trauma is not widely recognised. This study aimed to investigate its prevalence, relation to sex and age, and identifying factors associated with the extensor mechanism using ultrasonography. We examined 248 thumbs from 127 participants who were asymptomatic for hand disorders and had no history of hand injury. Boutonnière deformity was identified in 20 thumbs of 17 participants and was significantly more prevalent among elderly participants than among young participants. No significant differences in sex or hand do

Posterior Deltoid-to-Triceps Tendon Transfer for Elbow Extension in a Tetraplegia Patient: A Case Report.

In tetraplegia patients, activities of daily living are highly dependent on the remaining upper limb functions. In other countries, upper limb reconstruction surgery to improve function has been applied to diverse cases, but few cases have been reported in Korea. The current authors experienced a case of posterior deltoid-to-triceps tendon transfer and rehabilitation in a complete spinal cord injury with a C6 neurologic level, and we introduce the case-a 36-year-old man-with a literature review. The patient's muscle strength in C5 C6 muscles were normal, but C7 muscles were trace, and the Spin

Posterior ankle arthroscopy portal safety regarding proximity to the tibial and sural nerves.

The aim of this cadaveric study was to assess the relative safety of posterior ankle arthroscopy portal sites regarding their distance from the tibial and sural nerves. We dissected 20 embalmed cadaveric lower limbs, carefully exposed the nerves, preserving their original position, and established the entry points of five posterior ankle portals using pins. We measured distances with a digital calliper and used Friedman test and Wilcoxon Signed Ranks tests for statistical analyses. There was unequal safety between the five portals (p = 0.00001). There was no statistically significant differenc

Evaluating Failure Rates of Primary Quadriceps Tendon Repairs Across Different Surgical Techniques.

BACKGROUND: Quadriceps tendon ruptures occur infrequently in the general population. Biomechanical data suggest advantages with the use of suture anchor fixation for major tendon repair. Clinical studies of quadriceps tendon repair have been limited to small case series. PURPOSE: To evaluate clinical failure following primary quadriceps tendon repair with transosseous tunnel or suture anchor repair. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: The Military Health System Data Repository was queried to identify all adult patients who underwent primary quadriceps tendon repair in

Incidence and Risk Factors of Acute Patellar Tendon Rupture, Repair Failure, and Return to Activity in the Active-Duty Military Population.

BACKGROUND: Patellar tendon ruptures have a reported incidence of 0.68 per 100,000 person-years in the general population. The epidemiology of surgically treated patellar tendon ruptures in the US military has yet to be reported, which would provide opportunity for identification of risk factors for these otherwise healthy and active patients. PURPOSE: To determine the incidence of patellar tendon rupture in the Military Health System (MHS) population and to analyze demographic patterns, surgical fixation methods, and rerupture rates. STUDY DESIGN: Case-control study; Level of evidence, 3.

Extended Reverse Dorsal Metacarpal Artery Flap: A Case Report.

We present the case of a 77-year-old male agricultural worker with poorly controlled type 2 diabetes mellitus (HbA1c 9.2%) who experienced a dorsal middle phalanx injury. He was otherwise healthy, with no history of hypertension, peripheral vascular disease, or coronary artery disease. The patient was a non-smoker and was not on anticoagulants, antiplatelet agents, or immunosuppressive drugs at the time of injury. He sustained a dorsal middle phalanx injury with extensor tendon exposure caused by a rotary grinder. An extended reverse-flow dorsal metacarpal artery (DMA) flap based on the second

Internal Fixation of Displaced Intra-articular Fractures of the Hallux Through a Dorsomedial Approach: A Technical Tip.

UNLABELLED: Phalangeal fractures of the toes represent common injuries of the forefoot. In the hallux, most fractures occur at the distal phalanx and frequently result from a direct crushing type of injury. Intra-articular fractures of the hallux are usually treated nonoperatively, except when the fragments are displaced and the joint is incongruent. Displaced fractures treated nonoperatively can result in degenerative arthritis of the interphalangeal joint, causing pain and range of motion limitation, hindering gait and weightbearing. The aim of this study was to present an option of operativ

"Fight-bite": not just a hand problem.

Human bite wounds around the knee are rarely seen, yet may require the same urgent attention as a fight bite to the hand. Two cases of polymicrobial septic arthritis of the knee secondary to a human bite wound are described. In both the cases, the diagnosis of the septic arthritis was delayed because the intra-articular wound was unrecognized. The injuries were initially deemed superficial and managed with local wound care. In each case, the knee was flexed at the time of injury and the quadriceps tendon was penetrated by a tooth which inoculated the knee joint. Septic arthritis of the knee pr

Elbow lateral collateral ligament injuries.

The lateral collateral ligament (LCL) of the elbow is a complex capsuloligamentous structure critical in stabilizing the ulnohumeral and radiocapitellar articulations. LCL injury can result in elbow instability, allowing the proximal radius and ulna to externally rotate away from the humerus as a supination stress is applied to the forearm. Elbow dislocation is the most common cause of LCL injury, followed by iatrogenic injury. LCL pathology resulting in late recurrent instability is rare but disabling. The diagnosis requires a high index of suspicion, detailed history, and focused physical ex

[Trauma of the bones of the hand and wrist].

Fractures of the distal radius are common counting for 17 percent of all fractures. With conventional radiography they can be classified readily. Magnetic resonance imaging (MRI) is a suitable technique for the detection of occult forms of fractures and of associated soft tissue injuries. In the wrist, fractures are 10 times less common than in the distal radius, with the scaphoid bone to be involved most commonly. Indications for computed tomography (CT) are complex fractures, occult fractures, or dislocations. MRI should be performed to evaluate the soft tissues, to detect occult fractures,

Hallucal sesamoid pain: causes and surgical treatment.

The hallucal sesamoids, although small and seemingly insignificant, play an important role in the function of the great toe by absorbing weight-bearing pressure, reducing friction, and protecting tendons. However, the functional complexity and anatomic location of these small bones make them vulnerable to injury from shear and loading forces. Injury to the hallucal sesamoids can cause incapacitating pain, which can be devastating to an athlete. Although traumatic injuries usually can be diagnosed easily, other pathologic conditions may be overlooked. Careful physical and radiologic examination

Anatomic single vs. double-bundle ACL reconstruction: a randomized clinical trial-Part 1: clinical outcomes.

PURPOSE: Compare clinical outcomes of anatomic single-bundle (SB) to anatomic double-bundle (DB) anterior cruciate ligament reconstruction (ACLR). It was hypothesized that anatomic DB ACLR would result in better International Knee Documentation Committee Subjective Knee Form (IKDC-SKF) scores and reduced anterior and rotatory laxity compared to SB ACLR. METHODS: Active individuals between 14 and 50 years of age that presented within 12 months of injury were eligible to participate. Individuals with prior injury or surgery of either knee, greater than a grade 1 concomitant knee ligament injury

Increased risk of osteoarthritis after anterior cruciate ligament reconstruction: a 14-year follow-up study of a randomized controlled trial.

BACKGROUND: The reported prevalence of radiological osteoarthritis (OA) after anterior cruciate ligament (ACL) reconstruction varies from 10% to 90%. Purpose/ HYPOTHESIS: To report the prevalence of OA after ACL reconstruction and to compare the OA prevalence between quadrupled semitendinosus tendon (ST) and bone-patellar tendon-bone (BPTB) grafts. The hypothesis was that there would be no difference in OA prevalence between the graft types. The secondary aim was to study whether patient characteristics and additional injuries were associated with long-term outcomes. STUDY DESIGN: Randomized

A Bear of a Case: Multidisciplinary Reconstruction Following a Grizzly Bear Mauling.

Grizzly bear attacks are rare but often result in devastating craniofacial injuries requiring staged, multidisciplinary reconstruction. A 61-year-old male sustained extensive facial trauma, including complete lower lip avulsion and anterior mandibular loss, after a grizzly bear attack. He underwent early stabilization followed by virtual surgical planning-guided fibula free flap reconstruction with immediate dental implants, and a radial forearm free flap incorporating palmaris tendon to restore oral competence. The patient's postoperative course was uncomplicated and he was decannulated and d

Osteopathic Manipulative Treatment for a Chronic Rotator Cuff Tear: A Case Report.

Rotator cuff tears, particularly involving the supraspinatus muscle and/or tendon, are highly prevalent among individuals engaged in repetitive shoulder motions. Occupations demanding constant and repetitive shoulder movements are especially susceptible to rotator cuff injuries, potentially leading to prolonged joint wear and tear and an increased likelihood of joint replacement. Considering the impact of social determinants of health, including access to healthcare and socioeconomic status, it is imperative to explore conservative treatment modalities that alleviate financial burdens and redu
