Health in the central and eastern countries of the WHO European Region: an overview.

The enormous social, political and economic changes that began in the CCEE/NIS in the late 1980s included the revelation and public discussion of a widening health gap between these countries and the other Member States of the European Region. The continuing economic problems and their effects on health increase the urgency of the need for assistance from the international community. Diverging trends in life expectancy became evident in the mid-1970s, and the gap continued to widen in the 1980s for all major causes of death, particularly cardiovascular diseases. The situation is worse in the N

Neonatal symptoms in medium chain acyl coenzyme A dehydrogenase deficiency.

Medium chain acyl coenzyme A dehydrogenase (MCAD) deficiency has not been thought to be associated with significant neonatal symptoms. To determine the validity of this, all known MCAD cases from New South Wales were reassessed. A total of 16 confirmed and three presumed cases has been identified in New South Wales, from 15 families. The casenotes of patients were reviewed, and where possible the mothers interviewed, either directly or by telephone, to obtain information about neonatal events. Six of the 16 confirmed cases had significant neonatal symptoms, with onset from 17 hours to 3 days o

Health Services Usage in Patients Receiving Buprenorphine for Opioid Use Disorder or Long-Term Opioid Therapy for Chronic Pain: Retrospective Cohort Study.

BACKGROUND: Patients using buprenorphine for opioid use disorder (OUD) or long-term opioid therapy for chronic pain are at risk for poor outcomes if care is interrupted. Both treatments are highly regulated, with prepandemic requirements for in-person care. COVID-19 may have resulted in barriers to accessing in-person care through disruptions in care delivery. However, there were also opportunities for improved access to telemedicine visits through policy changes. OBJECTIVE: This study aims to evaluate changes in health care and telemedicine use during the COVID-19 pandemic among patients usi

Early discharge of premature infants. A critical analysis.

Although significant advances in the medical management of acutely ill preterm infants have resulted in unprecedented rates of survival, issues surrounding the convalescent care, discharge preparation, and readiness of parents or other caregivers have been less well studied and represent the art of medicine. Recent consensus statements provide a degree of content validity; however, important areas of scientific inquiry remain. Much is left to understand about the pathophysiology, management, and outcomes of apnea, bradycardia, and oxygen desaturation episodes continuing at term. Why do the mos

Skin-to-skin contact improves sleep stability in preterm infants.

BACKGROUND: Sleep supports neurophysiological maturation in preterm infants, yet the impact of developmental care interventions on sleep remains poorly quantified. Skin-to-skin contact (SSC) improves physiological stability, but its effects on sleep structure measured with automated methods are unclear. We hypothesized that SSC increases sleep duration and reduces sleep fragmentation. METHODS: In this prospective observational study (September 2024-May 2025), clinically stable preterm infants born at 29 + 0 to 33 + 6 weeks' gestation were enrolled in a level III Neonatal Intensive Care Unit.

Sleep Quality and Pain Empathy Among Nursing Interns: The Serial Mediating Roles of Burnout and Moral Identity.

BACKGROUND: Empathy for patients' pain is a core component of compassionate nursing care and is increasingly recognised as important for care quality and workforce sustainability. Nursing interns commonly experience poor sleep quality and burnout during clinical placement, yet the interrelationships between these factors and pain empathy remain unclear. AIM: To examine the association between sleep quality and pain empathy among nursing interns and the potential serial mediating roles of burnout and moral identity in this association. DESIGN: A multicentre cross-sectional study. METHODS: A

Hospitalization costs associated with 30-day unplanned readmissions and potentially preventable readmissions in US older patients with Alzheimer's disease and related dementias.

BACKGROUND: Older patients with Alzheimer’s disease and related dementias (ADRD) face high rates of unplanned hospital readmissions, many of which may be avoidable through better care coordination and post-discharge support. This study aims to examine the costs associated with 30-day unplanned readmissions and potentially preventable readmissions (PPRs) among US older patients with ADRD. METHODS: This study utilized the 2022 Nationwide Readmission Database to identify index admissions, 30-day unplanned readmissions, and PPRs in patients ≥ 65 years with ADRD, following the criteria established

Obesity and Metabolic Complications in Cystic Fibrosis.

OBJECTIVE: Cystic fibrosis (CF) is an autosomal recessive disorder caused by mutations in the CF transmembrane conductance regulator gene, resulting in impaired chloride and bicarbonate transport across epithelial cells. This defect results in multisystem involvement, characterized primarily by chronic pulmonary infections, obstructive lung disease, and respiratory failure, alongside extrapulmonary manifestations including pancreatic insufficiency, CF-related diabetes, and liver disease. Historically, malnutrition represented a major clinical challenge due to chronic catabolism, increased ener

Fellowship at Home: A Model for Expanding Neurosurgical Capacity in Low- and Middle-Income Countries.

BACKGROUND AND OBJECTIVES: Short-term global neurosurgical missions and off-site fellowships rarely achieve durable independence in low-resource settings. Sustainable capacity building requires longitudinal, locally embedded training that integrates technical skill with system development. The objective of this study was to evaluate whether a locally embedded "Fellowship at Home" model could establish sustainable independent cerebrovascular surgical capacity and identify factors associated with success. METHODS: We established a longitudinal partnership (2022-present) between Brigham and Wome

Comparing paper Letters in addition to Emailed Audit and feedback in Refining Asthma treatment to Improve clinical and environmental Results in primary care through a cluster randomised controlled trial: the CLEAR AIR study.

BACKGROUND: Suboptimal use of preventer inhalers and salbutamol reliever overprescribing are associated with preventable asthma deaths and are a major source of primary care carbon emissions. Audit and feedback produces modest behaviour change by assessing clinical performance and delivering feedback to encourage improvement. Although feedback is increasingly delivered digitally, clinicians may respond more to additional printed feedback reports. We evaluated whether combined digital and paper feedback was more effective than digital-only feedback in promoting safer and greener asthma prescrib

Evaluation of the Emergency Care Preparedness of Frontline Junior Doctors: A Training Needs Assessment in Ghana.

Introduction In low- and middle-income countries (LMICs) like Ghana, junior doctors - house officers and medical officers - serve as the primary frontline providers of emergency care, often in resource-limited settings. Despite their critical role, emergency medicine (EM) training in undergraduate and housemanship curricula remains non-standardized. This study conducted a bottom-up training needs assessment to identify clinical and procedural gaps among junior doctors in Ghana. Methods A cross-sectional digital survey was conducted among 75 junior doctors (house officers and medical officers w

Sexual rehabilitation across the cystectomy care pathway: pragmatic guidance for multidisciplinary teams; a narrative review.

BACKGROUND: Radical cystectomy with urinary diversion can substantially affect sexual function, body image, and intimate relationships. Structured sexual rehabilitation guidance that is specific to cystectomy and usable within nursing and rehabilitation pathways remains limited, especially for women and for patients living with urinary diversion. OBJECTIVE: To develop a practice-oriented, evidence-informed and consensus-based framework for sexual rehabilitation across the cystectomy care pathway. METHODS: We conducted a targeted narrative review of literature published between January 2000 a
