Hyperoxia for accidental hypothermia and increased mortality: a post-hoc analysis of a multicenter prospective observational study.

BACKGROUND: Supraphysiologic oxygen administration causes unfavorable clinical outcomes in various diseases, including traumatic brain injury, post-cardiac arrest syndrome, and acute lung injury. Accidental hypothermia is a critical illness that reduces oxygen demands, and excessive oxygen is likely to emerge. This study aimed to determine whether hyperoxia would be associated with increased mortality in patients with accidental hypothermia. METHODS: A post-hoc analysis of a nationwide multicenter prospective observational study (ICE-CRASH study) on patients with accidental hypothermia admitt

Fatal head injuries in children under the age of 5 years in Pretoria.

The incidence of fatal injuries in children has been reported to be highest among children aged 1 to 4 years. Major causes of head injury include road traffic accidents, falls, and intentional or inflicted injury (such as nonaccidental injury syndrome). This study reviewed the profile of children (under 5 years of age) who had been admitted to a large urban medicolegal mortuary (in Pretoria, the capital city of South Africa), after having suffered fatal head injuries. This study was conducted over a 5-year period (from January 2004 through December 2008), and a total of 107 cases were identifi

Assessment of injury potential in pediatric bed fall experiments using an anthropomorphic test device.

Falls from beds and other furniture are common scenarios provided to conceal child abuse but are also common occurrences in young children. A better understanding of injury potential in short-distance falls could aid clinicians in distinguishing abusive from accidental injuries. Therefore, this study investigated biomechanical outcomes related to injury potential in falls from beds and other horizontal surfaces using an anthropomorphic test device representing a 12-month-old child. The potential for head, neck, and extremity injuries and differences due to varying impact surfaces were examined…

Traumatic deaths in Jamaica: a coroner's (medico-legal) autopsy study from the University Hospital of the West Indies.

This study reviewed the trauma-related deaths in a coroner's (medico-legal) autopsy series at the University Hospital of the West Indies and represents only the second such study reported from Jamaica. The autopsy protocols of all coroner's autopsies performed during the 15-year period January 1, 1983 to December 31, 1997 were reviewed retrospectively, and the clinico-pathological characteristics of trauma-related deaths were analysed. Trauma accounted for 470 (28.7%) of the 1,640 coroner's autopsies and the causes of death in descending order of frequency were motor vehicle accidents [MVAs] (

[Occupational injuries with risk of transmission of blood-borne pathogens. A study of a 2-year material from the Holstebro Central Hospital].

In order to illuminate causes of accidental occupational exposure of the house staff at the Central Hospital of Holstebro (CHH) to the potential hazards of HIV- and HBV-transmission, all cases of reported injuries occurring in 1991-1992 have been analysed. Needleprick lesions were found to account for the great majority of the reported cases (71%). Sixty-six percent of all injuries were self-inflicted during medical procedures, while 34% were caused by another person, either directly (simultaneous contact of two persons with the contaminated sharp object) or indirectly as a result of carelessn

Epidermolysis bullosa misdiagnosed as child abuse. A report of 3 cases.

Three cases of epidermolysis bullosa are reported; the typical skin lesions were misdiagnosed as non-accidental injury to the children. Awareness of the manifestations of this uncommon genodermatosis, as well as the wearing of identifying Medic Alert discs, should prevent this inappropriate diagnostic stigmatization.

Unexpected death of an infant suffocated in the course of breastfeeding when the mother fell asleep.

Death following accidental suffocation due to overlaying is often overlooked, and still attributed to Sudden Infant Death Syndrome (SIDS). We report a case of sudden infant death following accidental suffocation due to breast overlaying during breastfeeding. We report the death of a two-month-old male baby, without medical history, who was found lifeless wedged under his mother's breast. The mother subsequently admitted that she had fallen asleep while breastfeeding and awoke a few minutes later to find him wedged under her breast and not breathing. External examination of the infant's body re

Hypothermia among resort skiers: 19 cases from the Snowy Mountains.

Even in relatively temperate environments, accidental hypothermia is a potentially lethal complication of exposure. We have reviewed our experience of accidental hypothermia among recreational alpine skiers at an Australian resort during the 1983 and 1984 seasons. There were 19 cases of accidental hypothermia, which occurred in 10 men and nine women who were aged between six and 47 years (mean age, 15.9 years) and who had rectal temperatures that ranged from less than 35 degrees C to 36 degrees C. The temperature at presentation to the Ski Injury Clinic was less than 35 degrees C in seven case

Injury patterns and the role of tendons in protecting neurovascular structures in wrist injuries.

PURPOSE: The purpose of this study was to evaluate the anatomical features of injured structures, investigate the protection provided by the specific tendon of each corresponding important neurovascular structure (radial artery, median nerve, and ulnar nerve/artery) and to compare the results among the three categories of wrist injuries. METHODS: This study included 114 patients who underwent primary repair for damaged wrist structures; 40 patients sustained accidental damage without intention (group 1), 40 had self-inflicted damage (group 2), and 34 patients had a stab or penetrating wound c

Handedness as a predictor of increased risk of knee, elbow, or shoulder injury, fractures and broken bones.

Several recent studies suggest that left-handers have higher susceptibility to accidental injury. Two studies investigated the suggested link between sinistrality and accidental injury by assessing the prevalence of joint problems, bone breaks, and fractures, which may be the sequelae of such mishaps. Study 1 used retrospective responses on medical history checklists completed by 1064 males on active duty with the United States Air Force. It found that left-handers were at significantly higher risk for knee problems (RR = 2.74), elbow and shoulder problems (RR = 3.03), and also swollen and pai

External causes of death in younger than 18 years old in Portugal in the last 10 years - a retrospective analysis.

Pediatric mortality from external causes has been a worldwide concern in the last decades. In particular, the prevalence of accidental deaths is a key concern, especially traffic accidents. This is retrospective study based on autopsy reports of violent deaths in individuals younger than 18 years from 2014 to 2023 in Portugal, aimed at providing valuable insight in order to help formulate preventive strategies. There were 554 pediatric deaths due to exogenous causes, with a predominance of males (68,95%). Adolescents were the most prevalent age group. The leading cause of death was land transp

Severe Acute Subdural Hemorrhage in a Patient With Glutaric Aciduria Type I After Minor Head Trauma: A Case Report.

Glutaric aciduria type I is a rare metabolic disorder caused by deficiency of glutaryl-coenzyme A dehydrogenase. Chronic subdural hematomas have been reported in glutaric aciduria type I and are considered as important differential diagnosis of nonaccidental head trauma. However, chronic subdural hematomas are usually thought to remain clinically silent in these patients. Here we report on a hitherto asymptomatic glutaric aciduria type I patient who developed severe, acute subdural hemorrhage after minor accidental head injury at age 23 months. Computed tomography confirmed significant mass ef

Trends in injury-related mortality among residents of Jiangsu Province from 2012 to 2021: an age-period-cohort analysis.

OBJECTIVE: We investigated the temporal trends and examined age-, period-, and cohort-specific effects of injury-related deaths among residents in Jiangsu to provide evidence for future injury prevention. METHODS: This study included 406,936 injury deaths from the Jiangsu provincial population death registration system. The average annual percent change (AAPC) in age-standardized mortality rates (ASMRs) was analyzed using joinpoint regression. Age-period-cohort models were generated to explore the effects of age, period, and birth cohort effects on mortality risk. RESULTS: ASMRs for all inju

An unusual case of symmetric quadruple limb fractures.

A 19-year-old man had an accidental fall from the 2nd floor and sustained multiple injuries. On radiological evaluation, the patient had symmetrical quadruple limb involvement with bilateral humerus shaft, bilateral olecranon, bilateral femur shaft, and bilateral patella fractures. The patient was actively managed using damage control orthopaedics, and a sequence of skeletal fixation was planned based on the hemodynamic stability and periodical serum lactate assessment. Symmetrical quadruple limb fractures are very rare, which could be associated with higher mortality. A meticulous clinical ev

Bullet in the pharynx: endoscopic management.

Gunshot wounds of the neck are diagnostically and therapeutically challenging cases and treatment should be individualized and multidisciplinary. We present an unusual case of bullet injury to the neck. A fourteen year old male patient sustained an accidental bullet injury to the submental area of the neck, with no serious injuries to the vital structures. The bullet was found close to the epiglottis, embedded in the pharynx between the base of the tongue and the lower pole of the left tonsil. The patient underwent successful endoscopic removal with no serious postoperative complications. In t

Usefulness and limitations of postmortem computed tomography in forensic analysis of gunshot injuries: Three case reports.

Gunshot injury has always been an important field of investigation in postmortem forensic radiology. The localization and retrieval of the bullet and of potentially important fragments are vital to these cases. Using postmortem multidetector-row computed tomography (MDCT) prior to forensic autopsy, we sought to illustrate the importance of this modality in the noninvasive characterization of gunshot wounds. We obtained and analyzed MDCT images in three cases of gunshot wounds (accidental close-range shotgun shooting, suicidal contact gunshot to the head and accidental long-range buckshot shoot

The significance of early panendoscopy in caustic ingestion in children.

Fourteen cases of esophageal and gastric burns following the accidental ingestion of a caustic substance are presented. Early fiberoptic endoscopy was necessary, for it made possible a precise assessment of the severity and extent of injury and the application of proper conservative therapy. Signs and symptoms of caustic burns in children are an unreliable guide to injury, therefore, panendoscopy should be performed as soon as possible. A delay in the performance of a fiberoptic examination could result in the perforation of the affected organs.
