Correlation of angiographic circulation time and cerebrovascular reserve by acetazolamide-challenged single photon emission CT.

BACKGROUND AND PURPOSE: Although cerebral circulation time (CCT) is one of the main parameters in cerebral blood flow measurements, its clin … the contralateral side was calculated as the rACCT ratio. To estimate the cerebrovascular reserve, acetazolamide-challenged single photon emission CT was used. Patients with a decreased cerebrovascular reserve were defined as the "poor reserve" group, and those without a decrease were defined as the "normal reserve" group. The ratio of the radioactivity count …

Acetazolamide-enhanced neuroSPECT scan reveals functional impairment after minimal traumatic brain injury not otherwise discernible.

A patient suffering from aggressive personality changes and cognitive impairment following head trauma, without neurological or anatomical imaging findings, underwent neuroSPECT scans with and without acetazolamide injection, both before treatment and during treatment with valproate. Acetazolamide injection induced increased prefrontal perfusion not evident at baseline. Valproate treatment was associated with increased prefrontal perfusion concomitant with clinical improvement and abolished response to acetazolamide challenge.

Magnetic resonance imaging of regional cerebral blood oxygenation changes under acetazolamide in carotid occlusive disease.

BACKGROUND: Gradient-echo magnetic resonance imaging can demonstrate changes in cerebral blood oxygenation with high spatiotemporal resoluti … han flow velocity. Dynamic recording monitored signal intensities before and after injecting 1 g of acetazolamide. In sections covering the hand area of the primary sensorimotor cortex, acetazolamide-induced magnetic resonance signal increases were attenuated in the vascular territories of occluded arteries. Lateralization of responses in the left and right hem …

Cerebral vasodilatory capacity mapping using technetium-99m-DTPA-HSA SPECT and acetazolamide in moyamoya disease.

A cerebral vasodilatory capacity map, consisting of a 99mTc-DTPA-HSA SPECT image obtained after acetazolamide injection minus the baseline image, was produced in a patient with moyamoya disease. The map demonstrated diminished capacity in the posterior region of the right cerebral hemisphere. Subsequently, cerebral infarction occurred in the corresponding area. This observation suggests that regions of low vasodilatory capacity identified by cerebral vasodilatory capacity mapping may be at high risk of ischemic stroke.

Carbon dioxide elimination after acetazolamide in patients with chronic obstructive pulmonary disease and metabolic alkalosis.

Acetazolamide, an inhibitor of carbonic anhydrase, which catalyzes hydration/dehydration of carbon dioxide, has been used for correction of metabolic alkalosis in patients with chronic obstructive pulmonary disease (COPD). Animal experiments have shown that the gradient between tissue and the alveolar CO2 tension increases after inhibition of carbonic anhydrase, suggesting retention of CO2. In order to determine the true degree of carbon dioxide retention after total inhibition of carbonic anhydrase, 10 patients with COPD and pronounced metabolic alkalosis (base excess above 6) under controlle…

Transient myopia induced by acetazolamide in a pregnant patient with idiopathic intracranial hypertension: A case of successful rechallenge.

BACKGROUND: Acetazolamide is the mainstay of treatment for idiopathic intracranial hypertension (IIH). Transient myopia is a rare ocular adverse effect, usually attributed to ciliary body edema and forward displacement of the lens-iris diaphragm. CASE PRESENTATION: A 44-year-old woman developed IIH during pregnancy following hormonal therapy for in-vitro fertilization. She was started on acetazolamide 1 g/day and, after five doses, experienced sudden bilateral blurred vision that improved with myopic correction and resolved spontaneously within 24 hours after discontinuation of acetazolamide.…

Acetazolamide-Assisted Weaning From Mechanical Ventilation in a Child With Mixed Sleep Apnea: A Case Report.

An elevated central apnea-hypopnea index is typically considered indicative of abnormal breathing in children. Treatment of central sleep apnea (CSA) is complex, and multiple treatment options are tried with variable outcomes. Acetazolamide, a carbonic anhydrase inhibitor, is known to induce metabolic acidosis and can stimulate the respiratory drive. We report a 13-year-old boy who was having obesity and mixed apnea with an apnea-hypopnea index of 67.4/hour, who was hospitalized with hypercapnic respiratory failure. He was successfully weaned from prolonged mechanical ventilation following ace…

Nocturnal cerebral oxygenation in patients with COPD at altitude: data from a randomized clinical trial of acetazolamide.

STUDY OBJECTIVES: Patients with chronic obstructive pulmonary disease may experience cerebral tissue deoxygenation during altitude travel, especially during sleep. We quantified nocturnal cerebral (CTO) and arterial oxygenation (SpO2) in patients with chronic obstructive pulmonary disease during a stay at 3,100 m and evaluated the effects of preventive acetazolamide therapy. METHODS: Patients with moderate-to-severe chronic obstructive pulmonary disease, living at an altitude < 800 m, underwent nocturnal pulse oximetry and cerebral tissue oximetry by near-infrared spectroscopy at 760 m and du…

A mathematical model for temporal cerebral blood flow response to acetazolamide evaluated in patients with Moyamoya disease.

BACKGROUND: Paired cerebral blood flow (CBF) measurement is usually acquired before and after vasoactive stimulus to estimate cerebrovascular reserve (CVR). However, CVR may be confounded because of variations in time-to-maximum CBF response (tmax) following acetazolamide injection. With a mathematical model, CVR can be calculated insensitive to variations in tmax, and a model offers the possibility to calculate additional model-derived parameters. A model that describes the temporal CBF response following a vasodilating acetazolamide injection is proposed and evaluated. METHODS: A bi-exponen…

Effectiveness of add-on acetazolamide in children with drug-resistant CHD2-related epilepsy and in a zebrafish CHD2 model.

CHD2-related epilepsy is characterized by early-onset photosensitive myoclonic epilepsy with developmental delay and a high rate of pharmacoresistance. We sought to evaluate the efficacy of acetazolamide (ACZ) in CHD2-related epilepsy, due to ACZ's unexpected efficacy in our first patient harboring a pathogenic CHD2 variant. We collected patients from different Eastern European countries with drug-resistant CHD2-related epilepsy who were then treated with ACZ. Patients underwent video EEG before and during ACZ treatment. In a zebrafish model of CHD2-related epilepsy, ictal-like events were rec…

Drug reaction with eosinophilia and systemic symptoms syndrome secondary to acetazolamide associated with markedly elevated procalcitonin.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is an important cause of multi-organ dysfunction and can mimic other disorders including sepsis. We describe a patient presenting with septic shock and accompanying high procalcitonin. Although initially treated empirically with antibiotics, the emergence of eosinophilia during the admission lead to a revised diagnosis of DRESS syndrome, presumed secondary to acetazolamide. This case highlights the importance of regular clinical assessment and re-evaluation is key in identifying emerging features such as eosinophilia, rash …

Investigation of the protective effect of acetazolamide and SLC-0111 on carbon tetrachloride-induced toxicity in fruit fly.

Sulfonamide-based compounds in the development of drugs used in cancer treatment have been started to be investigated recently. In the current work, it was determined the protective effect of Acetazolamide (AAZ) and SLC-0111 on carbon tetrachloride-induced toxicity in the fruit fly (Drosophila melanogaster). AAZ and SLC-0111 were used as a nonselective and selective inhibitor of carbonic anhydrase isozymes, respectively, to compare the selectivity effect of drugs on toxicity. The experimental toxicity was created by carbon tetrachloride (CCl4) that causes tissue damage to the first stage larva…

Side effects of acetazolamide: a systematic review and meta-analysis assessing overall risk and dose dependence.

INTRODUCTION: Acetazolamide (AZM) is used for various conditions (eg, altitude sickness, sleep apnoea, glaucoma), but therapy is often limited by its side effect profile. Our objective was to estimate the risk of commonly reported side effects based on meta-analyses. We hypothesised that these risks are dose-dependent. METHODS: We queried MEDLINE/EMBASE (Medical Literature Analysis and Retrieval System Online/Excerpta Medica dataBASE) up until 04/10/2019, including any randomised placebo-controlled trial in which adults received oral AZM versus placebo reporting side effects. Eligibility asse…

AZATAX: Acetazolamide safety and efficacy in cerebellar syndrome in PMM2 congenital disorder of glycosylation (PMM2-CDG).

OBJECTIVE: Phosphomannomutase deficiency (PMM2 congenital disorder of glycosylation [PMM2-CDG]) causes cerebellar syndrome and strokelike episodes (SLEs). SLEs are also described in patients with gain-of-function mutations in the CaV2.1 channel, for which acetazolamide therapy is suggested. Impairment in N-glycosylation of CaV2.1 promotes gain-of-function effects and may participate in cerebellar syndrome in PMM2-CDG. AZATAX was designed to establish whether acetazolamide is safe and improves cerebellar syndrome in PMM2-CDG. METHODS: A clinical trial included PMM2-CDG patients, with a 6-month…

Acetazolamide therapy in a case of fingolimod-associated macular edema: early benefits and long-term limitations.

UNLABELLED: Fingolimod is a potent drug in relapsing forms of multiple sclerosis. Visual impairment due to fingolimod-associated macular edema (FAME) usually leads to discontinuation of fingolimod therapy. METHODS: We report on a 24-year old woman with bilateral FAME. RESULTS: We continued fingolimod and added oral acetazolamide, which led to recovery of visual acuity and regression of macular edema. However, fingolimod had to be discontinued when fluorescein angiography revealed an enlarged foveal avascular zone. DISCUSSION AND CONCLUSION: Oral acetazolamide might be a treatment option for…
