A quality assessment of online patient information regarding tonsillitis using the EQIP tool.

BACKGROUND: Tonsillitis is a common paediatric condition. The decision to seek medical attention and consent to treatment is usually made by parents or guardians. With increased accessibility of the internet, online medical information plays an increasingly significant role in the decision-making process. Little is known regarding the quality of online patient information on tonsillitis, as this has not been previously studied. OBJECTIVE: The aim of our study was to identify the quality of information regarding tonsillitis on websites intended for the public. METHODS: We conducted a systemat

Simultaneous enucleation of the tonsils in "Pickwickian patients".

Tonsillar surgery was, from the 19th century, the reference treatment for apneic patients. Adapting to the technical limitations of the time, surgeons devised ingenious procedures. The purpose of this historical note is to travel back to that time and rediscover one of the techniques favored by Chassaignac: "simultaneous enucleation" of the tonsils.

Patient reported pain-related outcome measures after tonsil surgery: an analysis of 32,225 children from the National Tonsil Surgery Register in Sweden 2009-2016.

The objective of this study was to describe factors affecting pain after pediatric tonsil surgery, using patient reported pain-related outcome measures (pain-PROMs) from the National Tonsil Surgery Register in Sweden. In total, 32,225 tonsil surgeries on children (1 to <18 years) during 2009-2016 were included; 13,904 tonsillectomies with or without adenoidectomy (TE ± A), and 18,321 tonsillotomies with or without adenoidectomy (TT ± A). Adjustments were made for variables included in the register to compensate for contributable factors in the analysis. When compared to TE ± A for surgical ind…

Changes in body composition and growth pattern after adenotonsillectomy in prepubertal children.

OBJECTIVE: Adenotonsillar hypertrophy and chronic tonsillitis are associated with growth interruption during childhood, while adenotonsillectomy has been associated with growth improvement and increased body mass index (BMI). However, no reported study has investigated the effect of adenotonsillectomy on the proportion of body muscle and fat mass. The aim of this prospective study was to evaluate the effect of adenoidectomy and adenotonsillectomy on body muscle and fat composition in prepubertal children. METHODS: Thirty prepubertal children (22 boys, 8 girls; 3-9 years of age) were followed …

Acoustic and perceptual aspects of vocal function in children with adenotonsillar hypertrophy--effects of surgery.

OBJECTIVE: To evaluate outcome of two types of tonsil surgery (tonsillectomy [TE]+adenoidectomy or tonsillotomy [TT]+adenoidectomy) on vocal function perceptually and acoustically. STUDY DESIGN: Sixty-seven children, aged 50-65 months, on waiting list for tonsil surgery were randomized to TE (n=33) or TT (n=34). Fifty-seven age- and gender-matched healthy preschool children were controls. Twenty-eight of them, aged 48-59 months, served as control group before surgery, and 29, aged 60-71 months, served as control group after surgery. METHODS: Before surgery and 6 months postoperatively, the c…

Death following tonsillectomy in a child with Williams syndrome.

Williams syndrome (WS) is an uncommon genetic syndrome due to a deletion of several genes on chromosome 7. The syndrome is associated with dysmorphic facies, neurological manifestations, idiopathic hypercalcemia, and cardiac abnormalities, particularly supravalvular aortic stenosis (SVAS). Children with Williams syndrome may have chronic serous otitis media and/or obstructive sleep apnea. Hyperacusis is also commonly seen in these children. We report a case of sudden death at the time of tonsillectomy/adenoidectomy and bilateral tympanostomy tube placement in a child with Williams syndrome. Al…

Concentrations of cefpodoxime in plasma, adenoid, and tonsillar tissue after repeated administrations of cefpodoxime proxetil in children.

Cefpodoxime proxetil was administered to 36 children undergoing tonsillectomy, adenoidectomy or both. It was very well tolerated. The detectable tissue concentrations of cefpodoxime were moderate but remained constant (approximately 0.05 mg/kg) 3, 6, and 12 h after the last dose of the drug, while the respective plasma concentrations were declining. This suggests the possibility of twice-daily administration. However, 30% of children did not have quantifiable concentrations in the tonsil and more than half the adenoids did not have quantifiable levels. Whether a higher dosage would lead to hig…

Markedly delayed postoperative malignant hyperthermia.

Malignant hyperthermia (MH) may be triggered by exposure to commonly employed anesthetic agents and muscle relaxants, and often manifests itself during the period of anesthesia. Delayed-onset MH occurring one to four hours postoperatively has been described in isolated case reports. A case of delayed-onset MH occurred 11 hours following routine tonsillectomy and adenoidectomy. The patient demonstrated tachypnea, tachycardia, hyperthermia, and metabolic acidosis. Prompt intravenous administration of dantrolene sodium was therapeutic. Serial serum creatine phosphokinase evaluation verified the d…

Airway obstruction in an immunosuppressed child.

Recipients of organ transplants who require continuous immunosuppression appear to have an increased incidence of lymphoproliferative disorders. A case of severe upper airway obstruction resulted from adenotonsillar hypertrophy in a 2-year-old girl following liver transplantation. Tonsillectomy and adenoidectomy dramatically reversed the airway problem and confirmed a transplant-associated lymphoproliferative disorder. A generalized lymphadenopathy also regressed with reduction of cyclosporine immunosuppression. We urge vigilance in monitoring transplant recipients, particularly children, for …

[On open nasal speech following operative procedures in the oro- and nasopharynx. A combined speech and cephalometric analysis (author's transl)].

A combined speech and cephalometric analysis was performed on 60 patients with class III malocclusion to evaluate the interaction between the cranio-facial skeleton and the disorder of an open nasal speech. There was an increase of durable open nasal speech after tonsillectomy but not after adenoidectomy. This predisposition to open nasal speech in class III patients appeared to be increased by the formation of the surrounding bony structures. In dependency to the cranio-facial morphology in the vertical dimension, the configuration of the bony nasopharynx and the sagittal jaw relationship wer…
