Transdermal estradiol improves bone density when used as single agent therapy for prostate cancer.

PURPOSE: Current androgen deprivation therapies for men with prostate cancer cause accelerated osteoporosis and a significant risk of osteoporotic fracture. We have recently shown that transdermal estradiol is an effective alternative for such patients. Here we report the impact of transdermal estradiol therapy on the bone mineral density of men with prostate cancer. MATERIALS AND METHODS: A total of 20 patients with newly diagnosed locally advanced or metastatic prostate cancer were treated with transdermal estradiol patches. Bone mineral density of the lumbar spine and the proximal femur wa

Topical medications: a focus on antifungals and topical steroids.

Because skin disease is accessible, it can be treated with locally applied medication, which offers great advantages--exposure to a drug is limited to the affected skin and systemic effects of potentially toxic drugs are minimized. Ointments, creams, antifungals, and antibiotics all have their place in treating various skin diseases. Topical steroids, the largest group of topical medications, are effective but present the potential for side effects. This article discusses current and new topical medications that can be used to treat a range of skin diseases.

Transdermal estradiol. A review of its pharmacological profile, and therapeutic potential in the prevention of postmenopausal osteoporosis.

The estradiol transdermal therapeutic system is a percutaneous delivery device which, when affixed to the skin, delivers estradiol (the major estrogen in premenopausal women) into the systemic circulation at a constant rate for up to 4 days. Because this method of delivery avoids first-pass hepatic metabolism, premenopausal levels of estradiol can be maintained in postmenopausal women using very low dosages. Bone density decreases at an accelerated rate after the menopause, which can lead to development of osteoporosis and increased fracture risk. In common with other estrogen therapies, trans

Nonmosaic verruca plantaris: treatment with a salicylic acid polymeric matrix delivery system.

Ten patients who presented with nonmosaic plantar warts were treated with a new polymeric matrix patch delivery system containing salicylic acid USP, 21 percent. Clinicians then assessed the potential value of this patch system in terms of efficacy, length of treatment time required, patient compliance, and tolerance.

Marginal vitiligo: an unusual depigmenting disorder.

BACKGROUND: Marginal vitiligo, or inflammatory vitiligo with raised borders is a unique subset of vitiligo vulgaris presenting with scattered depigmented, pruritic patches surrounded by a raised, erythematous border. Alternative diagnoses include discoid lupus erythematosus and cutaneous T-cell lymphoma. PURPOSE: To properly guide treatment, it is important to exclude other conditions that present with a similar pattern of depigmentation. This requires the ability to identify specific cutaneous and histologic clues that support this diagnosis. MATERIAL AND METHODS: A 22 year-old man presente

[New routes of administration: epidermal, transcutaneous mucosal ways of vaccination].

A successful vaccine triggers the interaction of various cells of the immune system as does a regular immune response. It is thus necessary to introduce the vaccine antigens into an anatomic site where they will contact immune cells. The route of administration is thus critical for the outcome of vaccination. Intramuscular or subcutaneous injections are the most popular. Antigens injected intramuscularly can form persistent precipitates that are dissolved and re-absorbed relatively slowly. If injecting antigens is a quick, easy and reproducible way to vaccination, it requires trained personnel

Clonazepam-induced lichenoid drug eruption: a case report.

BACKGROUND: Lichenoid drug eruption is rare and can mimic idiopathic lichen planus and other dermatoses. Clonazepam, a commonly used drug for the treatment of anxiety-related disorders and seizures, is known to be an unlikely cause of cutaneous adverse effects. Only one case report of LDE due to clonazepam has been reported. CASE PRESENTATION: A 81-year-old male patient with Alzheimer's disease developed a lichenoid eruption after taking clonazepam. He developed a violaceous scaly patch on his lower extremities, from both buttocks to the feet. The cutaneous eruption resolved 2 months after ce

Unveiling and Benefits of Topically Applied l-(+)-Ergothioneine in Periwound Region.

Oxidative stress significantly impairs wound healing by prolonging inflammation and damaging surrounding tissue. l-(+)-Ergothioneine (EGT), a naturally occurring thione-thiol antioxidant transported by OCTN1, exhibits potent cytoprotective and anti-inflammatory properties. The aim of the study was to investigate whether EGT applied topically to the periwound region can accelerate healing via transdermal antioxidant action. Full-thickness excisional wounds were created on the dorsum of Wistar rats. Animals were divided into three groups: the untreated rats (control), the rats treated with EGT a

Tranexamic Acid for Hyperpigmentation Disorders: A Literature Review on Efficacy and Safety in Melasma and PIH.

BACKGROUND: Hyperpigmentation disorders, including melasma and post-inflammatory hyperpigmentation (PIH), are common dermatologic conditions associated with significant cosmetic and psychological burden. Tranexamic acid (TXA), an antifibrinolytic agent, has gained increasing attention due to its anti-inflammatory and antimelanogenic properties. OBJECTIVE: To review the current evidence on the efficacy and safety of tranexamic acid in the management of hyperpigmentation disorders, particularly melasma and PIH. METHODS: A narrative literature review was conducted using PubMed, Scopus, and Goog

Advanced drug delivery systems for topical treatment of common skin diseases: a decade in review.

Common skin diseases including fungal skin infections, atopic dermatitis, acne vulgaris, and psoriasis represent a substantial health burden and remain therapeutically challenging. Creams, ointments, gels and other conventional topical formulations are constrained by drug instability, uncontrolled drug release, inadequate skin penetration, and the need for frequent application, which compromise their therapeutic effectiveness and reduce patient adherence. Advanced topical drug delivery systems have emerged as promising strategies to overcome these limitations. This paper provides a disease-dri

Topical Foam for Simultaneous Treatment and Decontamination of Chemical Warfare Agents on Dermal Exposure.

Being highly toxic and a quick-acting vesicant, even small amounts of lewisite if not decontaminated immediately are rapidly absorbed into systemic circulation via skin exposure, leading to acute poisoning and death. The skin is the first major target to such chemical weapons. Although the stratum corneum provides a barrier lewisite being a lipophilic molecule that readily permeates this barrier. This necessitates, making early and thorough decontamination prior to manifestation of adverse effects. For this, we aimed to decontaminate skin using an antidote-loaded topical foam, followed by trea
