The authors present a case of neonatal amebiasis in a 5-month-old girl who exhibited symptoms including vomiting, refusal to feed, abdominal distension, and mucoid, bloody stools. Sigmoidoscopy revealed multiple aphthous ulcers, and a colonic biopsy was submitted with clinical suspicion of eosinophilic colitis. The histologic examination of the biopsy, however, confirmed the diagnosis of amebic colitis through identification of trophozoites of Entamoeba histolytica. The infant, who was likely infected by her caregiver or mother, was successfully treated with intravenous metronidazole. This rep…

The clinical management of amebiasis is a growing concern, particularly among human immunodeficiency virus (HIV)-infected individuals who are predisposed to severe illness. Treatment with a luminal amebicide is strongly recommended following acute-stage treatment with a nitroimidazole. In 2004, the Japanese Research Group on Chemotherapy of Tropical Diseases introduced paromomycin, which was not nationally licensed, and offered it to a number of patients. From 2004 to 2011, 143 case records of amebiasis (123 with amebic colitis, 16 with amebic liver abscess, and 4 with both) in which patients …

Entamoeba histolytica, the protozoan parasite that causes amebic dysentery, greatly contributes to disease burden in the developing world. Efforts to exhaustively characterize the pathogenesis of amebiasis have increased our understanding of the dynamic host-parasite interaction and the process by which E. histolytica trophozoites transition from gut commensals to invaders of the intestinal epithelium. Mouse models of disease continue to be instrumental in this area. At the same time, large-scale studies in human populations have identified genetic and environmental factors that influence susc…

Experience of treatment of 8 HIV-patients with amebiasis is analyzed. Five patients with provisional diagnosis "acute appendicitis" underwent diagnostic laparoscopy, sanation and drainage of abdominal cavity. Ameboma with acute intestinal obstruction was diagnosed at 2 patients; they underwent extraperitonization of inflammatory tumor. One patient with provisional diagnosis "peritonitis" had large purulent-necrotic total process in colon; subtotal resection of colon has been performed. Extraintestinal complications of amebiasis have been also diagnosed at 6 patients in short-term postoperative…

Genital amebiasis by Entamoeba histolytica is rare, even in Mexico where the disease is endemic. We report a case of genital amebiasis in a female patient with a recto-perineal fistula and two-week history of a profuse vaginal discharge, a painful and friable vulvar ulcer and induration in gluteal and inner side of thighs. The PAP smear and the biopsy showed trophozoites, no malignant cells were observed. The findings were compatible with genital amebiasis. The serology test for E. histolytica was positive (> 1: 512). The patient was treated with metronidazol 750 mg tid for 3 weeks. Complete r…

Entamoeba histolytica infection and colitis occurred in 55% and 4%, respectively, of a cohort of Bangladeshi preschool children observed for … tyl-d-galactosamine lectin. This was associated with an 86% reduction in new infection over 1 year. Amebiasis is a common and potentially serious infection in children from Dhaka, and both innate and acquired immune responses limit infection.

Entamoeba histolytica was recently reclassified to recognize the existence of two genetically distinct but morphologically indistinguishable species: E. histolytica, the protozoan parasite that causes amebic dysentery and liver abscess, and Entamoeba dispar, a nonpathogenic intestinal parasite. Acceptance of this redefinition has dramatically changed both our understanding of the true epidemiology of E. histolytica and the optimal methods for diagnosing amebiasis. Molecular-based diagnostic tests using polymerase chain reaction (PCR) to amplify amebic DNA or enzyme-linked immunosorbent assay (…

Amoebic liver abscess is a common disease in developing countries. Although therapy is now perfectly standardized, errors and inaccuracies are frequent in daily practice. A study was performed at the Calmette Hospital in Phnom Penh (Cambodia) to evaluate and improve this situation. Diagnostic criteria and the outcome of treatment with oral or parenteral metronidazole were analyzed in 38 patients presenting amoebic liver abscess. Hepatalgia was observed in 100% of cases. Hyperleukocytosis and enhancement of the sedimentation rate occurred in 95% of cases. Diagnosis was confirmed by ultrasonogra

The antigens of E. histolytica recognized by IgA antibodies in sera from patients with either amebic liver abscess or intestinal amebiasis, as well as saliva from the latter, were determined in immunoblots of whole native trophozoite proteins. Results show that sera of patients with amebic liver abscess reacted mainly with amebic proteins of more than 200 kDa, whereas those of individuals with intestinal amebiasis recognized a protein of 145 kDa. The saliva of the latter detected a protein of 210 kDa which is not always seen when using the sera of patients with intestinal or hepatic amebiasis.

Clinical symptoms of the initial stage of the disease (before the onset of pathognomonic signs of amoebiasis--the formation of ulcers in the colon) have been analysed in 287 patients with intestinal amoebiasis (IA). The following variants of the clinical pattern have been identified: slow-progressing IA (59.2% of the patients examined) and fast-progressing IA (40.8% of the patients examined). In 5 patients with body eight deficit and clinical manifestations of vitamin C and B deficiency the disease assumed a fulminant course. The patients' condition was considered severe or very severe already

Rat livers inoculated intraportally with trophozoites of E. histolytica were studied by light and electron microscopy. At one hour post-inoculation, trophozoites were localized at the hepatic sinusoids without inflammatory reaction and only slight vascular congestion. From two hours post-inoculation, trophozoites were associated with small inflammatory foci distributed at random. At three to four hours, trophozoites showed signs of cellular damage. After eight hours, no parasites were seen and only some lymphoplasmocytic infiltration were detected. It is suggested that in the experimental hepa

Entamoeba histolytica is the most common pathogenic intestinal parasite in México. Prevalence rates of AM vary widely from place to place, being higher in areas with poor sanitation, households and institutions, and among male homosexuals. Early recognition and treatment may be necessary to prevent its spread in the community. Intervention by preventive health education is indicated. The timing of such educational endeavours may be adjusted to any rhythmic and to that extent predictable changes in the incidence of AM and of any other possibly related gastrointestinal parasitic disease, such as

Symptomatic intestinal amebiasis was highly endemic among the Cambodians living at Green Hill, an evacuation site on the Thai-Cambodian border between June 1987 through May 1989. Monthly incidence rates of intestinal amebiasis were determined to be inversely proportional to cumulative monthly rainfall. The highest incidence of amebic dysentery was 63/1000 in children 12-23 months old. Behavioral risk factors were investigated by conducting a case-control study. A questionnaire was administered to 73 families, each having at least one member with confirmed intestinal amebiasis within the past 3…

Experimental intrahepatic inoculation of the gerbil with Entamoeba histolytica trophozoites was used as a model of liver amebiasis to study the cellular immune response elicited by the parasite. It was shown that abscess-derived macrophages (5 to 20 days old) were deficient in their capacity to develop a respiratory burst, to secrete and express membrane-bound interleukin-1-like activity, and to kill E. histolytica trophozoites as well as to respond to lymphokines in vitro. However, macrophages isolated from the spleen and peritoneal cavities from the same infected animals were not significant…

A prospective evaluation was made of the therapeutic efficacy of paromomycin, an orally administered, nonabsorbable aminoglycoside, in 114 homosexual men with mild-to-moderate (nondysenteric) intestinal amebiasis. All patients received 25-35 mg/kg daily in three divided doses for seven days. Of the 80 patients with gastrointestinal complaints at the onset of therapy, 55 (80%) of 69 were asymptomatic within four to six weeks after completion of treatment; 11 patients were lost to follow-up. Paromomycin produced long-term eradication of intestinal Entameba histolytica infection in 92% of all men…

The congenitally athymic nude (nu/nu) mouse was studied as a model for amebic liver disease, using Entamoeba histolytica. Despite intrahepatic inoculation of massive numbers of amebae, we could not produce sustained infections in nu/nu mice. We also failed to induce hepatic amebiasis in thymus-intact nu/+ and +/+ littermates, or in nu/+ mice pretreated with rabbit antimouse thymocyte globulin. Humoral response was measured in both the nu/nu and nu/+ mice. Nu/+ but not nu/nu animals developed a specific IgG response after challenge with E. histolytica. There were no significant IgM responses. P…
