The clinical effectiveness of elexacaftor/tezacaftor/ivacaftor (ETI) for people with CF without a F508del variant: A systematic review and meta-analysis.

BACKGROUND: Access to elexacaftor/tezacaftor/ivacaftor (ETI) for people with cystic fibrosis (PwCF) without a F508del variant is limited due to lack of clinical data supporting efficacy. METHODS: In this systematic review and meta-analysis, we examined patient-level data from studies reporting the clinical response to ETI for PwCF with non-F508del CFTR variants. We searched electronic data sources including Embase, MEDLINE, and CENTRAL from January 1st, 2019 to May 14th, 2024. FINDINGS: Our search results identified 4,795 studies and 20 met the eligibility criteria. 120 of 164 (73 %) individ

One year of ETI reduces lung bacterial colonisation in adults with cystic fibrosis.

The triple combination elexacaftor-tezacaftor-ivacaftor (ETI) has provided unprecedented clinical benefits for people with cystic fibrosis (pwCF) and drastically transformed the outcome of this disease. We aimed to describe the evolution of lung bacterial colonization in 198 French adult pwCF taking into account the use of concomitantly respiratory treatment. We collected sputum cultures produced during the entire follow-up period starting 3 years before and ending 1 year after ETI initiation. All sputum cultures were centralized and analyzed at our bacteriological laboratory. Clinical data in

Comparison of a novel potentiator of CFTR channel activity to ivacaftor in ameliorating mucostasis caused by cigarette smoke in primary human bronchial airway epithelial cells.

BACKGROUND: Cystic Fibrosis causing mutations in the gene CFTR, reduce the activity of the CFTR channel protein, and leads to mucus aggregation, airway obstruction and poor lung function. A role for CFTR in the pathogenesis of other muco-obstructive airway diseases such as Chronic Obstructive Pulmonary Disease (COPD) has been well established. The CFTR modulatory compound, Ivacaftor (VX-770), potentiates channel activity of CFTR and certain CF-causing mutations and has been shown to ameliorate mucus obstruction and improve lung function in people harbouring these CF-causing mutations. A pilot

Extent of foetal exposure to maternal elexacaftor/tezacaftor/ivacaftor during pregnancy.

BACKGROUND AND PURPOSE: Cystic fibrosis (CF) patients are living longer and healthier due to improved treatments, e.g. cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapy elexacaftor/tezacaftor/ivacaftor (ETI), with treatment possibly occurring in pregnancy. The risk of ETI to foetuses remain unknown. Thus the effect of maternally administered ETI on foetal genetic and structural development was investigated. EXPERIMENTAL APPROACH: Pregnant Sprague Dawley rats were orally treated with ETI (6.7 mg·kg-1·day-1 elexacaftor + 3.5 mg·kg-1·day-1 tezacaftor + 25 mg·kg-1·day-1

Use of cystic fibrosis inhaled medication before and after elexacaftor/tezacaftor/ivacaftor initiation.

Elexacator/tezacaftor/ivacaftor (ETI) has improved cystic fibrosis (CF) outcomes. A reduction in use of maintenance medication after its initiation has been reported. Seventy-one adult people with CF (PwCF) who are followed in three CF centers and completed one year of treatment with ETI were included in this study. Their use of inhaled dornase-α, colistin, tobramycin, aztreonam and levofloxacin during this period was compared with the corresponding use during one year without ETI, using the Medication Possession Ratio (MPR). MPR was significantly decreased after ETI initiation for dornase-α (

Body mass index and nutritional intake following Elexacaftor/Tezacaftor/Ivacaftor modulator therapy in adults with cystic fibrosis.

BACKGROUND: Elexacaftor/Tezacaftor/Ivacaftor (ETI) modulator therapy is often associated with increased body mass index (BMI) in people with cystic fibrosis (CF). This is thought to reflect improved clinical stability and increased appetite and nutritional intake. We explored the change in BMI and nutritional intake following ETI modulator therapy in adults with CF. METHODS: Dietary intake, measured with myfood24®, and BMI were collected from adults with CF at baseline and follow-up as part of an observational study. Changes in BMI and nutritional intake in participants who commenced ETI ther

Safety and effectiveness of treatment with elexacaftor, tezacaftor and ivacaftor in adults with cystic fibrosis.

INTRODUCTION: Cystic fibrosis (CF) is a disease caused by mutations in the gene located on chromosome 7 that encodes the CF transmembrane conductance regulator protein. Several trials have demonstrated the efficacy and safety of the ELE/TEZ/IVA combination in patients who have at least one F508del mutation. The main objective of the study was to evaluate the safety at 3 and 6 months of treatment with ELE/TEZ/IVA in adult patients with CF. METHODS: This is a real-life, prospective, single-center, cross-sectional study that included adult patients from the CF multidisciplinary unit. The demogra

Drug-induced acne with elexacaftor/tezacaftor/ivacaftor in people with cystic fibrosis.

Elexacaftor/tezacaftor/ivacaftor (ELX-TEZ-IVA) is a Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) modulator shown to improve lung function and reduce sweat chloride in people with Cystic Fibrosis (CF). The only commonly reported dermatologic adverse effect with CFTR modulators including ELX-TEZ-IVA is rash. In this case series, we describe 19 patients who reported new onset or worsening of acne after initiation of this drug to their CF pharmacist or another member of their CF care team. The mechanism and frequency of this adverse effect is unknown.

Accumulation and persistence of ivacaftor in airway epithelia with prolonged treatment.

BACKGROUND: Current dosing strategies of CFTR modulators are based on serum pharmacokinetics, but drug concentrations in target tissues such as airway epithelia are not known. Previous data suggest that CFTR modulators may accumulate in airway epithelia, and serum pharmacokinetics may not accurately predict effects of chronic treatment. METHODS: CF (F508del homozygous) primary human bronchial epithelial (HBE) cells grown at air-liquid interface were treated for 14 days with ivacaftor plus lumacaftor or ivacaftor plus tezacaftor, followed by a 14-day washout period. Method for assessment of experimental allergy in guinea pigs adapted to cosmetic ingredients.

2 protocols are presented to predict and assess allergic reactions after application of chemicals or cosmetic products, either raw materials or finished products. The first is an open epidermal induction and challenge, the intensity of the reaction being maximized by injecting Freund's complete adjuvant into the foot pad. The second allows detection of weaker allergens and is a quasi-intradermal induction: both the adjuvant and test substance are injected into the foot pad. In both protocols, the challenge consisted of a single topical application in the lumbar region of 10 microliter of test

Cerebral blood flow, circulation, and blood homeostasis of dogs during slow cyanide poisoning and after treatment with 4-dimethylaminophenol.

The effects of 4-dimethylaminophenol . HCl (DMAP) and 100% oxygen on cerebral blood flow (CBF) and peripheral circulation, arterial and venous blood gases, and other parameters have been investigated in dogs in the course of slow cyanide infusion. The i.v. infusion of KCN increased the respiratory minute volume, accompanied by a rise in arterial pO2 and pH and a decrease in arterial pCO2 while the venous lactate concentration increased by about 500% and the hemoglobin content and hematocrit by about 30%. Heart rate and carotid artery blood flow decreased. Local CBF in the cingulum as measured

Effect of oxidant agents on normal and G6PD-deficient erythrocytes.

The effects of several oxidant agents on metabolic and membrane parameters of glucose-6-phosphate dehydrogenase deficient erythrocytes (Mediterranean type) were examined in order to explore a possible common mechanism for their action on the deficient cells. The increase in methemoglobin and the decrease in GSH levels elicited by e oxidants were more pronounced in the deficient cells, provided glucose was present during the incubation of the cells with the drugs. However, glucose did not differentially modify other parameters: ATP level, filterability and osmotic fragility. There was no correl

Modulation of host inflammatory pathways by Pseudomonas aeruginosa extracellular vesicles in cystic fibrosis: impact of pulmonary exacerbation and elexacaftor-tezacaftor-ivacaftor treatment.

BACKGROUND: Pseudomonas aeruginosa is a prevalent pathogen in persons with cystic fibrosis (pwCF), frequently causing chronic infections. These infections are often associated with episodic pulmonary exacerbations (PEx), yet the mechanisms linking changes in P. aeruginosa to these PEx remain poorly understood. It is unknown whether virulence changes in P. aeruginosa extracellular vesicles (EVs) contribute to PEx. We hypothesized that P. aeruginosa EVs from PEx will elicit increased inflammation response compared to Pa EVs from clinical stability in an in vitro cell culture model. METHODS: P.

Therapeutic hopes for CFTR-related recurrent pancreatitis: a case-series.

The presence of CFTR variants increases the risk of pancreatitis in pancreatic sufficient people with cystic fibrosis (pwCF) or with CFTR-related disorder (pwCFTR-RD). Modulators have been showed to decrease pancreatitis episodes in pwCF presenting recurrent pancreatitis. Whether CFTR modulators are efficient to prevent recurrent episodes of pancreatitis in pwCFTR-RD is not known. We collected clinical, biological, imaging and CFTR functional data in 4 patients with recurrent pancreatitis and CFTR-RD, treated with CFTR modulators and assessed the efficacy of the treatment. With a follow-up of

Changes to serum lipids, BMI and body composition in adults with cystic fibrosis on Elexacaftor/Tezacaftor/Ivacaftor (ETI): A scoping review.

Elexacaftor-Tezacaftor-Ivacaftor (ETI) has substantially improved lung function and life expectancy for people with cystic fibrosis (pwCF). Emerging evidence suggests that overweight, obesity and dyslipidaemia are now developing in pwCF prescribed ETI. This scoping review, conducted adhering to PRISMA guidelines, aimed to provide an overview of the research currently available that investigated body mass index (BMI), body composition and serum lipid profiles in adults with CF treated with ETI. Peer-reviewed articles and conference abstracts were identified through database searches and confere

Systemic effects of cystic fibrosis transmembrane conductance regulator modulators on the plasma and serum proteome.

Cystic fibrosis (CF) is caused by dysfunction of the cystic fibrosis transmembrane conductance regulator (CFTR) anion channel in epithelial organs leading to a complex multi-organ disease. CFTR modulator drugs improve mutant CFTR function and markedly improve clinical outcomes in people with CF (pwCF), but their broader molecular and systemic effects remain to be fully elucidated. We employed mass-spectrometry-based proteomics to compare the plasma and serum proteomes of pwCF treated with the dual combination CFTR modulator lumacaftor/ivacaftor (LUM/IVA) or the triple combination of elexacafto
