Serum amyloid A (SAA) is an acute phase first class protein discovered a quarter of the century ago. Its concentration depends on clinical findings of the patient, illness activity and the therapy applied. SAA increases moderately to markedly (100-1000 mg/l) in bacterial and fungal infections, invasive malignant diseases, tissue injuries in the acute myocardial infarction and autoimmune diseases such as rheumatoid arthritis and vasculitis. Mild elevation (10-100 mg/l) is often seen in viral infections, systemic lupus erythematosus and localized inflammation or tissue injuries in cystitis and c…

For the better part of the past two decades, studies on the molecular, biochemical and cellular mechanisms of Alzheimer disease have focused on amyloid-beta protein, the major proteinaceous component of senile plaques. In fact, the Amyloid Cascade Hypothesis has come to dominate the field both in terms of proposed disease mechanism as well as potential for therapeutic intervention. In this review, we look at the Amyloid Cascade Hypothesis from the perspective of pathology, cell biology, and genetics. In all cases, alternate interpretations of old data as well as new evidence indicates that amy…

Insoluble amyloid formation by islet amyloid polypeptide (IAPP) in the islets of Langerhans of the pancreas is a major pathophysiological feature of noninsulin dependent diabetes mellitus (NIDDM) or type II diabetes. Because in vivo formed amyloid colocalizes with areas of cell degeneration and IAPP amyloid aggregates are cytotoxic per se, the process of IAPP amyloid formation has been strongly associated with the progressive pancreatic cell degeneration and thus much of the pathology of type II diabetes. IAPP is a pancreatic polypeptide of 37 residues that, in its soluble form, is believed to…

Alzheimer's disease is the major cause of dementia. The neuropathological basis for the diagnosis is the presence of senile plaques and neurofibrillary degeneration in brain tissue. Senile plaques consist of a central core of fibrillar amyloid beta-protein and some other proteins, surrounded by swollen neurites. Three genes causing early-onset autosomal dominant Alzheimer have so far been described. Recently, polymorphisms in three other genes have been shown to influence the risk for late-onset Alzheimer's disease. All these genes seem to influence the metabolization of the beta-protein or th…

beta A4, an hydrophobic peptide containing from 39 to 43 amino acids, is the major constituent of the amyloid core of characteristic lesions of Alzheimer's disease (AD) known as senile plaques. By cDNA cloning, it was demonstrated that beta A4 is derived from a much larger precursor named the amyloid peptide precursor or APP. The isolation of cDNA clones and the characterization of their nucleotide sequence has shown that several APP proteins containing from 365 to 770 amino acids are produced by alternative splicing of a single primary transcript. The major APP isoforms contain a large extrac…

The frequency, nature and tissue distribution of localized amyloid deposits in articular cartilage of young and elderly patients, with and without evidence of arthritic disease, was determined. Localized amyloid deposits in articular cartilage were not found in young patients with osteoarthritis of the hip or chondromalacia/osteoarthritis of the patella. However, in elderly patients with osteoarthritis of the hip, amyloid deposits were commonly found, although at no greater frequency than in elderly patients with no evidence of arthritis. Amyloid deposits were commonly present (in 95% of cases…

Islet amyloid polypeptide (IAPP) is the constituent peptide of amyloid in pancreatic islets of Type 2 diabetic patients and in insulinomas. Amyloid formation in Type 2 diabetes is associated with islet cell destruction which may promote formation of autoantibodies to IAPP. An ELISA method has been developed to detect IAPP autoantibodies and used to assay serum from 80 non-diabetic subjects, 49 Type 1 and 228 Type 2 diabetic patients, and 10 patients with insulinomas. Microtitre plates coated with IAPP 1-37 were used to detect antibody binding followed by an alkaline phosphatase conjugated anti…

A patient is reported who has had episodes of spontaneous haemarthroses of the shoulder. On aspiration of the acute haemarthrosis, amyloid was found after appropriate staining of the centrifuged sediment. This may be a predisposing factor for haemarthrosis in haemodialysed patients.

In this immunocytochemical study, the brains of nine squirrel monkeys (Saimiri sciureus), ranging from 8 to 27 years of age, were examined for the presence and distribution of beta/A4 amyloid, a 4-kilodalton peptide. In aged squirrel monkeys, amyloid is associated primarily with intracerebral and meningeal capillaries and arterioles and occurs to a lesser degree as small and/or diffuse deposits in the neural parenchyma and in the dense cores of senile plaques. Cerebrovascular amyloid is found primarily in neocortex, amygdala, and septum verum and is rare or nonexistent in other subcortical gra…

An 81-year-old man presented with a globoid caruncular mass of recent onset involving the left medial canthus. Pathological examination of the mass after its complete excision showed a roughly spherical subepithelial tissue deposit of amyloid, surrounded and sparsely infiltrated by plasma cells. The amyloid showed permanganate-resistant congophilia consistent with immunamyloid, and since most of the plasma cells stained positive for lambda light chains on immunohistochemical cytoplasmic staining, with no demonstrable heavy chains, the lesion was regarded as an extramedullary amyloid-producing …

Amyloidoses are a very heterogeneous set of diseases, characterized by extracellular deposition of fibrillar proteins in different tissues. It is still a matter of debate whether the different forms of amyloidosis can share some common etiological mechanisms, or they are completely unrelated. The biochemical characterization of the protein component of the deposits provides a powerful system of classification for the different amyloidotic disorders and shades light on the molecular mechanisms of selective precipitation from soluble precursors and of tissue-specific deposition. Furthermore, ide…

Out of approximately 16,000 horses referred for clinical examination, nine had amyloidosis. Six of these horses had localised amyloid deposits in the wall of the nasal meatus and ventral turbinates associated with epistaxis. Horse 1 also developed malignant histiolymphocytic lymphosarcomas. The amyloid deposits were potassium permanganate-resistant and tryptophan-positive. Gel filtration of solubilised amyloid fibrils from Horse 1 revealed a major retarded fraction with an apparent molecular weight of 20 kD. This protein had an amino acid composition similar to human AL-amyloid proteins and ho…
