Rotational Vertebral Artery Dissection Secondary to Anomalous Entrance into Transverse Foramen.

BACKGROUND: Rotational vertebral artery occlusion is a rare syndrome characterized by vertebrobasilar insufficiency secondary to position-dependent occlusion of the vertebral artery. Most cases reported in the literature have been attributed to osteophytic compression, either from the occipital condyle or within the transverse foramen. However, vertebral artery dissection secondary to motion in the setting of anomalous anatomy has not been reported. CASE DESCRIPTION: To the authors' knowledge, rotational vertebral artery occlusion and dissection secondary to anomalous entrance into the transv

Three-dimensional rotational digital angiography in a complicated case of patent ductus arteriosus transcatheter closure.

There has been no report so far as to the usefulness of 3D rotational angiography in congenital heart disease interventions. We present a case of a difficult patent ductus arteriosus in which 3D rotational angiography was the key to successful closure. A 64-year-old woman who had been operated on many years previously for a patent ductus arteriosus came to our attention for a heart murmur on the left upper sternal border. Echocardiography revealed a still patent duct with moderate shunt. The patient underwent cardiac magnetic resonance imaging, which confirmed patency of the duct, but its diam

Comprehensive time-resolved MRI of peripheral vascular malformations.

OBJECTIVE: The purpose of this study was to evaluate the usefulness of MRI and MR angiography for the assessment of peripheral vascular malformations compared with the usefulness of conventional duplex sonography, venography, and arteriography. SUBJECTS AND METHODS. Nineteen patients (age range, 8-64 years; 11 males and eight females) with clinically diagnosed or suspected angiodyplastic abnormalities of the extremities were examined on a 1.5-T whole-body MR scanner. Using parameters based on a fast localizer sequence, we acquired transverse or coronal T1-, T2-, short tau inversion recovery (S

Early spontaneous occlusion of a vertebral artery dissecting aneurysm caused by subarachnoid hemorrhage: A case report.

We report a case of vertebral artery dissecting aneurysm (VADA) that developed with subarachnoid hemorrhage and was found to be occluded based on subsequent digital subtraction angiography. Few reports have been published on ruptured VADA in which ipsilateral vertebral arteries are occluded. The proper management of this type of aneurysm is controversial. A 44-year-old woman developed a sudden onset headache. Computed tomography and three-dimensional computed tomography were immediately performed and showed subarachnoid hemorrhage and VADA distal to the right posterior inferior cerebellar arte

De Novo Intracranial Arteriovenous Malformation-Case Report and Literature Review.

BACKGROUND: Intracranial arteriovenous malformations (AVMs) have been considered congenital. We present and discuss a case of a child who had no evidence of an AVM at 6 years of age when presenting with parenchymatous hemorrhage due to cavernous angioma and who developed the lesion during 10 years of follow-up. CASE DESCRIPTION: A 6-year-old female presented with parenchymatous hemorrhage and was diagnosed with cavernous angioma of the right occipital lobe. She was treated with lesion removal and remained asymptomatic during the initial follow-up. At age 16, she presented to the emergency dep

Recurrent Secondary Trigeminal Neuralgia Caused by Obliterated Tentorial Dural Arteriovenous Fistula Cured with Surgical Resection: Case Report and Literature Review.

BACKGROUND: Trigeminal neuralgia (TN) caused by a tentorial dural arteriovenous fistula (TDAVF) is quite rare. To date, only 10 cases and 2 small case series have been reported in this regard, and most were treated with either embolization or surgery. Here, we report a unique case of a TDAVF presented as TN, which was embolized with Onyx first and resected later. CASE DESCRIPTION: A 57-year-old male presented with right-sided TN. Magnetic resonance imaging revealed a variceal venous dilation occupying the right lateral pontine cistern and multiple venous flow void signals adjacent to the righ

Bronchial arterial angio-CT: evaluation of intradural and oesophageal enhancement before bronchial arterial infusion.

Ultrafast CT combined with bronchial angiography (BA angio-CT) demonstrated a supply from the bronchial arteries to the oesophagus and spinal cord which is not identified on conventional bronchial arteriography using a digital subtraction technique. 20 patients with bronchial carcinoma and one with lung metastasis were examined using BA angio-CT, before bronchial artery infusion. 20 ml of non-ionic iodinated contrast medium (300 mgI ml-1) was injected into the bronchial artery, and ultrafast CT of the whole mediastinum was commenced when 10 ml had been injected. The 6 mm single slice mode was

Vascular imaging of the mesenteric vasculature.

Imaging of the mesenteric vasculature is crucial in diagnosing vascular disease of the gastro-intestinal tract such as acute or chronic mesenteric ischemia caused by arterial stenosis, embolism or thrombosis, mesenteric vein thrombosis and mesenteric aneurysm or dissection. The reference standard for imaging of the mesenteric vasculature is digital subtraction angiography. However, modalities as duplex ultrasonography, computed tomography angiography and magnetic resonance angiography are developing rapidly and may provide accurate imaging non-invasively. This review provides an up-to-date ove

Revascularization of vertebrobasilar artery occlusion at the chronic stage.

We describe a patient who underwent intracranial angioplasty and Solitaire stent placement for recanalization of a vertebrobasilar artery occlusion 2 months after symptom onset. Computed tomography angiography and digital subtraction angiography showed that both vertebral arteries and the proximal basilar artery were occluded. Balloon angioplasty was performed on a segment of the occluded left vertebral artery and basilar artery, followed by successful detachment of one Solitaire stent. Repeat angiography showed near normal patency of the left vertebrobasilar artery. The patient`s symptoms imp

Intravascular Wrap for Treatment of Basilar Artery Perforator Aneurysm.

Basilar artery perforator aneurysms are very rare and usually present with subarachnoid hemorrhage. High-quality imaging systems with digital subtraction angiography and three-dimensional rotational angiography are crucial for the detection of these posterior circulation vascular lesions, which may still be missed on the initial angiogram. We present the first use of a Flow Re-Direction Endoluminal Device (Microvention, Aliso Viejo, California) for treatment of a ruptured basilar artery perforator aneurysm via snuffbox vascular access and use of cangrelor for antiplatelet management.

Life-threatening pseudoaneurysm of the facial artery after dental extraction: successful treatment with emergent endovascular embolization.

Vascular complications after third molar extraction are very rare but may cause a life-threatening situation. The case presented is that of a 25-year-old male who appeared at our emergency department with severe dyspnea and increasing swelling of the left submandibular area 3 days after dental extraction. CT angiography and digital subtraction angiography demonstrated a pseudoaneurysm of the facial artery. Subsequent emergent endovascular embolization with n-Butyl-2-Cyanoacrylate was performed and the patient had a rapid satisfactory resolution.

A case of congenital absence of the portal vein.

We experienced a girl with congenital absence of the portal vein. She was examined by computed tomography (CT), three-dimensional computed tomographic angiography (3DCTA), digital subtraction angiography and liver biopsy. Nodular regenerative hyperplasia of the liver was detected, presumably due to an abnormal hepatocellular response to absent portal flow. 3DCTA showed that the splenic vein and superior mesenteric vein joined to form a common trunk, which directly entered the right atrium. 3DCTA may be a valuable noninvasive tool for identifying portal malformations.

[Role of multidetectori compted tomography angiography and three-dimensional post-processing in a case of a Valsalva graft in aortic valve repair and replacement with the Cabrol procedure].

Patients with combined valvular-aortic disease undergo surgical reconstruction of the aortic root with a valved composite graft. Two of the techniques used to replace the aortic valve and ascending aorta are Bentall and Cabrol procedures. Cabrol surgical procedure uses a Dacron tube that enables coronary artery revascularization. Graft limb occlusion may however occur. Digital subtraction angiography, magnetic resonance angiography and multidetector computed tomography angiography are useful imaging methods for the evaluation of surgical-related complications. We report the case of a patient t

Failing hemodialysis access grafts: evaluation of complete vascular tree with 3D contrast-enhanced MR angiography with high spatial resolution: initial results in 10 patients.

Ten patients with failing hemodialysis access underwent contrast material-enhanced magnetic resonance (MR) angiography within 7 days before digital subtraction angiography (DSA). MR angiography was performed at 1.5 T by using a multistation multiinjection three-dimensional technique, and contrast material was injected via intravenous cannula. In all patients, MR angiographic images displayed the complete arterial inflow tract from the subclavian artery and access proper. The complete venous outflow tract up to the superior caval vein could be evaluated in all but one patient. DSA showed hemody

Selective conventional arteriography and computed tomography with and without contrast infusion: minimal requirements for the assessment of transient ischemic attacks.

The authors report two unusual cases referred for extracranial bypass surgery. These patients presented with symptoms of transient ischemic attacks (TIAs) and demonstrated the necessity of conventional selective angiography and computed tomography (CT), with and without contrast infusion, as part of the preoperative assessment. The authors discuss how digital subtraction angiography alone is inadequate and how CT without contrast enhancement may fail to visualize lesions that may exhibit TIA-like symptoms. In the face of the multiplicity of preoperative tests used to evaluate patients with TIA

Clinics in diagnostic imaging (28).

A 44-year-old man presented with sudden headache and neck stiffness. Computed tomography (CT) demonstrated subarachnoid haemorrhage. CT and magnetic resonance (MR) angiography showed the cause to be a ruptured anterior communicating artery aneurysm. These findings were confirmed by digital subtraction angiogram and at surgery. The role of imaging in detection of cerebral aneurysms is briefly discussed.

The cervicothoracic junction on chest radiograph.

Fractures of the first rib may be detected by indirect signs, including apical extrapleural fluid collection, pneumothorax, and supraclavicular soft tissue mass. Thoracic outlet syndrome represents a complex series of abnormalities that may be clarified utilizing digital subtraction angiography. Major distortions of alignment can produce bony adaptive changes at the cervicothoracic junction simulating intra-spinal canal tumors.
