Drugs for treatment of peptic ulcers.

Pharmacologic management of peptic ulcer disease continues to evolve with the introduction of diverse types of new therapeutic agents. The i … de histamine H2-receptor antagonists, proton pump inhibitors, anticholinergics, prostaglandins, and antacids. H2-receptor antagonists have become first-line drugs for treatment of uncomplicated duodenal ulcers, gastric ulcers, prevention of ulcer relapse, and mild esophagitis. However, H2-receptor antagonists, like other gastric antisecretory/antiulcer dru …

Effectiveness of almagate in gastroesophageal reflux disease: a post-hoc analysis of a randomized cross-over double-blind study.

Almagate (aluminium-magnesium hydroxycarbonate hydrate) is a widely used antacid in Spain as an over-the-counter formulation. However, data on the control of acidity in patients with gastroesophageal reflux disease (GERD) are scarce. Herein we report a post-hoc analysis of a randomized crossover double-blind trial (ALR-006; data on file), in which equivalent single oral doses (1.5 g) of two formulations of almagate (AlmaxRetard® and Almax®, Laboratorios Almirall, Barcelona, Spain) were both effective for reflux esophagitis.

Gastric tube esophagoplasty.

After a short review of the historical development of replacement of the esophagus, the possibilities, techniques, and results of gastric-tube esophagoplasty are discussed. Ten of 14 patients were followed up for 14 years, two were lost to follow-up and two died. Fistulas, anastomotic strictures, and also reflux could be treated successfully. A single perforation by endoscopy was surgically corrected. A peptic ulcer in the gastric tube was healed with antacid therapy. The gastric tube can be assumed to be a sufficient esophageal substitute. Long-term follow-up is still necessary.

Hydrogen ion, pepsin and bile acid binding properties of hydrotalcite.

The hydrogen ion, pepsin and bile acid binding properties of hydrotalcite (Altacite) were investigated in human gastric juice both in vitro and in vivo. Hydrotalcite is an efficient antacid in vivo and between pH 2,5 and 3,5 it binds and inactivates pepsin in vitro. The results in one case suggest that this also occurs in vivo. Hydrotalcite is capable of removing substantial amounts of bile acids from bile-containing gastric juice in vivo. The findings indicate that hydrotalcite could be a useful agent in the treatment of both gastric and duodenal ulcers.

An Incidental Finding of Asymptomatic Gastric Diverticula.

Gastric diverticula (GD) are the rarest of the gastrointestinal diverticula and are characterized by protrusions of the stomach wall, that can either be congenital or acquired. Despite the fact that the majority of GD are asymptomatic and are detected inadvertently during endoscopy or gastrointestinal (GI) series studies, they might present with a variety of symptoms, including abdominal pain, vomiting, and weight loss. In mild symptomatic instances, GD is treated conservatively with antacids, but surgical excision is indicated for refractory gastric diverticula with persistent symptoms or com…

Gastric and duodenal ulcers during pregnancy.

The frequency, symptoms, and complication rate of peptic ulcer disease appear to decrease during pregnancy significantly. Clinicians, however, often have to treat dyspepsia or pyrosis of undetermined cause because the frequency of pyrosis increases during pregnancy. Physicians are reluctant to perform esophagogastroduodenoscopy (EGD) during pregnancy for pyrosis to reliably differentiate gastroesophageal reflux from peptic ulcer disease. Dyspepsia or pyrosis during pregnancy first should be treated with dietary and lifestyle changes, together with antacids or sucralfate. When symptoms persist,…

Aluminum and renal osteodystrophy A diminishing clinical problem.

Aluminum-associated osteomalacia is a serious complication of advanced renal disease. Sources o f aluminum include the water used for hemodialysis and the gastrointestinal absorption o f aluminum from the antacids used to bind dietary phosphorus in an effort to prevent hyperparathyroidism. Definitive diagnosis is made by bone biopsy and staining for aluminum. Noninvasive testing using the deferoxamine challenge test together with serum PTH levels is also of value in many instances. The goal for the future is to eliminate aluminum-related osteodystrophy by the avoidance of aluminum, control of …

[Barrett's esophagus does not revert after medical or surgical therapy for gastroesophageal reflux].

An evaluation was made of 34 patients having Barrett's oesophagus. Medical therapy consisted of antacids, H2 blockers, orthopramides, and postural measures (20 cases, mean follow-up 2.9 years). A Nissen fundoplication was done in 14 cases (mean follow-up 2.5 years). Clinical, endoscopic and histologic evaluation was done in all patients and pH measurements also done in all operated cases. Clinical results have been excellent in 70% of medically treated patients and in 100% of surgically treated ones, pH measurements were between normal limits in all operated patients. However, recovery of the …

[Efficacy of ambulatory treatment of patients with primary chronic gastroduodenitis].

The authors studied the efficacy of the treatment of 80 patients with primary chronic gastroduodenitis on an ambulatory basis. Use was made of spasmolytics, cholinolytics, antacids, sedative agents together with reflexotherapy, dietetic management and physiotherapy. As a result of the treatment, positive changes were recorded in the clinical, functional (intragastric pH-metry with the sodium carbonate test), endoscopic, histomorphologic, morphometric and immunologic characteristics. The changes in the characteristics indicated were examined both shortly after the treatment (1 to 1.5 month) and…

Treatment and prevention of NSAID induced gastroduodenal mucosal damage.

The use of aspirin and other nonsteroidal antiinflammatory drugs (NSAID) is associated with various degrees of gastroduodenal damage. The agents currently available for the treatment of NSAID induced gastric mucosal damage are histamine2-receptor antagonists, antacids, sucralfate and prostaglandin (PG) analogs. Although all of these agents are effective in healing gastric and duodenal injury if NSAID therapy is discontinued, currently available data suggest that there may be significant differences among these drugs in healing or preventing mucosal injury when NSAID therapy is continued. In pa…

Self-bougienage in the treatment of benign esophageal stricture.

Although recommended for patients requiring frequent dilations in whom surgery is not feasible, self-bougienage for benign esophageal stricture has not been examined critically. Thirteen patients performing self-dilations over a mean period of 4.8 years were evaluated. Dysphagia decreased in all patients and reflux symptoms were well-controlled in most patients with an antireflux regimen of elevation of the head of the bed, antacids, and cimetidine. Major complications of bleeding or perforation were not encountered, although three episodes of esophageal food impaction related to missed home d…

[Hypophosphatemias (author's transl)].

Severe hypophosphatemia may be observed after the ingestion of certain antacids which complex phosphorus in the intestinal lumen. In severe burn cases, during realimentation after denutrition, during intense and prolonged hyperventilations responsible for respiratory alkalosis, in diabetic ketoacidosis and in alcoholics. Hypophosphatemia lead to erythrocyte abnormalities which may eventually approach hemolysis, to phagocytosis and platelet function disorders and to neurological troubles which suggest a metabolic encephalopathy, a myopathy, a metabolic acidosis or a change in hepatic functionin…

[The emergency treatment of a recent burn of the oesophagus (author's transl)].

The emergency treatment of an oesophageal burn consist of: -- avoidance of a certain number of measures which may be considered to be dangerous or useless: gastric lavage, emetic agents, neutralising agents or oesophageal antacids; -- the treatment of shock and of oesophageal and gastric complications, in cases of massive ingestion in which the prognosis may be improved only by active, thorough and prolonged intensive therapy; -- attempting to prevent the development of subsequent stenosis following massive ingestions. The majority of authors are in agreement concerning the usefulness of early…

Medical treatment of gastric and duodenal ulcer.

Antacids prove ready if only temporary relief of the symptoms of ulcers. Physicians have been able to increase the rate of healing of gastric ulcers by advising patients to stop smoking and then either admit them to hospital for bed-rest, or use carbenoxolone sodium as an outpatient therapy. The early trials of some other drugs have shown increase in rate of healing of gastric ulcer, but there is no medical treatment which will prevent the high recurrence rate of benign gastric ulcer. The limited data indicate that adequate medical reduction in acid secretion and enhancement of mucosal resista…

A rapid-floating natural polysaccharide gel-raft with double-effect for the treatment of gastroesophageal reflux disease.

Gastroesophageal reflux disease (GERD) is a prevalent gastrointestinal condition characterized by regurgitating stomach contents into the esophagus, causing mucosal damage or erosion. Clinical physical protection treatment mainly relies on the use of floating rafts. Bletilla striata (BS) is widely regarded as the first-choice drug for treating digestive tract injuries in Chinese Medicine. The rapid-floating gel-raft (B-R) was prepared via a one-step swelling method using natural BS polysaccharide and glyceryl monooleate. Panax notoginseng saponins (PNS) were loaded to further prepare P/B-R acc

Use of food and food-derived products in the treatment of gastritis: A systematic review.

Gastritis is the acute or chronic inflammation of gastric mucosa and is triggered by diverse factors. Treatments used for non-bacterial gastritis include proton pump inhibitors, histamine H2 receptor inhibitors, and antacids, and their use is linked to various side effects. Research on alternative therapeutics using food or food-based products is extensive, mostly in preclinical research. We aimed at documenting the clinical advances in food-based therapies as alternative therapeutics for gastritis. Articles with information on the treatment of gastritis with food or food-based products publis…
