Is antibiotic prophylaxis necessary before midurethral sling procedures for female stress incontinence? A decision analysis.

INTRODUCTION AND HYPOTHESIS: A recent randomized controlled trial (RCT) with and without prophylactic antibiotics (pABX) for midurethral slings (MUS) was terminated early owing to lower than predicted infectious complications in both groups. Adequate power required an unattainable sample size. Because of low infectious risks, omitting pABX may be justified. Since an RCT was not feasible, we aimed to use decision analysis modeling to determine if pABX are necessary for MUS. METHODS: We created a decision analysis model comparing 1-year quality adjusted life years (QALYs) between women who do a

Antibiotic prophylaxis at elective cesarean section: a randomized controlled trial in a low resource setting.

OBJECTIVE: To determine the best time to administer prophylactic antibiotics at Cesarean delivery in order to reduce the postoperative maternal infectious morbidity in a low resource setting. MATERIAL AND METHODS: One hundred term primigravidae with singleton pregnancy were recruited and randomly allocated to two equal groups. Each woman received 2 g intravenous Cefazoline. Women in Group I received it prior to skin incision while those in Group II had it immediately after cord clamping. We measured the following outcome parameters: (1) Surgical site wound infection; (2) Endometritis and (3)

[Descriptive study of intrapartum antibiotic prophylaxis of neonatal infection by group B Streptococcus in Txagorritxu].

INTRODUCTION: GBS is one of the most important causes of neonatal sepsis of vertical transmission (1-2% of the carriers). The infection may involve newborn's death or severe complications. The culture of vaginal and rectal GBS is a screening test and it is performed to all pregnant women between 35-37 weeks of gestation. If it is positive, intrapartum penicillin is prescribed. If it is negative or unknown, the antibiotic per protocol is given according to risk factors: rupture of membranes more than 18 hours, intrapartum temperature, or prematurity. The aim of this study is to quantify women w

Meta-analysis: antibiotic prophylaxis for cirrhotic patients with upper gastrointestinal bleeding - an updated Cochrane review.

BACKGROUND: Antibiotic prophylaxis seems to decrease the incidence of bacterial infections in patients with cirrhosis and upper gastrointestinal bleeding and is considered standard of care. However, there is no updated information regarding the effects of this intervention. AIM: To assess the benefits and harms of antibiotic prophylaxis in cirrhotic patients with gastrointestinal bleeding by performing a systematic review of randomised trials. METHODS: We searched The Cochrane Hepato-Biliary Group Controlled Trials Register, The Cochrane Central Register of Controlled Trials in The Cochrane …

Antibiotic prophylaxis for spontaneous bacterial peritonitis in cirrhotic patients with ascites, without gastro-intestinal bleeding.

BACKGROUND: Spontaneous bacterial peritonitis is frequent among cirrhotic patients, associated with significant morbidity and mortality. Selective intestinal decontamination employing antibiotics is a proposed prophylactic measure. While data regarding this modality among cirrhotic patients with gastrointestinal bleeding exist, there is insufficient data synthesis regarding cirrhotic patients with ascites and no gastrointestinal bleeding. OBJECTIVES: To assess whether antibiotic prophylaxis decreases spontaneous bacterial peritonitis and mortality among cirrhotic patients with ascites and no …

Spondylodiscitis after lumbar microdiscectomy: effectiveness of two protocols of intraoperative antibiotic prophylaxis in 1167 cases.

The role of antibiotic prophylaxis in preventing postoperative lumbar spondylodiscitis is still controversial in medical, ethical, economic, and legal terms. The aim of this retrospective study was to evaluate the efficacy of two intraoperative antibiotic prophylaxis protocols in a large series of lumbar microdiscectomies performed in two different neurosurgical centres. We reviewed the outcome of 1167 patients operated on for a lumbar disc herniation with microsurgical technique, in order to detect the incidence of postoperative spondylodiscitis. Group A included 450 patients operated on in a…

[Antibiotic prophylaxis with amoxicillin/clavulinate (Augmentin) in extended and combined operations in gynecologic cancer patients].

Frequency of postoperation complications at oncological patients deviates in the range from 10 to 70 per cent. The highest frequency of infectious complications is registered after extended and complex operations (cervical carcinoma, cancer of vulva, ovarian cancer). The most frequent aerobic pathogens in oncology are enterococci, staphylococci and streptococci, in the case of urinary tract operations--enterococci and Erscherichia coli. Perioperation prophylaxis with amoxycillin/clavulanate (Augmentin) effective against this microorganisms and also against anaerobic bacteria (usual pathogens o

Role of preoperative antibiotic prophylaxis in preventing postoperative peritonitis in newly placed peritoneal dialysis catheters.

The role of vancomycin and other antibiotics in the treatment of acute peritonitis in peritoneal dialysis (PD) patients is well established. However, the role of preoperative vancomycin or cephalosporins in preventing early infection in newly placed PD catheters remains controversial. We performed a prospective randomized study to examine the role of vancomycin or cefazolin prophylaxis in decreasing the incidence of postoperative peritonitis. Over a 6-year period, 221 patients undergoing 254 permanent peritoneal catheter placement procedures were randomized into three groups. Group I patients

Discontinuation of intravenous antibiotic therapy during persistent neutropenia in patients receiving prophylaxis with oral ciprofloxacin.

To evaluate the mortality and morbidity associated with early discontinuation of intravenously administered antibiotics, we prospectively examined the incidence and cause of recurrent fever in patients with persistent neutropenia who responded to a short course of intravenous antibiotic therapy. Preventive measures included the use of oral ciprofloxacin as prophylaxis for infection by gram-negative bacteria during the entire neutropenic episode. The rate of response to either initial or modified intravenous antibiotic therapy was 96% (149 of 156 episodes of fever). Eighty-five patients had an

Pulp revascularization in reimplanted immature monkey incisors--predictability and the effect of antibiotic systemic prophylaxis.

In 32 monkeys 105 immature maxillary incisors were extracted and reimplanted either immediately or after 30 or 60 min wet or dry storage. Of the monkeys, 17 (group I) did not receive and 15 (group II) received prophylactic treatment with 4 mg/kg doxycycline before extraction and 2 mg/kg for 5 d after reimplantation. The observation time varied from 6 to 8 weeks. After being histologically processed, the material was evaluated with respect to the amount of vital tissue and presence of micro-organisms in the pulpal lumen. A comparison revealed no difference in the results between the groups. The

The effect of antibiotic prophylaxis on patients undergoing total abdominal hysterectomy. I. Effect on morbidity.

A prospective, double-blind study was conducted to evaluate the effect of cephalosporins on morbidity of women undergoing total abdominal hysterectomy. Each patient received a 5 day course of active drug or placebo beginning the night before surgery. Microbiologic studies, including culture and antibiotic sensitivities, were done on each patient pre- and postoperatively. Of the 46 women who received placebo, eighteen (39 per cent) were classified as morbid. Causes of morbidity included operative bed, wound, respiratory, or urinary tract infections. E. coli was the organism most frequently isol
