[Giant condyloma of the anus: tumor of Buschke-Loewenstein (author's transl)].

The authors report a case of a Buschke-Loewenstein tumor located at the anus. Initial treatment with Bleomycin was remarkably effective. A recurrence 3 months later did not respond any more to Bleomycin. Multiple surgical resections were performed in association with this form of chemotherapy.

Anal complication in Behçet's syndrome.

A 12 year-old boy suffering from Behçet's syndrome with an anal complication was presented. He had several anal ulcers with surrounding polypoid proliferations. Operative treatment and local application of steroid hormone to anal ulcer failed, whereas the general administration of steroid hormone improved his allover state so that the anal lesion almost completely healed. Dose of steroid hormone was gradually tapered off and finally stopped without any signs of recurrence. Anal ulcer seems to represent a rare complication of Behçet's syndrome.

The impact of physician supply on avoidable cancer deaths in Germany. A spatial analysis.

BACKGROUND: Measures of avoidable deaths incorporate the notion that deaths from certain causes should not occur within specified age groups given effective prevention or timely and appropriate access to health care. The present study investigated the impact on specific types of avoidable cancer deaths (ACD) of regional variations in the supply of health services over five years using German districts (Kreise und kreisfreie Städte) as units of analysis. METHODS: Age-standardized, average ACD rates were calculated for 2000-2004 for each of the 439 districts in Germany using unit-record mortali

Anal Cancers in Previously Screened Versus Unscreened Patients: Tumor Stage and Treatment Outcomes.

BACKGROUND: Targeted screening programs for patients at high risk for anal squamous-cell carcinoma have been proposed; however, the evidence in support of screening remains unclear. OBJECTIVE: This study aimed to determine whether screening high-risk patients (predominantly those living with HIV) detected squamous-cell carcinoma at an earlier stage compared to the routine practice of not screening. DESIGN: This is a cohort study. SETTINGS: This study was conducted at a quaternary care center in Canada. PATIENTS: Included patients were at least 18 years old with a pathologic diagnosis of in

Early primary anorectal malignant melanoma treated with endoscopic submucosal dissection: a case report.

BACKGROUND: Anorectal malignant melanoma (ARMM) is a rare disease accounting for less than 1% of primary anorectal malignancies. Here we first present a case of early primary anorectal malignant melanoma completely resected by endoscopic submucosal dissection (ESD). METHODS AND RESULTS: A 43-year-old woman visited our hospital because of suspected anal melanoma found by routine colonoscopy in her local hospital. Following series of tests including CT, MRI, and whole-body PET-CT did not show any evidence of metastasis. The lesion was removed by the method of ESD in en bloc and no delayed bleed

[A Case of Anal Canal Cancer with Skin Invasion to Which Abdominoperineal Resection and Perineum Reconstruction Were Performed after Neoadjuvant Chemoradiotherapy].

A 74-year-old man visited our hospital with an awareness of anal mass and bleeding. He was diagnosed as adenocarcino- ma of anal canal with wide spreading skin invasion. After neoadjuvant chemoradiotherapy(radiationtotal 45 Gy/25 Fr; cape- citabine 825mg/m2)was performed to reduce the mass volume, laparoscopic abdominoperineal resection with large perineum skin resection and lateral lymph node dissection was carried out. The perineal defect was repaired with a rectus abdominis musculocutaneous flap. Six days after surgery, the rectus abdominis musculocutaneous flap necrotized, and second perin

Precise endoscopic and pathologic features in a Crohn's disease case with two fistula-associated small bowel adenocarcinomas complicated by an anal canal adenocarcinoma.

The patient was a 40-year-old man who had suffered from Crohn's disease (CD) for 19 years and developed an intractable perianal fistula and two strictures in the small bowel. Dilatation of the two strictures using double-balloon endoscopy did not improve the subileus symptoms. An anal canal adenocarcinoma was also detected using double-balloon endoscopy. The ileum and rectoperianal area were partially resected, and a precise immunohistochemical pathologic assessment revealed that all three lesions were fistula-associated adenocarcinomas. Accumulating endoscopic findings of CD-associated cancer

[Perianal Buschke-Löwenstein tumour].

The Buschke-Löwenstein tumour is an extremely rare, slow-growing cauliflower-like mass, also known as giant condyloma acuminantum or verrucous carcinoma. It is located in the anogenital region and is characterized by its large size, locally destructive behaviour and a high recurrence rate. It has not the potentiel to metastasize but malignant transformation can occur. We report a case of a 49-year-old male patient with a 17 × 10 cm large perianal Buschke-Löwenstein tumour where histologic findings showed the presence of verrucous carcinoma.

Paget's disease of the perianal region--an aggressive disease?

BACKGROUND: Perianal Paget's disease is a rare entity, often associated with internal malignancies and a poor prognosis. METHODS: A chart review of patients with perianal Paget's disease who presented consecutively to Mayo during 25 years (starting in January 1970) was made. Patients included had Paget's disease located in or around the anus (3 cm). Patients were excluded for evidence of spread of vulvaperineal lesions or pagetoid extension of a rectal adenocarcinoma. Histology slides were reviewed, and immunohistochemistry was applied to confirm diagnoses. Follow-up was updated in all patien

Anogenital warts of the condyloma acuminatum type in HIV-positive patients.

Anogenital warts of the condyloma acuminatum type seem to occur quite often during HIV infection. These warts--according to our study--are not commonly caused by malignancy-associated human papilloma virus types, but by types 6 and 11 as seen in the nonimmune-compromised population. Widespread condylomata acuminata may appear in rather early stages of HIV infection and they may therefore represent early warning signs of HIV infection.

Synchronous and metachronous colon lesions in squamous cell carcinoma of the anal canal.

The charts of 79 patients with a histologic diagnosis of squamous cell carcinoma of the anal canal were reviewed to determine the incidence of synchronous and/or metachronous colon lesions. Forty-six patients underwent an adequate preoperative evaluation of the colon and rectum. Ten patients underwent colonoscopy, 33 patients underwent a barium enema, and three patients underwent colonoscopy and a barium enema. In this group six adenomatous polyps and three hyperplastic polyps were discovered. An adenocarcinoma of the sigmoid colon was found in another patient six months prior to the diagnosis

The association of carcinoid tumors of the rectum with myelofibrosis: report of two cases.

Two patients with myelofibrosis, a primary malignancy of the hematopoietic system, have developed carcinoid tumors of the rectum. This development is rare, as rectal carcinoids are more often associated with other carcinomas of the gastrointestinal tract. Rectal carcinoid tumors are, in general, less commonly associated with second malignancies than are carcinoid tumors of foregut and midgut origin.

The radiology corner. Anorectal melanoma.

A case of malignant anorectal melanoma is presented together with a short review of the literature. The lesion arises from the melanocytes normally present within the squamous epithelium of the anal canal. It presents as a small polypoid tumor and spreads rapidly into rectum, perirectal tissues, pelvic and inguinal nodes. It should be considered in the differential diagnosis of sessile lesions located or involving the anal canal.

Trends in primary surgery and overall survival in non-metastatic anal cancer: a population-based analysis.

BACKGROUND: Anal cancer incidence is rising in the United States, now exceeding 10 000 cases annually. Chemoradiation (CRT) is the standard curative-intent treatment for non-metastatic squamous cell carcinoma of the anus (SCCA), with surgery generally reserved for non-responders with persistent or progressive disease. Recent trials have refined management, supporting the assessment of response at 26 weeks before considering surgery. However, contemporary population-level patterns of upfront primary surgery and associated survival trends remain incompletely described. METHODS: Using the Survei

Microanatomy of the cervical and anorectal squamocolumnar junctions: a proposed model for anatomical differences in HPV-related cancer risk.

Human papilloma virus (HPV) infection causes cancers and their precursors (high-grade squamous intraepithelial lesions) near cervical and anal squamocolumnar junctions. Recently described cervical squamocolumnar junction cells are putative residual embryonic cells near the cervical transformation zone. These cells appear multipotential and share an identical immunophenotype (strongly CK7-positive) with over 90% of high-grade squamous intraepithelial lesions and cervical carcinomas. However, because the number of new cervical cancers discovered yearly world wide is 17-fold that of anal cancer,

Anal Human Papillomavirus Infection Progression to Disease Among Men Who Have Sex With Men.

BACKGROUND: Critical gaps exist in our understanding of the progression from anal human papillomavirus (HPV) infection to same-type HPV-associated anal disease in human immunodeficiency virus (HIV)-negative men who have sex with men (MSM). We conducted a post hoc analysis of a phase 3 randomized 4-valent HPV vaccine trial (NCT00090285) to assess the natural progression of anal HPV infections to associated anal lesions among MSM. PARTICIPANTS: A total of 602 HIV-negative MSM aged 16-27 years. METHODS: HIV-negative MSM aged 16-27 years enrolled in the placebo arm from 18 countries were include
