This report describes a rare case of bilateral congenital aphakia with anterior segment malformation in a 12-week-old male cocker spaniel puppy. The puppy was initially suspected of having glaucoma and was subsequently diagnosed with anterior segment dysgenesis and aphakia resulting in blindness. Ophthalmic examination showed corneal opacity, absence of pupillary openings, and negative pupillary light reflexes. B-mode and high-frequency ultrasonography showed absence of the lens, poorly defined anterior chambers, and hyperechoic tissue adherent to the cornea, consistent with anterior segment d…

We describe gross and histopathological features of multiple ocular and neuro-ophthalmic abnormalities in a fox squirrel (Sciurus niger). Ophthalmic findings included severe bilateral microphthalmos, with the right eye more affected than the left. Histopathology confirmed severe microphthalmia, aphakia, disorganized retinal tissue and small optic nerves, as well as agenesis of the optic chiasm and optic tract. This combination of neuro-ophthalmic abnormalities has not been previously described in wild animals.

A new intrascleral fixation technique for a standard 3-piece posterior chamber intraocular lens (PC IOL) using a simple guide is described. To secure the haptics, 27-gauge scleral tunnels are created. Both haptics are secured extraocularly. The leading haptic of the PC IOL placed in the cartridge is released partly and introduced into the lumen of the guide. The PC IOL is inserted into the eye, and the leading haptic is externalized by pulling the guide gently through the 27-gauge sclerotomy site. The trailing haptic is externalized using the same procedure. This technique facilitates achievin

AIM: To assess motor functioning in children aged 4 years 6 months enrolled in the Infant Aphakia Treatment Study, and to determine contributions of visual acuity and stereopsis to measured motor skills.
METHOD: One hundred and four children (53% female) with unilateral aphakia randomized to intraocular lens or contact lens treatment were evaluated at 4 years 6 months (age range 4y 6mo-4y 11mo) for monocular recognition visual acuity, motor skills, and stereopsis by a traveling examiner masked to treatment condition. Motor skills were assessed with the Movement Assessment Battery for Children…

PURPOSE: To compare the intraocular pressure (IOP) measurements obtained by the Icare and the hand-held Goldmann applanation tonometer (also called Perkins) in aphakic children after congenital cataract surgery.
METHODS: We investigated 125 children with aphakia after congenital cataract surgery in this study. A younger group (3 to 30 mo) and an elder group (31 to 72 mo) were divided in those patients by their age. In the younger group, all measurements were performed under sedation using chloral hydrate. Axial length of the eye and central corneal thickness (CCT) were also measured from all …

PURPOSE: To describe a video-documented assessment of cataract type in the eyes of patients with monocular infantile cataract who were enrolled in the Infant Aphakia Treatment Study.
METHODS: The Infant Aphakia Treatment Study is a randomized clinical trial in which the investigators compared intraocular lens (IOL) versus contact lens correction in 114 infants, aged 28 days to <7 months. A total of 83 videos were available for morphological analysis of cataract. Three examiners reviewed all surgical recordings and agreed on the cataract characteristics by using a score sheet to record the len…

Congenital primary aphakia (CPA) is a rare developmental disorder characterized by the absence of lens, the development of which is normally induced during the 4th-5th wk of human embryogenesis. This original failure leads, in turn, to complete aplasia of the anterior segment of the eye, which is the diagnostic histological criterion for CPA. So far, the genetic basis for this human condition has remained unclear. Here, we present the analysis of a consanguineous family with three siblings who had bilateral aphakia, microphthalmia, and complete agenesis of the ocular anterior segment. We show …

PURPOSE: To compare the preoperative evaluation of secondary intraocular lens (IOL) implantation in aphakic adults following cataract extraction in childhood using slitlamp examination and high-frequency ultrasound (HFU).
METHODS: In a prospective case series, patients who had had lensectomies for congenital cataracts without primary implantation IOL were evaluated for secondary IOL insertion. Slitlamp examination and HFU were performed to study the degree of ciliary sulcus support and iridocapsular adhesions. The choice of IOL (posterior sulcus supported or anterior chamber) was compared usi

Mouse aphakia (ak) is a recessive phenotype that spontaneously occurs in the 129/Sv-SlJ strain and is characterized by small eyes that lack a lens. We have recently identified a homeobox-containing gene, Pitx3, and have shown that it is expressed in the developing lens and maps to chromosome 19 close to ak in mouse. Human PITX3 gene was found to underlie anterior segment dysgenesis and cataracts. We have now obtained the entire sequence of the mouse Pitx3 gene including 10 kb of the 5' region and 5 kb of the 3' region. Of several microsatellite repeat regions identified within the Pitx3 sequen…

PURPOSE: To study the role of epikeratophakia in treating pediatric unilateral aphakia after traumatic cataract extraction.
METHODS: We performed epikeratophakia in 43 children to treat aphakia after traumatic cataract extraction, using the corneal lenses made by ourselves. The rehabilitation of the the postoperative vision and the prevention of amblyopia were observed with a mean follow-up period of 20 months.
RESULTS: All the lenses remained transparent. Postoperatively, 31 cases (72.1%) achieved uncorrected visual acuities (V.A) over 0.2, 32 cases (74.4%) achieved corrected V.A over 0.4. …

Eight patients with secondary strabismus as a result of longstanding unilateral traumatic cataract or uncorrected aphakia were seen in the Orthoptic Department between April 1993 and September 1994. At the initial orthoptic examination three patients did not have any binocular vision and five patients demonstrated some form of binocularity although the fusional amplitude was seriously reduced. Three of these five patients showed just a small heterophoria and a better fusional amplitude within six months after a good optical correction and/or prescription of prisms with or without strabismus su…

The results of Nd:YAG laser capsulotomy performed in 94 eyes of 90 patients (48 males and 42 females) are presented. Visual acuity 0.5 and better was achieved in 58.4% of cases. The authors discussed the techniques of the operations and some possible complications.

A retrospective study of 107 cases of retinal detachment in aphakic and pseudophakic eyes showed different types of retinal break depending upon surgical method. Three techniques were studied: intracapsular extraction (ICCE), intracapsular extraction with anterior chamber lens implant (ICCE, AC-IOL), and extracapsular extraction with posterior chamber lens implant (ECCE, PC-IOL). Results were compared for each technique with and without peripheral iridectomy. Typical aphakic and pseudophakic retinal breaks were found only in eyes having undergone peripheral iridectomy: our conclusion is that p

The authors report their procedure of contact lens adaptation in child and particularly after traumatic cataract. Adaptation in the briefest time following cataract surgery is absolutely necessary in order to undertake jointly the orthoptic treatment of amblyopia.

Although hydrogel extended-wear contact lenses (EWCLs) have been used extensively in the correction of aphakia in neonates, little is known about the effects of these lenses on infant corneas. Recent studies have demonstrated that long-term contact lenses can induce endothelial morphometric changes, including an increased coefficient of variation (CV) of mean endothelial cell area. Using wide-field specular microscopy, the authors studied 11 eyes of 10 patients, 1 to 3 years of age who, after lensectomy for congenital cataracts, wore EWCLs for the correction of aphakia. Except for two corneas …
