Sleep apnea syndrome: implications on cardiovascular diseases.

Global risk assessment is the standard of care for coronary artery disease management. In this setting, sleep apnea syndrome, which includes obstructive sleep apnea and central sleep apnea, is being increasingly recognized as a potentially modifiable risk factor for coronary artery disease. Emerging evidence points toward a cause and effect relationship between sleep apnea syndrome and medical conditions like insulin resistance, hypertension, heart failure, and myocardial ischemia. The effects of sleep apnea on coronary artery disease can be independent of many traditional risk factors. Contin…

Central sleep apnea and Cheyne-Stokes respiration.

Cheyne-Stokes respiration with central sleep apnea (CSR-CSA) is a form of periodic breathing, commonly observed in patients with heart failure (HF), in which central apneas alternate with hyperpneas that have a waxing-waning pattern of tidal volume. Uniform criteria by which to diagnose a clinically significant degree of CSR-CSA have yet to be established. CSR-CSA is caused by respiratory control system instability characterized by a tendency to hyperventilate. Central apnea occurs when Pa(CO(2)) falls below the threshold for apnea during sleep due to ventilatory overshoot. Patients with CSR-C…

[Twin pregnancy and severe obstructive sleep apnea].

Obstructive sleep apnea during pregnancy, associated with arterial hypertension, pre-eclampsia and adverse outcome of the newborn, has been described. Usually it can successfully be treated with non-invasive ventilation. A 36-year-old, twin-pregnant woman at 28 + 6 weeks of gestation presented with the complaints of snoring and nocturnal oxygen desaturations. Polysomnography confirmed the diagnosis of severe obstructive sleep apnea (respiratory disturbance index [RDI] 104/h, minimal oxygen saturation in pulse oximetry [SpO2/min] 75%). First therapeutic approaches including oxygen supplementati…

Association between habitual naps and sleep apnea.

STUDY OBJECTIVES: Taking an afternoon nap (siesta) is a common habit. A number of studies have shown an increased cardiovascular mortality in habitual nappers. Afternoon nappers have anthropometric characteristics similar to those of patients with sleep apnea. Nappers appear to suffer from more sleep apnea, which could contribute to cardiovascular disorders. Our aim was therefore to determine the association between sleep apnea and napping, as well as to analyze the relationship of sleep apnea and napping with hypertension. DESIGN: Case-control study. SETTING: Residents in the town of Cacere…

[Combination therapy for severe sleep apnea syndrome].

We assessed the treatment of severe SAS (sleep apnea syndrome) patients who had an AHI (apnea hypopnea index) over 100. Eleven (3.3%) of the 374 patients who came to our hospital between May 2002 and December 2003 had an AHI over 100. They received CPAP (continuous positive airway pressure) therapy as initial therapy, and the AHI recovered within normal limit in the five patients who did not have tonsillar hypertrophy. The other six patients had tonsillar hypertrophy, and the effect of CPAP was poor. Two of the six patients with tonsillar hypertrophy, underwent UPPP (uvuropalatopharyngoplasty)…

Olfactory stimulation prevents apnea in premature newborns.

OBJECTIVE: Methylxanthines and doxapram are currently used to treat apnea of prematurity but are not fully effective and often present undesirable side effects. The present study examines whether exposure to an odor known to modulate the infant's respiratory rate could reduce the frequency of apneic spells. METHOD: Fourteen preterm newborns born at 24 to 28 gestational weeks presenting recurrent apnea despite caffeine and doxapram therapy were exposed to a pleasant odor diffused during 24 hours in the incubator. Efficiency of the olfactory treatment was judged by comparing frequency and sever…

Circulating free nitrotyrosine in obstructive sleep apnea.

Obstructive sleep apnea (OSA) has been increasingly linked to cardiovascular disease, endothelial dysfunction, and oxidative stress, generated by repetitive nocturnal hypoxemia and reperfusion. Circulating free nitrotyrosine has been reported as a novel biomarker of nitric oxide (NO)-induced oxidative/nitrosative stress. Nitrosative stress has been implicated as a possible mechanism for development of cardiovascular diseases. We tested the hypothesis that repetitive severe hypoxemia resulting from OSA would increase NO-mediated oxidative stress. We studied 10 men with newly diagnosed moderate …

[Obstructive sleep apnea in children--pathophysiology update].

The obstructive sleep apnea syndrome (OSAS) is a common and potentially serious problem during childhood. Airway obstruction during sleep may occur due to narrowed bony passages in conditions such as congenital craniofacial anomalies or in the collapsible segments of the upper airways (naso/hypo pharynx). Adenotonsillar hypertrophy has been considered as the main and most important causative factor for OSAS in the pediatric population. However, adenotonsillar hypertrophy is not the sole cause for this syndrome. Normal infants and children have relatively narrowed upper airways. They maintain a…

Acetazolamide treatment for infantile central sleep apnea.

Central sleep apnea is a common respiratory pattern in healthy neonates. Nevertheless, frequent central sleep apnea associated with drops in oxygen saturation may contribute to infantile morbidity. Recently, low-dose acetazolamide was shown to reduce symptomatic central sleep apnea in adults. We treated 12 infants, median conceptional age 42 weeks (range, 40-44 weeks), with central sleep apnea. In all cases, the central apnea index was >40/h total sleeping time (apnea > or = 3 sec). The cumulative duration of drops in oxygen saturation below 90% was more than 3 min/h total sleeping time. All i…

Central sleep apnea and chronic heart failure.

Central sleep apnea with Cheyne-Stokes respiration (CSR) during sleep affects about 40 % of patients with chronic heart failure (CHF). During CSR simultaneous periodic fluctuations in wakefulness and respiration with accompanying changes in blood pressure and heart rate are observed. CSR can be described as an oscillation of the ventilatory feedback loop controlling respiration. The major synergistically acting mechanisms causing this oscillation include reduced body stores of oxygen and carbon dioxide, hyperventilation with concomitant hypocapnia, prolonged circulation time, and a relatively …

[Pharyngeal musculature and obstructive sleep apnea syndromes].

BACKGROUND: Although there are several efficacious vaccines against COVID-19, vaccination rates in many regions around the world remain insufficient to prevent continued high disease burden and emergence of viral variants. Repurposing of existing therapeutics that prevent or mitigate severe COVID-19 could help to address these challenges. The objective of this study was to determine whether prior use of bisphosphonates is associated with reduced incidence and/or severity of COVID-19. METHODS: A retrospective cohort study utilizing payer-complete health insurance claims data from 8,239,790 pat

[Pharyngeal muscles and sleep obstructive apnea syndrome].

An analysis of the way the pharyngeal musculature modulates the caliber of the pharynx is important to better understand and treat obstructive sleep apnea syndromes. The caliber of the pharynx at the soft palate depends on the action of the tensor veli, the palatoglossus, the palatopharyngeus and the uvula muscles. At the ligual level, the action of the genioglossus and the geniohyoideus predominate. These different muscle groups contract in co-ordination before the diaphragm contracts. Their activity is diminished and disorganized during sleep. These muscles appears to have a histological com…

[Sleep apnea syndromes in multiple system atrophy].

Multiple-system atrophy (MSA) is characterized by progressive autonomic failure with cerebellar, pyramidal and extrapyramidal signs. In MSA patients laryngoscopy often reveals unilateral or bilateral abductor vocal fold palsies. Snoring is very common. However, assessment of sites of obstruction and their severity is needed. We report our clinical experience in managing a 56-year-old, obese male MSA patient (1.76 m height, 100 kg weight, BMI 32.3 kg/m2) who was admitted for evaluation of snoring and excessive daytime sleepiness. Endoscopy while awake demonstrated an incomplete abductor vocal f
