Laparoscopic Appendectomy for Perforated Appendicitis Caused by a Fish Bone: A Case Report.

Foreign body ingestion is relatively common, with most objects passing through the gastrointestinal tract without complications. However, so … unding fat stranding, confirming acute appendicitis caused by appendicolith. Emergency laparoscopic appendectomy revealed a markedly swollen appendix with a central perforation. A 10-mm wedge-shaped fish bone was identified within the appendiceal lumen, confirming fish bone-induced perforated appendicitis. Histopathological examination confirmed severe inflamm …

Feasibility of Laparoscopic Appendectomy Among Children in Nepal: A Safe and Preferred Approach.

Background Acute appendicitis is a common condition in children, and appendectomy is the treatment of choice. The laparoscopic approach has many advantages over the open approach and should be the first choice. Open appendectomy is still a common practice in Nepal. Although laparoscopic appendectomy offers advantages over open surgery, its feasibility in the pediatric population of resource-limited settings like Nepal has not been well studied. This study was conducted to determine the feasibility of laparoscopic appendectomy in terms of technical difficulty, operating time, complications, and…

Comparison of maternal and fetal outcomes of laparoscopic versus open appendectomy in pregnancy.

Acute appendicitis in pregnancy is a condition that can cause both maternal and fetal complications, and in patients who have decided to undergo surgery, laparoscopic appendectomy (LA) or open surgery (laparotomy appendectomy [LAA]) options are available. In this study, we aim to shed light on the relative advantages and disadvantages of the 2 surgical techniques. This retrospective study included 56 pregnant patients (15 LA, 41 LAA) who underwent surgery with the diagnosis of appendicitis at Ondokuz Mayis University Medical Faculty Hospital between 2015 and 2025. Demographic data, gestational…

The association of appendectomy with prognosis and tumor-associated macrophages in patients with colorectal cancer.

The vermiform appendix plays an important role in colorectal immunity and the homeostasis of the gut microbiome. We aimed to evaluate the prognostic value of prior appendectomy for patients with colorectal cancer (CRC). This study revealed that prior appendectomy is an independent risk factor for the prognosis of patients with CRC, based on a multicentral CRC cohort. We further demonstrated that appendectomy induced a poor prognosis of CRC through the depletion of M1 macrophage cells in AOM-induced mice, which was confirmed in age-, sex-, and location-matched patients' cohorts and orthotopic m…

Appendectomy and Risk of Prostate Cancer in the Health Professionals Follow-up Study.

BACKGROUND: Appendectomy is a common surgical procedure to treat appendicitis. Limited studies have examined its association with prostate cancer, with one large cohort study suggesting a significantly increased risk of overall and advanced prostate cancer, especially among younger men. METHODS: A total of 49,104 men in the Health Professionals Follow-up Study were followed from 1986 to 2016. Cox proportional hazards models were applied to evaluate the association between self-reported history of appendectomy and risk of overall and subtype-specific prostate cancer, adjusted for multiple risk…

An International Consensus Survey among Pediatric Surgeons on the Role of Appendectomy in Malrotation.

INTRODUCTION: Ladd's procedure, originally described in 1936 for the treatment of malrotation, does not traditionally include appendectomy as a standard step. We conducted a multinational survey to investigate the current consensus on the role of appendectomy in Ladd's procedure. METHODOLOGY: An anonymous online survey was distributed to pediatric surgeons worldwide. The survey collected demographic data and explored surgical preferences related to the management of malrotation. Open-ended questions were used to assess the opinions regarding the necessity of appendectomy, decision-making fact…

Significance of Histopathology of Appendectomy Specimens: Analysis From a Teaching Hospital of Pakistan.

Background Histopathology of a tissue specimen plays a crucial role in formulating the final diagnosis of any disease. It confirms whether the histopathological findings are in correspondence with the clinical diagnosis and thus suggests an optimal management plan. Standard surgical practices guide that every human tissue specimen must undergo postoperative tissue analysis unless indicated otherwise. Objective To determine the significance of histopathology in determining the final diagnosis of appendectomy specimens. Materials and methods This retrospective clinical study conducted in May 202…

Foreign Body Ingestion by a Psychiatric Patient Requiring an Appendectomy: A Case Report.

In the surgical field, the correct approach to the psychiatric patient represents a medical challenge, given the special considerations to be taken in the individualization of their diagnosis and treatment. We present an uncommon case of a 29-year-old patient with associated psychiatric pathology who presented to the emergency room after the introduction of two foreign bodies into the nasal cavity. After the endoscopic removal of one of the foreign bodies, the X-ray follow-up shows a second foreign body into the esophagus, which progressed to the vermiform appendix, causing the classical clini

The Effect of Body Mass Index on Patients' Length of Stay Post-appendectomy.

BACKGROUND AND AIM: Appendicitis is defined as the appendix's inflammation, which requires an appendectomy for treatment. Obesity is one of the risk factors for post-surgical complications in appendicitis. This study aimed to explore obesity's influence on hospital length of stay among patients with appendicitis in Taif, Saudi Arabia. METHODS: The study subjects consisted of both children and adults with appendicitis who were admitted to the hospital during 2021 and 2022. The patients were divided into three groups according to body mass index (BMI) following the WHO criteria: normal weight (…

Laparoscopic approach for the treatment of acute complications after appendectomy: a systematic review.

INTRODUCTION: The significance of laparoscopy after appendectomy is still unclear, despite the fact that it is thought to be an appropriate technique for the detection and management of postoperative complications. We aimed to determine the incidence, risk factors, diagnosis, and laparoscopic approach for the treatment of acute complications after appendectomy performing a systematic review of the literature. EVIDENCE ACQUISITION: We performed systematic review of the PubMed/MEDLINE, Embase and GoogleScholar bibliographic databases between 1995 and 2022 regarding laparoscopic resolution of ea…

Ketorolac use and risk of bleeding after appendectomy in children with perforated appendicitis.

BACKGROUND: Ketorolac is an opioid sparing agent commonly used in children. However, ketorolac may be avoided in children with peritonitis owing to a possible increased risk of bleeding. METHODS: A retrospective cohort study of healthy children 2-18 years who underwent appendectomy for perforated appendicitis was performed using the Pediatric Health Information System (2009-2019). Multivariable logistic regression was used to evaluate the association between perioperative ketorolac use and postoperative blood transfusions within 30 days of surgery, adjusting for patient and hospital level fac…

Is appendectomy always the adequate treatment? Clinical manifestations of isolated actinomycosis in appendix.

Isolated appendiceal actinomycosis is a rare chronic progressive suppurative infection. Causative agent in humans is gram-positive saprophytic an- aerobic bacteria, Actinomyces Israelii. Herein, it was aimed to present a case of acute appendicitis that developed in a 54-year-old female due to isolated appendiceal actinomycosis. Diagnosis of appendiceal actinomycosis causing acute appendicitis is generally performed postoperatively histopathologi- cally, and appendectomy alone is not sufficient in treatment. It is an important point that should be considered by clinicians that definitive treatm…

Incidental Prophylactic Appendectomy Is Associated with a Profound Microbial Dysbiosis in the Long-Term.

Incidental prophylactic surgeries are performed in certain situations. Incidental prophylactic appendectomies were common practice within opened bariatric surgeries. The gut microbiota has emerged as an important actor within the homeostasis of the host. A new hypothesis has been formulated about the appendix function in relation to gut microbiota. Our objective was to study the gut microbiota profiles of patients that had suffered from an incidental prophylactic appendectomy during their bariatric surgeries, while comparing them to patients whose appendixes had remained intact. A case-control…

Influence of prior appendectomy and cholecystectomy on Clostridioides difficile infection recurrence and mortality.

BACKGROUND: Remote appendectomy was linked to increased incidence of Clostridioides difficile infection (CDI). We evaluated the effect of absence of vermiform appendix and/or gallbladder on recurrence rate and severity of CDI. METHODS: We assessed a systemwide patient cohort diagnosed with initial CDI in 2014 (n = 250). The primary outcome was recurrence. RESULTS: Appendix and gallbladder were absent among 47 and 64 patients, respectively. CDI recurrence rate was similar among patients without and with appendix (24/47, 51.1% versus 90/203 patients, 44.3%; p = 0.404) and similar among patient…

The Influence of the Appendiceal Base Diameter on Appendix Stump Closure in Laparoscopic Appendectomy.

BACKGROUND: Closure of the appendiceal stump is the most critical part of laparoscopic appendectomy. Establishing the average diameter of the inflamed appendix, and forming the appropriate size of clip, endoloop or stapler length, would make this critical phase of laparoscopic appendectomy easier. METHODS: One hundred and fifty consecutive patients, with the diagnosis of acute appendicitis, were included in this study and divided into three groups according to the histological verification of the status of the infection, as follows: phlegmonous, gangrenous and perforated forms of acute append…

Transumbilical Laparoscopic-Assisted Appendectomy in the Treatment of Acute Uncomplicated Appendicitis in Children.

Transumbilical laparoscopic-assisted appendectomy (TULAA) is increasingly being performed worldwide. The authors report their experience in the treatment of acute uncomplicated appendicitis in children with TULAA. From January 2008 to December 2012 all types of acute appendicitis were divided, according to the clinical and ultrasonographic findings, into complicated (appendiceal mass/abscess, diffuse peritonitis) and uncomplicated. Complicated appendicitis was treated by open appendectomy (OA). All patients with the suspicion of uncomplicated appendicitis were offered TULAA by all surgeons of …
