Arthritis in cystic fibrosis.

We have confirmed previous observations of a transient, non-disabling recurrent arthritis in patients with cystic fibrosis. This arthritis differs from classic rheumatoid arthritis, is frequently associated with skin arthritis lesions, and its occurrence is unrelated to the severity of lung disease.

Bronchiolitis in rheumatoid arthritis.

Bronchiolitis in association with rheumatoid arthritis has been reported, to our knowledge, in 18 patients to date. In some cases use of penicillamine has been strongly associated with the development of bronchiolitis. Most of the reported cases are described as having marked irreversible airways obstruction and hyperinflation. We describe a patient with rheumatoid arthritis whose respiratory tract symptoms began during gold therapy. Physiologic studies showed marked lung hyperinflation without pathologic findings in forced dry spirometric study. On open lung biopsy a mild degree of granulomat…

Arthritis in Roman Britain.

The pattern of arthritis in Roman Britain was investigated by examining the skeletons of 416 adults from the Roman cemetery at Poundbury Camp near Dorchester, Dorset. The mean height of the people was not much less than that of the current British population, and the prevalence of right handedness was similar to our own. There was a high prevalence of osteoarthritis for such a relatively young community, with particularly severe changes in the vertebral column. The pattern of joints affected by osteoarthritis was different from that seen now, but the prevalence of vertebral ankylosing hyperost…

Pleurisy in rheumatoid arthritis.

In 157 women and 77 men with rheumatoid arthritis, observed for a mean of 5.7 years, the annual incidence of pleural effusion was found to be 0.34% in the women and 1.54% in the men. In 4 pleural fluids the fluid-to-serum ratio for Waaler-Rose titres was 1:4, which agrees with the ratios for other proteins and does not suggest a pathogenetic role of the rheumatoid factor. In two other pleural fluids the findings suggested immune mechanisms implicating the rheumatoid factor. In general, the data, both clinical and any other, was compatible with the possibility of multiple etiological and pathog…

Septic arthritis of the wrist.

Twenty-eight patients (two of them children) with septic arthritis of twenty-nine wrists were treated with early surgical drainage, parenteral antibiotics, and early motion after surgical decompression. The etiology was trauma in seventeen patients, and Staphylococcus aureus was the organism that was most commonly recovered on culture. In twenty-two patients (twenty-three wrists) who were followed for six months to nine years there were no recurrences. The results were evaluated in terms of range of motion, grip strength, and subjective complaints of discomfort and disability. Of the ten wrist…

Synovectomy in bacterial arthritis.

Twenty patients with bacterial arthritis with either delay in diagnosis or no response to treatment are presented. Synovectomy, even when performed after 5 days and up to 4 weeks later, could prevent joint destruction in the knee but not in the hip. The average follow-up time was 5 (2-9) years.

Arthritis in Behcet's syndrome.

The clinical and radiological features of fourteen patients with Behcet's syndrome have been reviewed with particular reference to joint disease. Thirteen patients (93%) suffered recurrent peripheral oligo- or poly-arthritis, this symptom being the presenting complaint in two (14%). There were no radiographic abnormalities in peripheral joints. Ten patients (71%) complained of low back pain and in seven (50%) there was a mild erosive sacro-iliitis. Five patients (36%) had an enthesopathy (calcaneal spurs) one of whom also demonstrated vertebral body squaring. Patients with sacro-iliitis did no…

Arthritis in Whipple's disease.

Synovial fluid and membranes were studied in two patients with Whipple's disease. In the first, arthrocentesis revealed 28,350 white blood cells/mm3, 96% of which were polymorphonuclear leukocytes. A dense neutrophilic infiltrate, periodic acid-Schiff, positive macrophages and bacilliform structures were present in the synovial membrane. The synovial fluid and membrane findings in the second case, and during intercritical periods in the first case, showed mild to moderate nonspecific inflammation. Since polyarthritis and other systemic manifestations often precede the gastrointestinal features…

Sulphasalazine in rheumatoid arthritis.

Seventy-four patients with rheumatoid arthritis were treated with sulphasalazine. There was a significant improvement in clinical score, with substantial falls in serum C-reactive protein concentrations and erythrocyte sedimentation rate four weeks after starting the drug. Improvement was maintained in the 38 patients who remained on the drug for one year. The mean Rose-Waaler titre did not change. There was little difference between the results in seropositive and seronegative patients. The commonest adverse effect was dyspepsia, but five patients developed a megaloblastic anaemia and one pat…

Lyme arthritis in Wisconsin.

Rash, severe constitutional symptoms, and arthritis developed in three persons who were bitten by ticks in Wisconsin. On comparison with other reports of arthritis related to tick bites, we found that the illness of our patients had clinical features consistent with Lyme arthritis. Lyme arthritis appears not to be restricted to New England as has been previously reported.

Immunopathology of rheumatoid arthritis.

The central place of immunologic aberrations in the pathology of rheumatoid arthritis is now well established. It is the nature and interactions of these aberrations that still remain to be elucidated. Among the many puzzles being studied are the uniquely self-perpetuating nature of rheumatoid arthritis immune complexes and the role of complement and the prostaglandins in joint damage and pain.
