A patient with rheumatoid arthritis (RA) developed an infection caused by Listeria monocytogenes in her left knee and both shoulder joints. The clinical presentation of the disease was rather indolent with relatively moderate joint symptoms. Moreover, the synovial fluid sample was only slightly turbid with a white blood cell count of 23.5 x 10(9)/1. As compared to the earlier reported cases of L. monocytogenes septic arthritis, our patient is unique because she had infection in several joints. The polyarticular joint involvement combined with the clinical symptoms resembling the activation of

Twenty-four cases of septic arthritis in rheumatoid arthritis patients were compared with 99 cases of septic arthritis in patients without rheumatoid arthritis. In addition, 238 previously published cases of septic arthritis with rheumatoid arthritis were analyzed. Fifteen percent of our patients with septic arthritis had rheumatoid arthritis, which was typically of long duration (mean 15 years), erosive, and seropositive. Fifty-four per cent (28% in the literature) and 9% of patients with and without rheumatoid arthritis, respectively, had pyarthrosis of multiple joints. The knee represented

A 60-yr-old woman with a history of seropositive rheumatoid arthritis and using a low dose of corticosteroids presented with a very painful, swollen, red coloured knee with limited range of motion. Physical examination revealed a scratch on her left lower leg without discolouration or induration. Synovial fluid cultures yielded Pasteurella multocida. The pathogenetic significance of this commensal of cats and dogs in man is discussed.

Septic arthritis is associated with a definite morbidity which may be related to a delay in diagnosis and hence treatment. The cases of three patients with rheumatoid arthritis and chronic chest disease where the joint sepsis was not the predominant feature are presented. The responsible organism was Streptococcus pneumoniae which had spread after recent chest infections. Minimal joint symptoms or general malaise in association with an unexplained rise in erythrocyte sedimentation rate in these circumstances warrant a search for joint sepsis.

The chronic Erysipelas-polyarthritis in pigs has been considered an animal model resembling human rheumatoid arthritis. Fifteen specifically pathogenfree (SPF) pigs 45 days old were experimentally infectec with strain T 28 of Erysipelothrix rhusiopathiae-bacteria. During the subsequent 32 weeks several enzymatic, immunohistological and microbiological parameters were monitored. Compared to 5 age and sex matched healthy controls the infected pigs showed increased activity of plasma acid phosphatase starting 4 weeks after the infection. Acid phosphatase activity was usually enhanced in synovial

HL-A27 were found in 31 of 48 patients with Reiter's disease (65%) as compared with 8% of 2 103 health controls. It is suggested that HL-A27 is closely related to the inheritance of a special immune response leading to an increased susceptibility to a special type of reactive arthritis which Reiter's disease has in common with ankylosing spondylitis and a certain type of psoriatic arthritis.

Results of synovianalysis are reported in 129 children, 91 with juvenile rheumatoid arthritis (JRA), 13 with septic arthritis, 12 with enteroarthritis, 12 with acute transient arthritis and 1 with bacillus Calmette-Guerin arthritis. Mononuclear cells were dominant in the patients with oligoarticular JRA (mean 64%, median 74%), and among these, significantly lower proportions of polymorphonuclear (PMN) cells were found in patients with early onset disease with antinuclear antibodies or chronic uveitis than in those with late onset with HLA-B27. PMN cells were dominant in polyarticular and syste

HIV infection is common and a severe problem in Thailand. The clinical features of 100 HIV-infected patients (86 males and 14 females) with musculoskeletal symptoms were studied. Thirty-one patients were known to have HIV infection before developing musculoskeletal symptoms. The musculoskeletal manifestations were undifferentiated (seronegative) arthritis (UA) in 38, bone and joint infections in 30, psoriatic arthritis in 9, AIDS-associated arthritis in 9, Reiter's syndrome in 8, infectious myositis in 3, arthralgia syndrome in 2, and CPPD crystal-associated arthritis in 1 patient. Salmonella

We attempted to induce experimental arthritis in rats by systematically testing the effect of Yersinia infections in five strains of rats, using the intragastric, intraperitoneal, and intravenous routes of inoculation. We observed that Lewis rats which were given 10(4) to 10(5) Yersinia enterocolitica WA organisms via the intravenous route consistently developed arthritis. The arthritis was most severe at 3 weeks and subsided at 6 weeks. No arthritis was observed when this bacterial strain was administered to Buffalo, Fisher, DA, and LDA rats. No replicable bacteria were detected in the joints

Actinomycosis is an unusual and unrecognized cause of spinal cord compression of infectious origin. We report the case of a 57-year-old immunocompetent woman admitted for sub-acute lower limb ataxia. The diagnosis of spinal cord compression secondary to actinomyces infectious arthritis was established. Surgical decompression and long-term antibiotic treatment enabled complete recovery. Data from the literature indicate that actinomycosis is a potential cause of several neurological manifestations. Unusual but treatable, actinomycosis is a potential alternative when the main etiologies have bee

Adult Still's Disease was first described in 1971 by Bywaters in fourteen adult female patients who presented with symptoms indistinguishable from that of classic childhood Still's Disease (Bywaters, 1971). George Still in 1896 first recognized this triad of quotidian (daily) fevers, evanescent rash, and arthritis in children with what later became known as juvenile inflammatory arthritis (Still, 1990). Adult Onset Still's Disease (AOSD) is an inflammatory condition of unknown etiology characterized by an evanescent rash, quotidian fevers, and arthralgias. Numerous infectious agents have been

Helicobacter cinaedi is a rare human pathogen which has various clinical manifestations such as cellulitis, bacteremia, arthritis, meningitis, and infectious endocarditis. We report an abdominal aortic aneurysm infected with Helicobacter cinaedi, treated successfully with surgical repair and long-term antimicrobial therapy.
