Slow speech rate effects on stuttering preschoolers with disordered phonology.

To study the effects of clinicians' slow rate on the speech of children who stutter with and without a concomitant phonological disorder, an A-B-A-B single case design was used with six clinician-child dyads, where B = Clinician's slow speech rate model. Two boys and one girl, aged 49-54 months, stuttering with disordered phonology (S + DP), were compared to three boys aged 42-50 months, stuttering with normal phonology (S + NP). Articulation rates were measured in phones per second (pps) in clinician-child adjacent utterance pairs. The S + NP dyads showed improved fluency in the B condition t

The relationship of vocal tract dimensions and substitution of the palatal approximant /j/ for the alveolar trill /r/.

PURPOSE: To investigate the relationship of the vocal tract dimensions and dental arches in participants with substitution of palatal approximant /j/ for the trill variant of /r/. METHOD: Cephalograms and dental casts of 60 native speakers of Arabic were used for comparison. The experimental group included 30 participants with substitution of /j/ for the trill variant of /r/ (17 males and 13 females, age ranged between 15-20 years, M = 18.7 years, SD = 1.27). The control group included 30 normal speakers (15 males and 15 females, age ranged between 15-20 years, M = 18.3 years, SD = 1.67). Bot

[History and current state of the problem of negative disorders (research of nosological period)].

Pioneers of nosology in psychiatry considered mental deficit as a basic disorder obligated to all clinical forms of illness, as did E. Kraepelin in the concept of dementia praecox and E. Bleuler in the concept of schizophrenia. At the present stage of studies, this position is clearly articulated in the «deficit schizophrenia» concept of J. Klosterkotter. Negative disorders are considered in categorical approach to interpret psychopathological dimensions of schizophrenia as coordinated, strongly bound with positive syndromes (concept of «dialectical unity» of A.V. Snezhnevskiy, 1964) and class

[Diagnosis and management of unilateral temporomandibular ankylosis].

INTRODUCTION: Temporomandibular joint ankylosis is defined as permanent jaw constriction with an interincisal mouth opening of less than 30 … d bilateral coronoidectomy. Additional procedures are performed depending on the presence of dental articulation disorders, age and loss of ramus height. Early, aggressive and prolonged post-operative rehabilitation is essential to ensure that the mouth remains open and that there is no recurrence. CONCLUSION: This complex pathology needs to be well understood in order …

Framing Patellar Instability: From Diagnosis to the Treatment of the First Episode.

The patellofemoral joint (PFJ) is a complex articulation between the patella and the femur which is involved in the extensor mechanism of the knee. Patellofemoral disorders can be classified into objective patellar instability, potential patellar instability, and patellofemoral pain syndrome. Anatomical factors such as trochlear dysplasia, patella alta, and the tibial tuberosity-trochlear groove (TT-TG) distance contribute to instability. Patellofemoral instability can result in various types of dislocations, and the frequency of dislocation can be categorized as recurrent, habitual, or perman

Changes in voice onset time and motor speech skills in children following motor speech therapy: Evidence from /pa/ productions.

This study evaluated changes in motor speech control and inter-gestural coordination for children with speech sound disorders (SSD) subsequent to Prompts for Restructuring Oral and Muscular Phonetic Targets (PROMPT) intervention. We measured the distribution patterns of voice onset time (VOT) for a voiceless stop (/p/) to examine the changes in inter-gestural coordination. Two standardized tests were used (Verbal Motor Production Assessment for Children (VMPAC), GFTA-2) to assess the changes in motor speech skills and articulation. Data showed positive changes in patterns of VOT with a lower p

MR imaging of synovial disorders of the knee.

Synovial membranes line the diarthrodial (movable) joints, bursae, and tendon sheaths of the body. The primary function of this specialized, vascular tissue is to serve as a filter system that lubricates and nourishes the articular structures as well as serving as a shock-absorber. The synovium is affected by a variety of disorders that can be localized to a specific articulation or can be systemic in nature. These include inflammatory, infectious, degenerative, traumatic, or neoplastic categories of disease. Further, MR imaging provides an excellent non-invasive tool for the evaluation of the

The science and practice of LSVT/LOUD: neural plasticity-principled approach to treating individuals with Parkinson disease and other neurological disorders.

Our 15 years of research have generated the first short- and long-term efficacy data for speech treatment (Lee Silverman Voice Treatment; LSVT/LOUD) in Parkinson's disease. We have learned that training the single motor control parameter amplitude (vocal loudness) and recalibration of self-perception of vocal loudness are fundamental elements underlying treatment success. This training requires intensive, high-effort exercise combined with a single, functionally relevant target (loudness) taught across simple to complex speech tasks. We have documented that training vocal loudness results in d

The Adelaide preschool language unit: results of follow-up.

OBJECTIVE: To determine educational, social and behavioural functioning of children who had been involved in a preschool language intervention programme between 1982 and 1990. METHODOLOGY: Fifty children who attended the Unit were available for follow-up. The children were divided into three groups: (i) language disorder; (ii) speech disorder; and (iii) mixed speech and language disorder. A psychologist administered educational, cognitive and social behavioural tests. Speech, language and articulation were assessed by a speech pathologist. RESULTS: Cognitively, the 'mixed' speech and languag

Operant control of pathological tongue thrust in spastic cerebral palsy.

Pathological tongue thrust (reverse swallowing) has been associated with neuromuscular disorders and linked to dental malocclusion, articulation problems, difficulty in eating, and excessive drooling. In this study, observable tongue thrust in a 10-year-old retarded male was modified during mealtime using a procedure consisting of differential reinforcement (presentation of food contingent upon tongue in) and punishment (gently pushing the tongue back into the mouth with a spoon). Results of a reversal design showed substantial decreases in tongue thrust and food expulsion, and a large increas

Clinical classification of patellofemoral pain and dysfunction.

The patellofemoral joint is a complex articulation based on its dependence on both dynamic and static restraints for stability. Classification of patellofemoral disorders has been frought with confusion. However, progress has been made in the classification and understanding of these disorders by improved understanding of the biomechanics of the joint and by clarification of the terminology to describe patellofemoral pathology. The term chondromalacia patella, although once used as an all-inclusive term for anterior knee pain, is now widely accepted as a term used to describe pathologic lesion

Striatal network dysfunction underlying speech impairment in early Parkinson's disease.

The pathophysiology underlying Parkinson's disease (PD) speech impairments and its potential sex differences remain unclear. We investigated sex-specific alterations in speech and striato-cortical networks in 104 patients with early PD (68M/36F), and 44 healthy controls (23M/21F), using quantitative speech markers and multimodal neuroimaging. Speech features related to prosody, phonation, articulation, fluency, and rhythm, were extracted from voice recordings; striatal dopaminergic loss was quantified using DaT-SPECT, and functional connectivity between striatal and cortical language networks

Hypoglossal Nerve Neuropathies-Analysis of Causes and Anatomical Background.

The hypoglossal nerve is the last, and often neglected, cranial nerve. It is mainly responsible for motor innervation of the tongue and therefore the process of chewing and articulation. However, tumors, aneurysms, dissections, trauma, and various iatrogenic factors such as complications after surgeries, radiotherapy, or airway management can result in dysfunction. Correct differential diagnosis and suitable treatment require a thorough knowledge of the anatomical background of the region. This review presents the broad spectrum of hypoglossal neuropathies, paying particular attention to these

Developmental dyslexia, learning and the cerebellum.

Theoretical frameworks for dyslexia must explain how the well-established phonological deficits and the literacy deficits arise. Our longstanding research programme has led to a distinctive 'twin level' framework that proposes, first, that the core deficits are well described in terms of poor skill automaticity. Second, these 'cognitive level' symptoms are attributed to abnormal cerebellar function--a 'brain-level' analysis. The evidence includes data from behavioural, imaging, neuroanatomical and learning studies. The frame-work leads to an 'ontogenetic' analysis that links cerebellar deficit

Anterior approach versus posterior approach to surgical treatment of children's supracondylar fractures: comparative study of thirty cases in each series.

The purpose of this study was to consider the surgical treatment of severe supracondylar fractures of the elbow in children, and to compare the anterior approach with the posterior approach used in two homogeneous groups of 30 cases each by two experienced surgeons. Control procedures were maintained with the children of both groups when the plaster was removed, during the fourth month after surgery, and throughout the follow-up that continued for more than 1 year. A posterior approach to surgery is simpler than an anterior approach, but it creates supplementary anatomic damage that can cause

Genetics of speech and language disorders.

Vocal communication mediated by speech and language is a uniquely human trait, and has served an important evolutionary role in the development of our species. Deficits in speech and language functions can be of numerous types, including aphasia, stuttering, articulation disorders, verbal dyspraxia, and specific language impairment; language deficits are also related to dyslexia. Most communication disorders are prominent in children, where they are common. A number of these disorders have been shown to cluster in families, suggesting that genetic factors are involved, but their etiology at th…

Voice disorders in children with oral motor dysfunction: perceptual evaluation pre and post oral motor therapy.

A few clinical investigations of children's voices have suggested a relation between voice deviation and oral motor and sensory dysfunction. This motivated the present retrospective study of the occurrence of voice disorders in a group of children with oral motor problems. A further aim was to evaluate the effect of an oral motor and articulatory treatment programme upon voice function in this patient group. Subjects were 38 children with oral motor deficits who had received and finished therapy at the clinic. The recordings of a picture naming task, before onset of therapy and at therapy endi
