BACKGROUND: In case of suspicious CTG in labor a conservative attitude is recommended, when possible, through intrauterine resuscitation maneuvers.
AIMS: This study aims to evaluate the use of intrauterine resuscitation maneuvers (IRM) at two Italian hospitals; the secondary outcome is the assessment of the agreement in their application.
MATERIALS AND METHODS: Retrospective analysis was performed on the data of 80 deliveries (40 vs 40) from two different Italian hospitals, located in Novara (group 1) and in Borgomanero (group 2). In order to evaluate a varied series of CTG traces (normal an

Kidney dysfunction and acute kidney injury (AKI) frequently accompanies neonatal encephalopathy and contributes to neonatal morbidity and mortality. While there are currently no proven therapies for the treatment of AKI, understanding the pathophysiology along with early recognition and treatment of alterations in fluid, electrolyte and metabolic homeostasis that accompany AKI offer opportunity to reduce associated morbidity. Promising new tests and technologies, including urine and serum biomarkers and renal near-infrared spectroscopy offer opportunities to improve diagnosis and monitoring of

BACKGROUND: Therapeutic hypothermia (TH) is routinely provided to those with moderate or severe neonatal encephalopathy (NE). Subtle differences exist in the standardized exams used to define NE severity. We aimed to assess if an infant's TH eligibility status differed if they were evaluated using either the NICHD/Neonatal Research Network's (NICHD-NRN) or TOBY/British Association of Perinatal Medicine's (TOBY-BAPM) neurological exam.
METHODS: Encephalopathic infants ≥36 weeks with evidence of perinatal asphyxia and complete documentation of the neurological exam <6 h of age were included.

BACKGROUND: It is challenging to decrease neonatal mortality in middle-income countries, where perinatal asphyxia is an important cause of death. This study aims to analyze the annual trend of neonatal mortality with perinatal asphyxia according to gestational age in São Paulo State, Brazil, during a 10-year period and to verify demographic, maternal and neonatal characteristics associated with these deaths.
METHODS: Population-based study of neonatal deaths associated with perinatal asphyxia from 0 to 27 days in São Paulo State, Brazil, from 2004 to 2013. Perinatal asphyxia was considered as

Achieving the targets of the Every Newborn Action Plan by the year 2030 will require accelerating the current reduction in neonatal mortality. Educational programs addressing the three major causes of neonatal death - intrapartum-related events (asphyxia), prematurity and small size at birth, and infection - have the potential to significantly reduce preventable mortality. Helping Babies Breathe is an example of an educational program that not only has given health care providers around the world access to current resuscitation science but has changed provider behavior and patient outcomes and

Heart rate variability (HRV) is currently considered the most valuable non-invasive test to investigate the autonomic nervous system function, based on the fact that fast fluctuations might specifically reflect changes of sympathetic and vagal activity. An association between abnormal values of HRV and brain impairment has been reported in the perinatal period, although data are still fragmentary. Considering such association, HRV has been suggested as a possible marker of brain damage also in case of hypoxic-ischemic encephalopathy following perinatal asphyxia. The aim of the present manuscri

OBJECTIVES: To describe the diagnostic test properties of Cardiac Troponin-T (cTnT) in predicting myocardial dysfunction in asphyxiated term neonates by taking echocardiography as the gold standard and to establish the optimum cut-off values of cTnT for myocardial dysfunction, shock, severe hypoxic ischemic encephalopathy (HIE) and mortality by receiver operator characteristic (ROC) curve analysis.
METHODS: This was a prospective study based on diagnostic test evaluation. The study included 120 term asphyxiated neonates in a tertiary care neonatal intensive care unit (NICU) in Southern India

Hypoxic ischemic encephalopathy is a major complication of perinatal asphyxia, with high morbidity, mortality and neurologic sequelae as cerebral palsy, mostly in poor or developing countries. The difficulty in the diagnosis and management of newborns in these countries is astonishing, thus resulting in unreliable data on this pathology and bad outcomes regarding mortality and incidence of neurologic sequelae. The objective of this article is to present a new clinical diagnostic score to be started in the delivery room and to guide the therapeutic approach, in order to improve these results.

Perinatal hypoxic-ischemic brain injury is a major health problem. Adjuvant treatments that improve the neuroprotective effect of the current treatment, therapeutic hypothermia, are urgently needed. The growing knowledge about the complex pathophysiology of hypoxia-ischemia (HI) has led to the discovery of several important targets for neuroprotection. Early interventions should focus on the preservation of energy metabolism, the reduction of glutamate excitotoxicity and oxidative stress, the maintenance of calcium homeostasis, and the prevention of apoptosis. Delayed interventions should prom

AIM: The Helping Babies Breathe (HBB) programme is known to decrease neonatal mortality in low-resource settings but gaps in care still exist. This study describes the use of quality improvement to sustain gains in birth asphyxia-related mortality after HBB.
METHODS: Tenwek Hospital, a rural referral hospital in Kenya, identified high rates of birth asphyxia (BA). They developed a goal to decrease the suspected hypoxic-ischaemic encephalopathy (SHIE) rate by 50% within six months after HBB. Rapid cycles of change were used to test interventions including training, retention and engagement for

In this column, Sara Wickham takes a sideways look at issues relevant to midwives, students, women and families, inviting us to sit down with a cup of tea and ponder what we think we know. A recent paper on apparent life-threatening events (ALTEs) in newborn babies brought to mind an experience from practice, the cause of which remains a mystery. As many similar events are unexplained, is it acceptable that there is a tendency in the literature to claim that skin-to-skin contact and breastfeeding are risk factors for ALTEs?

Perinatal asphyxia is a clinical condition which results from oxygen deprivation of the fetus or newborn and the breakdown of perfusion in various organs. The aim of this study was to evaluate and compare troponin T levels over time as a marker of cardiac injury in cases of perinatal asphyxia and healthy newborns. The study included a total of 30 newborns diagnosed with perinatal asphyxia with a gestational age of 32-41 weeks, based on the last menstruation date, and 30 healthy newborns with a gestational age of 34-40 weeks, as the control group. Levels of troponin T and creatinin kinase MB we

The article reports on the exhumation and subsequent DNA analysis of the skeletal remains of an unknown male newborn from 1988. Molecular biological methods confirmed the maternity of a woman who was already convicted of infanticide. Since homicide could not be clearly proven and manslaughter becomes barred by the statute of limitation after 20 years, the woman cannot be held accountable for the alleged killing of her first child.

Proteomics and metabolomics are emerging in recent years as one of the most challenging topics in neonatology. They are characterized by a large amount of data that reflect the complexity of all biological systems more accurately than traditional methods utilized in clinical chemistry. In this review paper we present the modifications of the salivary proteome, which represents an easy and non-invasive method that offers the opportunity to investigate changes in the metabolism of preterm infants and in pediatric patients. Moreover, we present the metabolomics-histologic correlations in newborn
