Incidence, Prevention, and Management of Periprocedural Complications of Left Atrial Appendage Occlusion.

Major procedural complications related to left atrial appendage occlusion (LAAO) are relatively infrequent but may be associated with major morbidity and mortality. LAAO operators should be knowledgeable about these potential complications. Prompt recognition and treatment are necessary to avoid rapid deterioration and dire consequences. With stringent guidelines on operator training, competency requirements, and procedural-technical refinements, LAAO can be performed safely with low complication rates. This article focuses on commonly used devices, as well as prevention, treatment, and manage…

Percutaneous Management of Left Atrial Appendage Perforation: Keep Calm and Think Fast.

Left atrial appendage (LAA) perforation is a possible complication not only after release of the closure device, but also during the diagnostic phase due to sheath positioning in the LAA. We present an 83-year-old woman with permanent atrial fibrillation and high thromboembolic and bleeding risk who was admitted for elective percutaneous LAA closure. During angiographic study, she suddenly became hypotensive. Heart perforation with leakage of contrast in the pericardial space was evident and imaging confirmed cardiac tamponade. Rapid release of the closure device and pericardial evacuation all…

Left atrial appendage closure device implantation in patients with prior intracranial hemorrhage.

BACKGROUND: The Watchman device (Boston Scientific), used for left atrial appendage closure (LAAC), was approved for stroke prevention in patients with atrial fibrillation (AF) and an appropriate rationale to avoid long-term oral anticoagulation. Patients with AF and prior intracranial hemorrhage (ICH) were excluded from clinical trials because of perceived risks of perioperative anticoagulation. OBJECTIVE: The purpose of this study was to study the efficacy and safety of LAAC using Watchman in patients with AF and prior ICH. METHODS: In a multidisciplinary AF/stroke prevention clinic, 38 co…

Device-Related Thrombus After Left Atrial Appendage Closure: Incidence, Predictors, and Outcomes.

BACKGROUND: In patients with atrial fibrillation, left atrial appendage closure with the Watchman device prevents thromboembolism from the left atrial appendage; however, thrombus may form on the left atrial face of the device, and then potentially embolize. Herein, we studied the incidence, predictors, and clinical outcome of device-related thrombus (DRT) using a large series of clinical trial cohorts of patients undergoing Watchman implantation. METHODS: We studied the device arms of 4 prospective Food and Drug Administration trials: PROTECT-AF (Watchman Left Atrial Appendage System for Emb…

Left atrial appendage pericardial fluid: contrast-enhanced transesophageal echocardiography makes it visible.

BACKGROUND: Contrast-enhanced transesophageal echocardiography (TEE) for the opacification of left atrial appendage (LAA) is not clearly suggested by the current guidelines and only scarce data are available. CASE PRESENTATION: A patient with a membrane-like structure in the LAA cavity is presented. Contrast-enhanced TEE revealed LAA pericardial effusion and this diagnosis was confirmed by cardiac magnetic resonance that offered comparable images and tissue characterization. CONCLUSION: Contrast-enhanced TEE has been demonstrated to safely and accurately enlighten LAA anatomy. Hippokratia 20…

Impact of transcatheter aortic valve implantation on left atrial appendage flow velocities.

PURPOSE: Left atrial appendage (LAA) flow velocity has not been extensively studied in patients with severe aortic stenosis (AS) undergoing transcatheter aortic valve implantation (TAVI). The aim of this study was to assess the impact of TAVI on LAA flow velocity. METHODS: Medical records of consecutive TAVI recipients were reviewed retrospectively. Patients with persistent atrial fibrillation were excluded. LAA velocities were measured before and after TAVI by transesophageal echocardiography. RESULTS: Sixty-one patients were included. Mean LAA emptying (EV) and filling (FV) flow velocity b…

Cardiac CT angiography for device surveillance after endovascular left atrial appendage closure.

AIMS: Left atrial appendage (LAA) device imaging after endovascular closure is important to assess for device thrombus, residual leak, positioning, surrounding structures, and pericardial effusion. Cardiac CT angiography (CCTA) is well suited to assess these non-invasively. METHODS AND RESULTS: We report our consecutive series of non-valvular atrial fibrillation patients who underwent CCTA post-LAA closure with Amplatzer Cardiac Plug (ACP), Amulet (second generation ACP), or WATCHMAN devices. Patients underwent CCTA typically 1-6 months post-implantation. Prospective cardiac-gated CCTA was pe…

Thromboembolism Prevention via Transcatheter Left Atrial Appendage Closure with Transeosophageal Echocardiography Guidance.

Atrial fibrillation (AF) is an independent risk factor for stroke. Anticoagulation therapy has a risk of intracerebral hemorrhage. The use of percutaneous left atrial appendage (LAA) closure devices is an alternative to anticoagulation therapy. Echocardiography has a leading role in LAA closure procedure in patient selection, during the procedure and during followup. A comprehensive echocardiography study is necessary preprocedural in order to identify all the lobes of the LAA, evaluate the size of the LAA ostium, look for thrombus or spontaneous echo contrast, and evaluate atrial anatomy, inc…

Left atrial appendage function in patients with mitral stenosis in sinus rhythm.

AIMS: Left atrial appendage thrombi are believed to be the source of embolism in patients with rheumatic mitral stenosis in atrial fibrillation. There are a few studies which search the effects of left atrial appendage dysfunction in patients with mitral stenosis in sinus rhythm. METHODS AND RESULTS: Left atrial appendage function and flow patterns in 41 patients with rheumatic mitral stenosis in sinus rhythm and 11 healthy subjects were studied by transoesophageal echocardiography. Left atrial appendage flow profiles were recorded within the proximal third of the appendage. The left atrial a…

Influence of acutely altered loading conditions on left atrial appendage flow velocities.

OBJECTIVES: The purpose of this study was to identify the effects of altered loading conditions on left atrial appendage flow velocities. BACKGROUND: Although studies have suggested that Doppler analysis of left atrial appendage blood flow may have clinical utility, the hemodynamic and cardiac mechanical determinants of left atrial appendage flow are poorly understood. METHODS: Transesophageal Doppler echocardiography was performed in eight atrially paced anesthetized dogs instrumented with sonomicrometers on the left atrial appendage and the left ventricular minor axis and with left atrial …

Left atrial appendage occlusion for stroke prevention in atrial fibrillation: recent evidence, catheter ablation, and future trials.

INTRODUCTION: Oral anticoagulation (OAC) remains the standard for stroke prevention in atrial fibrillation (AF), but bleeding, non-persistence and thromboembolism despite treatment create important unmet needs. Catheter ablation and left atrial appendage occlusion (LAAO) address distinct mechanisms and do not reproduce systemic anticoagulation. AREAS COVERED: This narrative review covers randomized anticoagulation-withdrawal studies after ablation, trials and pooled evidence on LAAO, patients unable to sustain anticoagulation, procedural imaging and healing, post-implant antithrombotic therap…

Left Atrial Appendage Occlusion Among Cancer Patients with Atrial Fibrillation: a systematic review and update meta-analysis.

INTRODUCTION: Left atrial appendage occlusion (LAAO) is a well-established therapy for atrial fibrillation (AF) in patients with contraindications to oral anticoagulation. However, the clinical outcomes of LAAO in patients with cancer remain poorly defined. Therefore, we performed an updated meta-analysis to evaluate the safety and efficacy of LAAO in patients with cancer. METHODS: PubMed, Embase and Cochrane databases were systematically searched for studies evaluating LAAO in patients with AF and cancer. Our endpoints of interest were long-term stroke, bleeding and all-cause mortality. We r…

Left atrial appendage closure after a hospital admission for gastrointestinal bleeding in patients anticoagulated for atrial fibrillation.

BACKGROUND: There are no "real world" data on the actual impact of percutaneous left atrial appendage closure (LAAC) programs in the management of thromboembolic prevention after a hospital admission for gastrointestinal bleeding (GIB) in anticoagulated patients with atrial fibrillation (AF). We evaluated the utilization, predictors, and clinical outcomes of LAAC following GIB-related hospitalizations. METHODS: The multicenter PERSEO registry retrospectively enrolled consecutive anticoagulated AF patients discharged alive after a GIB (2021-2022) across all public Andalusian hospitals with LAA…

Updated Meta-Analysis of Left Atrial Appendage Closure Versus Oral Anticoagulation to Prevent Stroke in Atrial Fibrillation.

BACKGROUND: Ninety percent of left atrial thrombi in nonvalvular atrial fibrillation arise in the left atrial appendage, which can be occluded percutaneously (left atrial appendage closure [LAAC]). Two 2026 trials (CHAMPION-AF and CLOSURE-AF) more than doubled the available randomized evidence comparing LAAC with oral anticoagulation. We evaluated pooled effects of LAAC on stroke, bleeding, and mortality by bleeding risk, anticoagulation eligibility, and comparator. METHODS: In this systematic review and meta-analysis, we searched PubMed, Scopus, and Embase from inception to April 15, 2026, f…

Cryoballoon-Based Left Atrial Appendage Isolation and Closure in Patients with Atrial Fibrillation-The LALALAND Pilot Study.

Background: Atrial fibrillation (AF) remains the most common cardiac arrhythmia, with pulmonary vein isolation (PVI) established as the cornerstone of interventional treatment. However, in patients with persistent AF (PersAF), the success rates of PVI alone tend to be limited. A promising additional target is the left atrial appendage (LAA). In recent years, cryoballoon (CB) technology has become a tool for achieving durable PVI. Its application for LAAI has been investigated as a potentially advantageous alternative to radiofrequency ablation, and a positive effect on long-term outcome has be…
