In cases of out-of hospital cardiac arrest (OHCA), falling to the ground can cause brain and neck trauma to the patient. We present a case of a man in his mid-60s who suffered from an OHCA resulting in a violent collapse. The patient received immediate cardiopulmonary resuscitation, but his spine was immobilised only after a large frontal haematoma was found. The resuscitation efforts resulted in return of spontaneous circulation and discharge from hospital. After this, doctors performed angioplasty, followed by a cardiopulmonary bypass. Later, CT scan examination reported a displaced and unst

OBJECTIVE: To assess the progression of radiographic sacroiliitis in a cohort of patients with early axial spondyloarthritis over a period of 2 years and to explore predictors of progression.
METHODS: 210 patients with axial spondyloarthritis from the German Spondyloarthritis Inception Cohort have been selected for this analysis based on availability of radiographs at baseline and after 2 years of follow-up. Radiographs were centrally digitised and the sacroiliac joints were scored independently according to the grading system of the modified New York criteria for ankylosing spondylitis (AS)

A 29-year-old male presented with symptoms of acute severe neck pain with no other neurological symptoms. He had noticed lymph node swelling with pain in his neck about 2 months before admission and had been treated by the transoral administration of antibiotics for about 10 days. His neck was protected with a neck collar and an evaluation of any possible neck lesion was performed. Plain X-ray film and CT scans of the cervical spine showed an anterior subluxation of the atlas and an anterior declination and erosion of the dens. The MRI findings showed long T1 and T2 values in many parts of the

The axial skeleton develops from the sclerotome, a mesenchymal cell mass derived from the ventral halves of the somites, segmentally repeated units located on either side of the neural tube. Cells from the medial part of the sclerotome form the axial perichondral tube, which gives rise to vertebral bodies and intervertebral discs; the lateral regions of the sclerotome will form the vertebral arches and ribs. Mesenchymal sclerotome cells condense and differentiate into chondrocytes to form a cartilaginous pre-skeleton that is later replaced by bone tissue. Uncx4.1 is a paired type homeodomain t

Thirty-nine consecutive patients, 22 male and 17 female with an average age of 37.6 years, with traumatic spondylolisthesis of the axis were reviewed. The cause of injury in 75% of the patients was a road traffic accident. The fractures were classified according to Effendi et al., the type II fractures were further divided into three subgroups: flexion, extension and listhesis injuries. There were 10 type I (25.7%) and 29 type II fractures (74.4%); of these, 12 (30.8%) were classified as flexion-type, 2 (5.1%) as extension-type and 15 (38.5%) as listhesis-type. We did not identify any case of

Head trauma and vigorous physical activity were followed by delayed-onset posterior circulation stroke in a 5-year-old boy with odontoid aplasia and C1-C2 subluxation. Angiography demonstrated vertebral artery narrowing and occlusion of the left anterior-inferior cerebellar and posterior cerebral arteries. Odontoid aplasia and other atlantoaxial dislocations are treatable causes of posterior circulation insufficiency in childhood; these defects may be overlooked without appropriate radiographic study of the cervical spine using flexion-extension views.

Posttraumatic spinal cord cysts have traditionally been diagnosed late. The condition is usually considered after the neurological sequelae of spinal cord trauma have not resolved. The diagnosis has usually required invasive radiographic techniques, or even surgical exploration. Magnetic resonance imaging is a new, non-invasive diagnostic technique that appears to be more accurate than previous procedures. Moreover, it has the ability to delineate spinal cord parenchymal lesions much earlier than other methods. We present a case report and literature review of posttraumatic spinal cord cyst.

One hundred and seventy consecutive multiple trauma victims were studied postmortem by radiography and direct inspection at autopsy for the presence of cervical spine injuries. Of the total 38 fractures and/or dislocations, eight had bilateral pedicle fractures of the axis. Only atlanto-occipital dislocations were more frequently encountered. Dissection of six of the eight spines with hangman's fractures showed variable osseous and ligamentous injuries. In only two of the eight cases were significant ligamentous structures sufficiently intact to prevent gross displacement on flexion and extens

PURPOSE: Cervical pedicle screws (CPSs), though associated with complications and steep learning curve, have significantly increased strength and stability as compared to any other posterior instrumentation methods. Using anatomical referral techniques, pedicle screws can be inserted safely with a high accuracy rate obviating the need for anterior stabilization. Our present study aims to investigate the safety and outcomes of lateral vertebral notch (LVN) referred entry point for subaxial CPSs by freehand technique.
MATERIALS AND METHODS: We retrospectively studied 22 patients who underwent C

BACKGROUND: For posterior fixation of C2 vertebra (the axis), several fixation techniques such as pedicle screw, laminar screw, and pars screw have been reported. A pedicle screw (PS) is considered the strongest among the techniques, but certain situations make PS fixation impossible or difficult. These include patients with a narrow C2 pedicle or high-riding vertebral artery. We introduced an alternative screw technique for C2 that addressed the aforementioned problems with safely placing it, which we termed a cortical screw (CS) for C2.
METHODS: Among a total of 28 cases using C2 CS for hig

OBJECTIVE: This retrospective cohort study aimed to develop a formal predictive model distinguishing between symptomatic and asymptomatic distal junctional kyphosis (DJK). In this study the authors identified a DJK rate of 32.2%. Predictive models were created that can be used with high reliability to help distinguish between severe symptomatic DJK and mild asymptomatic DJK through the use of surgical factors, radiographic parameters, and patient variables.
METHODS: Patients with cervical deformity (CD) were stratified into asymptomatic and symptomatic DJK groups. Symptomatic: 1) DJK angle (D

BACKGROUND CONTEXT: Instability of the atlantoaxial spine is a recognized problem in children. Safe passage of pedicle screws at C2 poses challenges because of the proximity to the vertebral artery, size of the pedicles, and variations in the location of the foramen transversarium.
PURPOSE: The C2 translaminar technique is a useful option and its stability is comparable to that offered by C2 pedicle screws. In this follow-up from our previously published study, we wanted to verify the safety and suitability of the C2 laminar screw in the treatment of cervical instability in the pediatric popu

This study investigates the clinical and radiographic characteristics of complex axis fractures with adjacent segment instability and describes the outcome of surgical treatment. Twenty-one patients (14 male, seven female; mean age=34 years) with complex axis fractures and adjacent segment instability who were treated between August 2003 and June 2009 were retrospectively reviewed. Treatment selection was based on fracture type and stability of the upper cervical segments. All patients were immobilized with a hard collar for three months after surgery. The mean follow-up period was 12 months (

STUDY DESIGN: In vitro biomechanical flexibility experiment studying 5 sequential conditions.
OBJECTIVE: To determine the biomechanical differences among 3 fixation techniques after a simulated hangman's fracture.
SUMMARY OF BACKGROUND DATA: Type II hangman's fractures are often treated surgically with a C2-C3 anterior cervical discectomy, fusion, and plating. Other techniques include direct fixation with C2 pars interarticularis screws or posterior C2-C3 fixation connecting C2 pars screws to C3 lateral mass screws.
METHODS: Seven cadaveric specimens (Oc-C4) were tested intact, after a simu
