Blindness from late presenting undiagnosed pancraniosynostosis mimicking pseudotumor cerebri.

We report the unique case of late-onset pancraniosynostosis presenting with rapid visual deterioration, without other symptoms of increased intracranial pressure. A 10-year-old girl had episodes of blurry vision for 1 month. Magnetic resonance imaging (MRI) demonstrated a borderline Chiari I malformation. Ophthalmologic exam confirmed papilledema and decreased visual acuity. Opening pressure on lumbar puncture was 55 mmH2O. The patient was diagnosed with pseudotumor cerebri and prescribed Diamox. Four days later, she re-presented with worsening vision and increased papilledema and was taken fo

GNAT1 associated with autosomal recessive congenital stationary night blindness.

PURPOSE: Congenital stationary night blindness is a nonprogressive retinal disorder manifesting as impaired night vision and is generally associated with other ocular symptoms, such as nystagmus, myopia, and strabismus. This study was conducted to further investigate the genetic basis of CSNB in a consanguineous Pakistani family. METHODS: A consanguineous family with multiple individuals manifesting cardinal symptoms of congenital stationary night blindness was ascertained. All family members underwent detailed ophthalmic examination, including fundus photographic examination and electroretin…

Blindness following severe midfacial trauma--case report and review.

PURPOSE: Severe trauma of the viscerocranium or neurocranium may result in impaired visual acuity or even blindness. Case based epidemiology, pathomechanism and actual strategies in midfacial trauma for initial therapy and prevention of posttraumatic blindness are discussed. CASE AND REVIEW: A 58-year old patient was treated at our Department of Oral and Maxillofacial Plastic Surgery after his central midface had been hit by a swinging steel girder. Initially he was blind on both eyes. Initial treatment started by applying 24 mg of dexamethasone and omeprazole. During the following 2 weeks, a…

Causes of blindness and visual impairment in Latin America.

We review what is known in each country of the Latin American region with regards to blindness and visual impairment and make some comparisons to Hispanic populations in the United States. Prevalence of blindness varied from 1.1% in Argentina to 4.1% in Guatemala in people 50 years of age and older, with the major cause being cataract. Diabetic retinopathy and glaucoma are starting to make serious inroads, although epidemiological data are limited, and age-related macular degeneration is now a concern in some populations. Infectious diseases such as trachoma and onchocerciasis are quickly dimi…

Keratoconus associated with congenital stationary night blindness type 1.

A 35-year-old man presented with keratoconus; his best corrected visual acuities were -18.00/+10.00 ×180 (6/60) oculus dexter and -10.00/+8.00 ×5 (6/36) oculus sinister. Bilateral steep central corneal thinning, paracentral ectasia and Vogts striae were present. Normal fundi. Corneal topography disclosed 7.4 dioptres of irregular astigmatism in the central 3 mm with thinning (335 μm). Electroretinography (ERG) showed no response. There were no medical or environmental influences for his keratoconus. Occurrence of keratoconus and congenital stationary night blindness (CSNB) in the patient may r…

Abnormal diffusion of cerebral white matter in early blindness.

Early visual deprivation may lead to both abnormal and plastic changes in the visual and other systems of the brain. Such secondary changes in the gray matter of the early blind have been well studied, but not so well in the cerebral white matter whose subtle changes may be revealed by diffusion tensor imaging. The first purpose of this study is to explore the possible changed white matter regions of the early blind in whole brain manners, using voxel-based analysis (VBA) and tract-based spatial statistics (TBSS) methods. The second purpose is to investigate the changes of diffusion eigenvalue

Blindness due to Acanthamoeba: first case report from Sudan.

Acanthamoeba is a free-living protozoan widely distributed in soil, water, and air. It can cause corneal infection (keratitis) in man, and may lead to blindness of the affected eye in 15% of untreated cases. The main risk factors are eye trauma and contact lens wearing. Early presentation of Acanthamoeba keratitis patients usually includes ocular pain, photophobia, and a unilateral red eye. The story of a female Sudanese housewife who suffered a blind left eye due to Acanthamoeba is described in this report.

[Night blindness and xerophthalmia after surgery for morbid obesity].

CASE REPORT: We describe ocular findings due to a vitamin A deficiency in a 50-year-old man. The patient had undergone intestinal bypass surgery two years before. After therapy with oral vitamin A the symptoms improved. DISCUSSION: The incidence of morbid obesity is increasing throughout much of the developed world, with intestinal bypass surgery the treatment of choice for most people with the condition. This type of surgery can lead to a vitamin A deficiency, with remarkable ophthalmological consequences, which without correct treatment may ultimately cause blindness. For this reason, the p…

Trachoma: global magnitude of a preventable cause of blindness.

OBJECTIVES: Trachoma is the leading cause of infectious blindness worldwide. It is known to be highly correlated with poverty, limited access to healthcare services and water. In 2003, the WHO estimated that 84 million people were suffering from active trachoma, and 7.6 million were severely visually impaired or blind as a result of trachoma: this study provides an updated estimate of the global prevalence of trachoma based on the most recent information available. METHODS: A literature search of recent published and unpublished surveys in the 57 endemic countries was carried out: the result …

Early blindness results in abnormal corticocortical and thalamocortical connections.

Studies in congenitally blind and bilaterally enucleated individuals show that an early loss of sensory driven activity can lead to massive functional reorganization. However, the anatomical substrate for this functional reorganization is unknown. In the present study, we examined patterns of corticocortical and thalamocortical connections in adult opossums that had been bilaterally enucleated neonatally, prior to the formation of retinogeniculate and geniculocortical connections. We show that in addition to normal thalamocortical projection patterns from visual nuclei, enucleated animals also

[Dual cause of blindness: chorioretinitis sclopetaria and homonymous hemianopsia].

CLINICAL CASE: We describe the case of a man who, after a gunshot wound to the right facial region, exhibited chorioretinitis sclopetaria of the right eye and a contralateral homonymous hemianopsia. DISCUSSION: Chorioretinitis sclopetaria is a rare entity resulting from an ocular injury caused by a bullet passing through the orbit. It is not common to find the condition associated with a damaged visual field caused by the subsequent path traversed and lodging of that bullet in the brain.

Human infection patterns and heterogeneous exposure in river blindness.

Here we analyze patterns of human infection with Onchocerca volvulus (the cause of river blindness) in different continents and ecologies. In contrast with some geohelminths and schistosome parasites whose worm burdens typically exhibit a humped pattern with host age, patterns of O. volvulus infection vary markedly with locality. To test the hypothesis that such differences are partly due to heterogeneity in exposure to vector bites, we develop an age- and sex-structured model for intensity of infection, with parasite regulation within humans and vectors. The model is fitted to microfilarial d…

Unusual presentation of brucellosis in a child: acute blindness.

UNLABELLED: In brucellosis, visual impairment due to optic nerve involvement is rare, and acute onset visual loss is an unusual presenting feature. We report a 15-y-old girl who had pancytopenia and who was admitted to our hospital with acute onset of bilateral blindness and fever. There was no light perception, and anterior segment and fundus examination were normal in both eyes. No other abnormal neurological findings were detected. Increased latencies and decreased amplitudes were found in visual evoked potentials. Cranial MR and CT revealed no abnormality. Blood culture was found to be pos…

Cyclosporin-induced cortical blindness in a patient with dermatomyositis.

Cyclosporin A (CsA) has various adverse effects including neurotoxicity. We report the case of a 49-year-old Japanese woman with dermatomyositis who showed CsA-induced cortical blindness. The patient demonstrated cortical blindness despite having a normal blood concentration of CsA. The risk factors of CsA-induced cortical blindness include the use of corticosteroids and vascular injury, which are frequently observed in patients with systemic autoimmune diseases. Clinicians should consider CsA neurotoxicity when using CsA for patients with systemic autoimmune diseases.
