Bilateral femoral retroversion associated with acetabular dysplasia. A case report.

Retroversion of the proximal femur is associated with a number of acquired conditions but is unusual in a congenital form. It is even more unusual to be associated with acetabular dysplasia. A 38-year-old woman with bilateral hip pain had roentgenographic evidence of acetabular dysplasia with valgus neck-shaft angles. Physical findings were consistent with femoral retroversion, and computed tomography demonstrated 28 degrees of retroversion on both sides. Symptomatic relief was obtained with bilateral varus internal rotation osteotomies of the proximal femur. To date, a case of retroverted fem

Extrinsic fetal akinesia and skeletal development: a study in oligohydramnios sequence.

Long-bone morphometry and cephalometry were performed in 13 newborns with oligohydramnios sequence (OS) in order to establish whether or not skeletal changes existed in extrinsic fetal akinesia similar to those observed in the fetal akinesia deformative sequence (FADS) (i.e., hypoplastic long bones and micrognathia). Oligohydramnios sequence was caused by bilateral renal agenesis in five cases and obstructive uropathy in eight cases. Twenty-one stillborns and newborns who had died from conditions other than renal anomalies or congenital malformations were used as controls. Normal longitudinal

Microspherophakia-metaphyseal dysplasia: a 'new' dominantly inherited bone dysplasia with severe eye involvement.

We report a father and son affected by a hitherto unpublished bone dysplasia with moderately severe dwarfism. On initial radiographs, thickening of the diaphyses of the long bones was striking. The small bones of the extremities were almost unaffected. With age, the metaphyseal deformation became more prominent. The epiphyses became irregular and their growth was delayed (particularly the femoral heads). The femoral neck showed an unusual 'lip' on the inner edge. Later, the stubby appearance of the long bones faded and, in adulthood, only enlarged metaphyses and deformed femoral necks persiste

[Coxa vara. Isolated growth of the greater trochanter. Prevention-treatment].

Prevention of avascular complications is a primary aim. The ischemic insult to the femoral head provokes different types of morphologic deformities depending on its location. When the lateral part of the growth plate is affected, the head will be in valgus with a short neck, on the contrary, when the medial part is affected, a coxa vara occurs. The sooner the growth is stopped, the shorter the neck will be. Nevertheless, the greater trochanter will continue its growth and under certain conditions, will extend beyond the head. The Articulo-Trochanteric Distance is a measurement of the deformity

Distal tibial physeal arrest: a complication of meningococcal septicemia.

Two cases of meningococcal septicemia are presented. Distal tibial physeal arrest occurred in both cases due to hypotension and disseminated intravascular coagulation. To our knowledge, this is the first report of distal tibial physeal arrest with resultant varus deformity at the ankle subsequent to meningococcal septicemia.

The radiographic differential diagnosis of juvenile chronic arthritis.

In conclusion, this short communication has emphasized that the well known radiographic features of juvenile chronic arthritis are by no means pathognomonic of this entity. A large differential diagnosis must be considered, including haemophilia, idiopathic multicentric osteolysis, the mucopolysaccharidoses, a variety of skeletal dysplasias, infection, and synovial haemangioma. Correlation of radiographic observations with clinical findings will permit the establishment of an accurate and specific diagnosis in most instances, underscoring the need for close communication between the paediatric

The Opitz syndrome: a new designation for the clinically indistinguishable BBB and G syndromes.

The BBB and G syndromes are multiple congenital anomaly (MCA) syndromes characterized by a developmental defect of the midline field. Prominent clinical manifestations are hypertelorism and, in males, hypospadias. Transmission is most likely autosomal dominant in both syndromes. Examination of two new cases and scrutiny of the literature led us to conclude that there are no discriminating qualitative differences between the two conditions. Therefore we propose that they both be designated by the common term "Opitz syndrome."

Wedge-shaped epiphyses of the knees in two siblings: a new recessive rare dysplasia?

A peculiar form of metaphyseal dysplasia, mainly of the lower limbs, occurred in two male siblings born to healthy, unrelated parents. The clinical and radiological features were short stature, psychomotor retardation, accelerated bone maturation, the limbs and especially the knees showing cup-shaped widening of the ends of the metaphyses and wedge-shaped widening of the epiphyses. This condition does not fit the description of any syndrome reported so far and may therefore be classified as a new recessive dysplasia.

An unusual case of metaphyseal chondrodysplasia with an abnormal perilacunar matrix associated with agranulocytosis and hypoplasia of the thymus.

We report herein an unusual skeletal dysplasia in a 6-month-old boy characterized by metaphyseal dysplasia associated with agranulocytosis and hypoplasia of the thymus. A radiological survey revealed generalized metaphyseal abnormalities showing widening and increased density. Pathological changes in the chondro-osseous tissues were unusual and distinctive. There was widespread evidence of abnormal chondrocytes with an abnormal perilacunar matrix containing a PAS-positive, diastase resistant substance. Chondrocyte maturation and regular columnar arrangement were absent in most growth plates wi

Weismann-Netter-Stuhl syndrome: toxopachyostéose diaphysaire tibio-péronière.

Three cases of Weismann-Netter-Stuhl syndrome in nonrelated patients are reported. The classic feature of the syndrome is a congenital anterior bowing of the middiaphyseal part of both tibiae and fibulae. The authors' experience and review of 37 cases reported since its original description in 1954 has revealed a definite association of diaphyseal bowing of other long bones and squaring of the pelvis. The association of mental retardation, goiter, and anemia previously described are felt to be coincidental. At present, this syndrome seems to be some sort of diaphyseal dysplasia, which may be f

Congenital bowing of the long bones. A review and phenotype analysis of 13 undiagnosed cases.

Phenotype analysis of 13 patients with congenital bowing of long bones and otherwise undiagnosable conditions allowed sorting into three major groups. Patients in group 1 had normal bone texture; bowing was confined to the femora, the long bones were relatively thin, there were no epiphyseal or metaphyseal abnormalities, and associated malformations or CNS abnormalities were common. Patients in group 2 had osteopenia; bowing was more generalized, the long bones were relatively thick, there were metaphyseal ossification abnormalities. Two brothers belonged to a third group with normal bone text

[Pancreatic lipomatous hypoplasia associated with skeleton anomalies. Shwachman's syndrome (author's transl)].

A five month old girl with exocrine pancreatic insufficiency, skeletal deformities and a non constant neutropenia (Shwachman's syndrome) is presented. Infectious complications, mainly septic and pulmonary, which caused her death, were frequent. The pathologic findings showed a pancreatic lipomatous hypoplasia and absence of zymogen granulos, as well as anomalies of the growing cartilage where no radiological abnormalities could be seen.

[Precocious puberty associated with Silver's syndrome].

Precocious puberty has been reported as an important feature of the Silver-Russell syndrome. The present case refers to a boy entered puberty at the chronological age of 11 years and the bone age of 6 years 6/12. Criteria for precocious puberty in this syndrome are discussed by comparison with previously published cases, and it is concluded that abnormalities in clinical pubertal development are quite unusual in this syndrome.
