BACKGROUND: Brain-computer interfaces (BCIs) have emerged as powerful tools to restore neurological function lost due to injury or degeneration. Despite scientific advancements, successful commercialization remains challenging. This article analyzes two rehabilitative BCIs—the Argus II retinal prosthesis by Second Sight and the ARC-IM spinal cord stimulation system by Onward Medical—as case studies to highlight critical factors influencing their commercialization outcomes. MAIN TEXT: The Argus II, initially heralded as groundbreaking, restored partial vision to patients blinded by retinitis pi…

Accumulating evidence suggests that neurons are key regulators of skeletal tissues. A recent study by Chen et al. demonstrated that aged brain neurons export WD repeat and FYVE domain containing 1 (WDFY1) to bone via extracellular vesicles. After uptake, WDFY1 engages in retromer-dependent endosome-to-Golgi recycling, shifting bone stem cell fate from osteogenesis toward adipogenesis, revealing a novel mechanism whereby brain aging regulates bone aging.

Mechanisms directing meningeal lymphatic development have remained largely unexplored. Tavares et al.1 identify macrophage-mediated sculpting as a developmental program that establishes cerebrospinal fluid drainage pathways, revealing how disrupted lymphatic maturation may contribute to neurodevelopmental disorders.

Brain macrophages are not uniform responders to neurodegeneration, but anatomically specialized populations whose functions depend on niche, developmental history, and regulatory state. The goal of this chapter is to discuss how parenchymal microglia and brain border associated macrophages as components of a distributed macrophage network that operates across the central nervous system. Microglia are embedded within the parenchyma and are central to surveillance, synaptic remodeling, myelin maintenance and tissue repair, whereas BAMs occupy vascular, meningeal, and choroid plexus interfaces wh…

In animals, the central nervous system (CNS) plays a crucial role as the primary organ responsible for integrating environmental information and coordinating the responses of the organism. To protect its intricate and delicate structure, the CNS is enclosed within the rigid confines of the skull, an effective defence against physical trauma. However, threats to the brain also arise internally, in the form of toxins, pathogens, or activated immune cells. This has driven the evolution of specialised vascular adaptations that tightly control exchanges between the blood and the CNS. These 'blood b…

This study provides evidence of lymphatic vessels in the mouse brain, specifically in the cortex, thalamus, and hippocampus. Using confocal microscopy, Western blotting, and real-time PCR, lymphatic vessels were identified by the expression of Lyve1, Prox1, and VEGFC, alongside endothelial markers CD31 and CD34. Paraffin-embedded brain slices from wild-type Balb/C mice were stained with antibodies against Lyve1, CD31, and CD34, revealing lymphatic vessels through 2D and 3D imaging. A coiled 3D structure of lymphatic vessels was observed in the hippocampus, indicating a complex drainage network…

The prevalence of brain tumors in patients with headache is very low; however, 48% to 71% of patients with brain tumors experience headache. The clinical presentation of headache in brain tumors varies according to age; intracranial pressure; tumor location, type, and progression; headache history; and treatment. Brain tumor-associated headaches can be caused by local and distant traction on pain-sensitive cranial structures, mass effect caused by the enlarging tumor and cerebral edema, infarction, hemorrhage, hydrocephalus, and tumor secretion. This article reviews the current findings relate…

A 31-year-old man, with no past medical history, presented with headaches, sudden loss of vision, right exophtalmia, bilateral papilledema, and fever. Brain imaging noted a right basi-temporal lesion. Excision of the lesion was performed. The histological examination noted a glial tissue with acute inflammatory changes and multinucleated giant cells. Within this infiltrate there were septate and branched hyphae consistent with aspergillosis. These filaments were stained with PAS. The patient died post-operatively.

The presence of pregnancy in a brain-dead woman is a rare circumstance. We present a case of a 31-year-old woman who was 22 weeks pregnant at the time of diagnosis of brain death after intracranial and subarachnoid hemorrhage. After a multidisciplinary approach, the decision was made to continue somatic support to maintain the pregnancy until optimal fetus viability. Cesarean section was performed after 11 weeks (33 weeks gestational age) of brain-death diagnosis with a successful delivery of a live infant. Management of brain-death complications during pregnancy is described.

We present a previously well woman in her 70s who was admitted for 2 weeks of progressively worsening abdominal pain, high fever and drowsiness. She was eventually diagnosed with Clostridium septicum brain abscess, meningoencephalitis and ventriculitis. The diagnosis was challenging as cerebrospinal fluid cultures were negative and a microbiological diagnosis was only obtained on brain biopsy. Despite early initiation of antibiotics that would have been effective against C. septicum, her central nervous system (CNS) infection progressed, and she eventually succumbed to the infection. Infection…

BACKGROUND: The term 'brain fog' is increasingly used colloquially to describe difficulties in the cognitive realm. But what is brain fog? What sort of experiences do people talk about when they talk about brain fog? And, in turn, what might this tell us about potential underlying pathophysiological mechanisms? This study examined first-person descriptions in order to better understand the phenomenology of brain fog.
METHODS: Posts containing 'brain fog' were scraped from the social media platform Reddit, using python, over a week in October 2021. We examined descriptions of brain fog, themes…

A therapeutic effect of vagus nerve stimulation has been reported for a wide range of neurological, medical and psychiatric conditions. New research provides evidence that this effect results from extensive increase of physiological arousal and brain activation.

The global prevalence of neurologic disorders is rising, and yet we are still unable to deliver most drug molecules, in therapeutic quantities, to the brain. The blood brain barrier consists of a tight layer of endothelial cells surrounded by astrocyte foot processes, and these anatomic features constitute a significant barrier to drug transport from the blood to the brain. One way to bypass the blood brain barrier and thus treat diseases of the brain is to use the nasal route of administration and deposit drugs at the olfactory region of the nares, from where they travel to the brain via mech…
