Cognitive rehabilitation for executive dysfunction in adults with stroke or other adult non-progressive acquired brain damage.

BACKGROUND: Executive functions are the controlling mechanisms of the brain and include the processes of planning, initiation, organisation, inhibition, problem solving, self monitoring and error correction. They are essential for goal-oriented behaviour and responding to new and novel situations. A high number of people with acquired brain injury, including around 75% of stroke survivors, will experience executive dysfunction. Executive dysfunction reduces capacity to regain independence in activities of daily living (ADL), particularly when alternative movement strategies are necessary to co

Low-dose nitroglycerine improves outcome after cardiac arrest in rats.

OBJECTIVE: The aim of this study was to evaluate the outcome of intravenously applied nitroglycerine (NTG, 1μgkg(-1)min(-1) for 1h) after resuscitation from an asphyxia cardiac arrest (ACA) insult. We hypothesized that NTG infused for 1h after the return of spontaneous circulation (ROSC) would improve functional and neuro-morphological outcomes. METHODS: Adult rats were subjected to 8min of ACA followed by resuscitation. There were three treatment groups: ACA, ACA+NTG and sham operated. Vital and blood parameters were monitored during the 1h post-resuscitation intensive care phase. After surv

Hyperoxia in very preterm infants: a systematic review of the literature.

Supplemental oxygen plays a critical role in the management of infants born at the lower limit of viability, but not without the risk of complications resulting from high levels or prolonged exposure. Longitudinal studies of very premature infants, born at less than 28 weeks' gestation, establish a clear relationship between pulse oximetry saturation readings above 92%, or hyperoxia, and development of severe retinopathy of prematurity, chronic lung disease, and brain injury. Hyperoxia is neither natural nor random. It is an unintended consequence of intervention. A comprehensive review of the

Hermann Zingerle's "Impaired perception of the own body due to organic brain disorders". 1913. An introductory comment, and an abridged translation.

In the late 1970's Edoardo Bisiach and his coworkers provided definitive evidence that spatial unilateral neglect involves a disorder of the internal representation of extra-personal space. A few years later Bisiach and Berti (1987) revived the so far neglected contribution of an Austrian neurologist, Hermann Zingerle (1913). Ninety years ago Zingerle had described the symptom-complex of two right-brain-damaged patients, who showed left hemisomatoagnosia, unawareness of left hemiplegia, and left motor neglect. In addition to the detailed case reports, Zingerle had put forward a unitary interpr

[Etiology of cerebral palsy].

The "perinatal asphyxia" is regarded to be one of the causes of cerebral palsy, though in the very most of the children with cerebral palsy there is found no hypoxia during labour. It should be mentioned, that the definition of "perinatal" and "asphyxia" neither are unic nor concret. And also there is no correlation between nonreassuring fetal heart rate patterns and acidosis in fetal blood with the incidence of cerebral palsy. Numerous studies in pregnant animals failed in proving an acute intrapartal hypoxia to be the origin of the cerebral palsy. Myers (1975) describes four patterns of anat

The effect of retrograde cerebral perfusion after particulate embolization to the brain.

Neurologic injury as a consequence of cerebral embolism of either air or atherosclerotic debris during cardiac or aortic surgery is still a major cause of postoperative morbidity and mortality. While exploring various means of improving cerebral protection during complex cardiothoracic procedures, we have developed a chronic porcine model to study retrograde cerebral perfusion. We have previously demonstrated that retrograde perfusion results in a small amount of nutritive flow and provides cerebral protection that appears to be superior to simple prolonged hypothermic circulatory arrest. The

Selective impairment in manipulating Arabic numerals.

This paper describes an acalculic patient (B.A.L.) with an unusual selective deficit in manipulating arabic numerals. The patient was unimpaired in reading aloud letters, words and written number names but unable to read aloud single arabic numerals. Furthermore, his ability to produce the next number in the sequence and his ability to produce answers to simple addition and subtraction was relatively spared when the stimuli were presented as number names but impaired when the stimuli were presented as arabic numerals. Using magnitude comparison tasks it was demonstrated that his knowledge of c

Nucleated red blood cells: a marker for fetal asphyxia?

OBJECTIVE: Our purpose was to determine whether a relationship exists between the presence of nucleated red blood cells, hypoxic ischemic encephalopathy, and long-term neonatal neurologic impairment. STUDY DESIGN: Nucleated red blood cell data from 46 singleton term neurologically impaired neonates were compared with cord blood nucleated red blood cells of 83 term nonasphyxiated newborns. The neurologically impaired neonates group was also separated as follows: nonreactive, nonreactive fetal heart rate from admission to delivery; tachycardia, reactive fetal heart rate on admission followed by

The social misperception syndrome in children with learning disabilities: social causes versus neurological variables.

Family shortcomings or other intrinsic psychological deficits are frequently viewed as contributing factors to the social misperceptiveness frequently seen in children with learning disabilities. This article attempts to synthesize research defining the social problems of some children with learning disabilities. Particularly emphasized is the role of communication skills deficits. Breakthroughs in neurophysiology negate, to a large extent, traditional emphases on family dysfunction, school failure, or personality distrubances as primary causes of the social problems of some children with LD.

Early orientation of attention toward the half space ipsilateral to the lesion in patients with unilateral brain damage.

Posner has suggested that unilateral spatial neglect could be due to a difficulty in disengaging attention from its current focus to orient it toward the neglected half space. Clinical and experimental data suggest, however, that this disengaging difficulty could be only one aspect of a more complex disturbance also characterized by an early automatic orienting of attention toward the half space ipsilateral to the lesion. To test this hypothesis, two different investigations in unselected groups of patients with right and left brain-damage were carried out. The first investigation, to evaluate

A study on nesidioblastosis in hyperinsulinemic hypoglycemia--diagnosis, treatment, and neurologic sequelae.

The medical records of six cases of nesidioblastosis were examined to determine the diagnostic approach, treatment, and neurologic sequelae. All six patients were male, and their ages at the onset of the disease ranged from one day to six months (mean 3.36 +/- 2.5 mo.). Initial clinical features were seizure, cyanosis, poor feeding, and apnea. Other subsequent symptoms were developmental delay, hyperactivity, and cold sweating. The Birth weight of the neonatal onset group was heavier than the postneonatal onset group (4.4 +/- 0.3 vs 3.26 +/- 0.04 kg). Before the diagnosis of hyperinsulinism, s

Variation in the pattern of omissions and substitutions of grammatical morphemes in the spontaneous speech of so-called agrammatic patients.

We describe the patterns of omissions (and substitutions) of freestanding grammatical morphemes and the patterns of substitutions of bound grammatical morphemes in 20 so-called agrammatic patients. Extreme variation was observed in the patterns of omissions and substitutions of grammatical morphemes, both in terms of the distribution of errors for different grammatical morphemes as well as in terms of the distribution of omissions versus substitutions. Results are discussed in the context of current debates concerning the possibility of a theoretically motivated distinction between the clinica

Idiosyncratic strategies in sentence production: a case report.

In their case study of a patient with selective apraxia of phonation, Marshall, Gandour, and Windsor (1988, Brain and Language, 35, 313-339) reported that many of his utterances were ill-formed syntactically. In this note, a detailed syntactic analysis of his speech is presented. Although the patient's syntax is deviant, it is seen to result from the application of certain consistent, identifiable, compensatory strategies. Moreover, it is shown that the particular, abnormal syntactic structures are internally consistent with his phonatory apraxia coupled with the normal rules of English prosod

Effect of emotional context on bucco-facial apraxia.

Patients with left- and right-hemisphere cerebrovascular pathology and normal adult controls were videotaped while executing tasks of bucco-facial praxis in emotional and nonemotional conditions. Each practic movement was assessed for accuracy and motor execution. Left-brain-damaged patients were significantly impaired on these tasks relative to right-damaged patients and controls. When emotional context was provided, apractic performance improved significantly.

Digit repetition in brain-damaged adults: clinical and theoretical implications.

Digit repetition was investigated in samples of unilaterally (N = 87) and bilaterally (N = 75) brain-damaged adults. The study was designed to investigate the hemispheric and neuropsychological factors that underlie performance on these two dissimilar tasks. Digit repetition was disproportionately depressed in such patients, especially those with left hemisphere lesions. The incidence of individually impaired digit repetition performance, especially of digits backward, was significantly higher than in normals, again particularly within the left hemisphere sample. However, impaired digit repeti

Ventricular shapes, distortions, and deformities: mirrors of past cerebral insults. A study based on early sonographic follow-up studies.

Ventricular distortions and deformities often represent the endpoint of neuropathologic reactions that evolve early in life. Once fully developed, they may offer no clues as to preceding etiologic agents or pathogenesis. Serial sonography in the neonatal period of 14 patients with periventricular leukomalacia provided documentation of the evolution of certain ventricular patterns. When recognized early, these ventricular changes have prognostic significance because periventricular leukomalacia can produce serious neurologic sequelae. If found later in childhood, periventricular leukomalacia ca
