Ultrasonographic and electrophysiological adjuncts to surgery within the brain stem: technical note.

The surgical approach to intrinsic lesions within the brain stem is undertaken by the surgeon knowing the potential for significant operative morbidity. We report the use of real-time intraoperative ultrasound and electrophysiological mapping techniques to aid in the localization and resection of an intrinsic brain-stem tumor in a child. These techniques permitted an aggressive surgical approach without encountering adverse postoperative neurological sequelae.

["Palatolaryngeal hemiplegia" in transient brain stem ischemia--a contribution to neurogenic dysphagia].

A 65-year old man suffering from dysphagia with aspiration was examined. ENT examination showed a Horner syndrome and cranial nerve palsy with paralysis of the soft palate and one vocal cord (palatolaryngeal hemiplegia, Avellis' syndrome). Pharyngeal manometry and videofluoroscopy depicted an asynergic swallowing with cricopharyngeal achalasia. CT scans of mediastinum, head, neck, and skull base showed no signs of abnormality. MR imaging of the brain stem demonstrated an enrichment of contrast medium in the dorsal region of the upper medulla oblongata in the level of the centre of the glossoph…

Using the derived auditory brain stem response to estimate traveling wave velocity.

An estimate of the traveling wave velocity (TWV) was evaluated in 18 young adults to determine the technique's applicability as a possible tool in evaluating Meniere's disease. This TWV estimation uses a derived auditory brain stem response procedure requiring high-intensity masking. The results indicate that on average, TWVs were similar to those obtained via psychophysical measures or electrocochleography. However, instances of undermasking, due to subjects' intolerance, resulted in a number of subjects exhibiting faster than expected TWVs.

[Cavernous malformation of the brain stem: clinical symptom and its surgical indication].

Clinical features and surgical indication of cavernous malformation (CVM) of the brain stem are reviewed, based on the analysis of five personally encountered cases and nineteen previously reported cases. Nineteen out of these twenty four cases presented sudden onset of neurological deficits suggesting brain stem hemorrhage. Although the majority of those patients with acute onset improved gradually within a couple of weeks, all cases, except for six cases treated with radical removal following their first bleeding episodes, presented recurrent hemorrhages, and one third of them died due to th…

Conductive hearing loss during infancy: effects on later auditory brain stem electrophysiology.

Long-term effects on auditory electrophysiology from early fluctuating hearing loss were studied in 27 children, aged 5 to 7 years, who had been evaluated originally in infancy. For controls (Group A), infant auditory brain stem responses (ABRs) were normal from birth to age 2 years. A second group (Group B) had intermittent conductive hearing loss. A third group (Group C) had more abnormal ABRs during infancy than Group B and more severe and frequent conductive hearing loss. For this follow-up study, all children had normal peripheral hearing at test. ABRs were obtained to monaural and binaur…

Slowly progressive facial paralysis due to vascular malformation of the brain stem.

Slowly progressive facial paralysis is suggestive of a diagnosis other than Bell's palsy. A case of slowly progressive facial paralysis caused by a cavernous angioma of the brain stem is presented. The classification of vascular malformations of the central nervous system is described with emphasis on the MRI appearance of these lesions.

Effect of broadband noise on the human brain stem auditory evoked response.

The purpose of this paper is to describe the effect of broadband continuous noise on brain stem auditory evoked responses elicited from normal-hearing and hearing-impaired individuals. The motivation for this study derives from the increasing use of noise masking paradigms in diagnostic electrophysiology, the universal presence of background noise in everyday listening environments, and the frequent observation that background noise is more detrimental to the performance of the hearing-impaired individual than to the normal-hearing individual. Four studies were designed to evaluate: (1) the se…

Characterization of cat nasal afferents and brain stem neurones receiving ethmoidal input.

Stimulation of the nasal mucous membrane can initiate protective reflexes, particularly sneezing and apnea. Very little is known about the r … es of ethmoidal afferents in the trigeminal ganglion; and to study the second-order neurones in the brain stem on which these afferents make contact. Results of single fiber recording indicated that, in addition to being tactile, approximately half of the neurones studied also responded to noxious chemical stimuli. The conduction velocity of the majority of …

[A mild form of brain stem encephalitis due to herpes simplex virus].

A 10-year-old boy had gait and speech disturbances 17 days after the initial symptoms of a fever, headache and cough. Four days later he was admitted to a hospital with mild disturbances of gait, speech, writing, visual acuity, left facial nerve, nystagmus and consciousness. Impairments of cranial nerves (II, III and VII), pyramidal sign and cerebellar sign were noticed. EEG showed generalized slow waves. Auditory brain stem response showed prolongation of the interval between I and V waves and poor differentiation between them. Brain CT could not find any abnormalities. Brain stem encephaliti…

[Superficial temporal to superior cerebellar artery anastomosis for rostral brain stem infarction].

The authors report a case of superficial temporal to superior cerebellar artery anastomosis (STA-SCA anastomosis) for progressing rostral brain stem infarction with an excellent result. Precise operative techniques were also described. A 47-year-old male was admitted to our hospital on November 9, 1984, because of sudden onset of dysarthria and ataxic gait. CT revealed a low density area in the pons. Left vertebral angiogram showed occlusion of the left vertebral artery just distal to the origin of the posterior inferior cerebellar artery (PICA). Arterial branch of the left cerebellar hemisphe…

Correlates of brain-stem oculomotor disorders in multiple sclerosis. Magnetic resonance imaging.

Three patients with focal brain-stem oculomotor disturbances (nuclear sixth nerve syndrome, sixth nerve palsy, bilateral internuclear ophthalmoplegia) as a consequence of multiple sclerosis have been studied with high-volume delayed computed tomography and high-field magnetic resonance imaging. In all of them, high-volume delayed computed tomography was inconclusive in the brain stem, but magnetic resonance imaging showed an area of prolonged T1 and/or T2 in the region appropriate to the oculomotor findings. Magnetic resonance imaging is the imaging technique of choice of small plaques in the …

[Large listerial abscess of the brain stem. Favorable effect of antibiotic therapy].

A 52-year-old woman developed headache with fever followed after several days by a left hemiplegia, paralysis of the right IIIrd, Vth, and V … esses, which can develop in previously healthy subjects. The clinical picture is that of a solitary brain stem abscess with a fatal outcome whatever the nature of the germ. Van Gilder, Allen and Lesser (1974) published the first report of a case that recovered after surgical drainage. The present case is the only one of the 6 cases reported in the literature …

[Pathophysiological aspects of the brain stem in closed head injuries (author's transl)].

In a case of severe head injury, there is a disturbance of the functional cycle between hypothalamus/mesencephalon and the cortex cerebri. In this article, the causes and the pathophysiological, functional disturbances of primary and secondary unconsciousness will be discussed. In a case of a posttraumatic intracranial hypertension, the following causes are to be considered: cerebral oedema, intracerebral haematomas and the so-called pneumatocephalus: the collection of air in the ventricle system when open head injuries of the base of the skull occur. The midbrain syndrome which is caused by t
